O início sexual cada vez mais cedo propicia alta vulnerabilidade da adolescente a problemas da esfera sexual/reprodutiva, incluindo o câncer de colo uterino e a infecção pelo HPV. O presente estudo teve como objetivo identificar o conhecimento, atitude e prática na prevenção do câncer de colo uterino e infecção pelo HPV na população adolescente e avaliar as situações que as tornam vulneráveis. Trata-se de estudo transversal realizado em uma escola pública de São Paulo com 134 adolescentes entre 14 e 19 anos. Verificou-se idade de iniciação sexual aos 14,8 anos em média. Grande parte das adolescentes não apresentou conhecimento adequado sobre a prevenção desta neoplasia. A adesão ao Papanicolaou também se mostrou baixa. As estatísticas justificam a inserção da adolescente nos programas de detecção deste câncer. É preciso haver investimentos na educação sexual nas instituições de ensino e associar campanhas de Papanicolaou com atividades educativas, com enfoque adequado e linguagem apropriada.
Problema: A emergência da pandemia de Covid-19 desafiou a Atenção Primária à Saúde a modificar seus processos de trabalho para mitigar sua ocorrência, sem abandonar as ações programáticas que já vinham desenvolvendo. Método: Trata-se de um relato de experiência sobre a reorganização da Atenção Primária à Saúde (APS) para enfrentamento da pandemia da Covid-19, por meio da descrição das ações e estratégias adotadas pelo município de Diadema (SP). Resultados: Apresenta: rotinas e linhas de cuidados prioritárias; visitas domiciliares e ações no território; atendimento à demanda espontânea; fluxo de atendimento para usuários com Síndrome Gripal; imunização e campanha de vacinação da gripe; vigilância dos casos; Saúde Bucal; ações do Núcleo Ampliado de Saúde da Família e Atenção Básica e os procedimentos de enfermagem. Sintetiza o eixo operacional, as ações para o enfrentamento do Covid-19 e os desafios enfrentados. Desde o início da pandemia até janeiro de 2021, a APS foi o maior notificador municipal, responsável por 58% das notificações de casos suspeitos ou confirmados de Covid-19. Conclusão: A pandemia reforça a necessidade de defesa do protagonismo da APS para a alocação de recursos financeiros, otimizando gastos e organizando fluxos para reduzir gastos desnecessários com internações hospitalares, tanto pela Covid-19, quanto pelas demais causas sensíveis à APS.
Introdução: Pressionadas pela demanda e por agendas lotadas, equipes de saúde têm discutido estratégias de reorganização da agenda e ampliação do acesso. O Acesso Avançado vem ganhando espaço nesta discussão como uma estratégia desenvolvida nos Estados Unidos, que tem como princípio “Faça hoje o trabalho de hoje!”. Propôs-se descrever a influência da implantação do Acesso Avançado sobre acesso ao atendimento de usuários numa Unidade de Saúde da Família no município de Diadema. Métodos: Tratou-se de um relato de experiência da gerente da unidade no processo de implantação do Acesso Avançado, com a proposta de analisar as agendas e os relatórios de produção dos profissionais de saúde (médicos, enfermeiros e técnicos de enfermagem) no período de janeiro de 2017 a abril de 2018 e apresentar estatística descritiva da influência do Acesso Avançado sobre os atendimentos desses profissionais. O Acesso Avançado foi amplamente discutido com a equipe e com o conselho gestor, num processo de construção conjunta de fluxos e novas agendas. Resultados: Identificou-se aumento de 157% no número de usuários atendidos em consultas, passando de 1.048 em janeiro de 2017 para 2.694 atendimentos em abril de 2018, e queda de 13% para 2% na taxa de faltas dos usuários às consultas. Conclusão: Pôde-se demonstrar que a estratégia foi efetiva na ampliação do acesso ao serviço de saúde, gerando também efeitos positivos na organização da agenda e dos processos de trabalho das equipes. Espera-se contribuir e fomentar discussões acerca da otimização do acesso à Atenção Primária à Saúde (APS), suas complexidades e peculiaridades, e propor análise da influência do Acesso Avançado sobre os demais atributos da APS, particularmente nos atributos de longitudinalidade e integralidade do cuidado.
Background and Objectives: The aim of this retrospective cohort study was to search individual, sociodemographic and environmental predictors of COVID-19 outcomes. Materials and Methods: A convenience sample of 1036 COVID-19 confirmed patients (3–99 years, mean 59 years; 482 females) who sought treatment at the emergency units of the public health system of Diadema (Brazil; March–October 2020) was included. Primary data were collected from medical records: sex, age, occupation/education, onset of symptoms, presence of chronic diseases/treatment and outcome (death and non-death). Secondary socioeconomic and environmental data were provided by the Department of Health. Results: The mean time spent between COVID-19 symptom onset and admission to the health system was 7.4 days. Principal component analysis summarized secondary sociodemographic data, and a Poisson regression model showed that the time between symptom onset and health system admission was higher for younger people and those from the least advantaged regions (availability of electricity, a sewage network, a water supply and garbage collection). A multiple logistic regression model showed an association of age (OR = 1.08; 1.05–1.1), diabetes (OR = 1.9; 1.1–3.4) and obesity (OR = 2.9; 1.1–7.6) with death outcome, while hypertension and sex showed no significant association. Conclusion: The identification of vulnerable groups may help the development of health strategies for the prevention and treatment of COVID-19.
