Lymphoepithelial cyst of the oral cavity is a rare cystic lesion that presents as an asymptomatic, well-circumscribed, yellowish small submucosal nodule covered by normal overlying mucosa, usually located in the floor of the mouth or in the ventral or posterolateral surface of the tongue. Histopathological examination reveals a cyst lined by a stratified squamous epithelium surrounded by lymphoid tissue. In this paper, we report two cases of oral lymphoepithelial cyst, one on the anterior floor of the mouth and another on the posterior lateral tongue. In both cases, the patients were women aged approximately 50 years and the lesions were treated by local surgical excision, with no evidence of recurrence after 2 years of follow-up.
Os autores apresentam o primeiro levantamento estatístico (biênio 2005/2007) do Ambulatório de Estomatologia do Hospital Geral de Curitiba (HGeC). Além do atendimento ambulatorial, a equipe examina pacientes no leito, e participa de campanhas de prevenção do Câncer Bucal durante as Ações Cívico-Sociais do Exército Brasileiro.Nesse período foram atendidos 60 pacientes, com média de idade de 51 anos, 42 do sexo feminino e 18 do sexo masculino. Realizaram 39 biópsias. A partir da análise dos prontuários e dos laudos dos exames histopatológicos, avaliou-se a prevalência das lesões bucais de tecidos moles. As alterações observadas foram classificadas como: 1)processos proliferativos não neoplásicos -26,6% (16/60), 2) lesões pigmentadas (negras) -6,6% (4/60), 3)doenças infecciosas (estomatite protética) -10,0% (6/60); 4)tumores benignos (tecido mole) -6,6% (4/60), 5) glândulas salivares (mucocele) -8,3%(5/60), 6)lesões e condições cancerizáveis -25,0% (15/60) e 7) outras -16,6% (10/60). Os resultados demonstraram um predomínio das lesões proliferativas não neoplásicas e também lesões e condições cancerizáveis. Lesões raras como cisto linfoepitelial bucal e fibromatose gengival hereditária também foram diagnosticadas. Concluem que maiores cuidados no diagnóstico preciso das lesões bucais são necessários, considerando-se o alto número de lesões proliferativas relacionadas freqüentemente a iatrogenias, além da importância do diagnóstico precoce do câncer bucal.
O papilomavírus (HPV) é um DNA vírus que infecta ceratinócitos da pele ou das mucosas, sendo freqüente na região ano-genital e raro na mucosa oral. Sua implantação oral pode ser por auto-inoculação ou pelo contato oro-sexual. Em lesões bucais o HPV-6 e HPV-11 têm sido associados a um baixo risco de malignidade, enquanto o HPV-16 e HPV-18 são relacionados a neoplasias malignas. Várias lesões bucais são associadas ao HPV. Dentre as benignas, citam-se: papiloma, verruga vulgar, condiloma acuminado e hiperplasia epitelial focal. Em relação às malignas, destacam-se os carcinomas epidermóide e verrucoso. O diagnóstico de infecção por HPV é baseado em aspectos clínicos e histopatológicos, como hiperceratose, coilocitose, disceratose, papilomatose e acantose. O papiloma oral é uma proliferação benigna do epitélio escamoso estratificado, sendo os locais mais comumente afetados a língua e o palato mole. A lesão apresenta-se normalmente como um nódulo flácido, exofítico, pedunculado, com numerosas projeções semelhantes a dedos na superfície, que lhe dão aparência de couve-flor. Podem apresentar coloração branca, vermelho claro ou semelhante a da mucosa normal, dependendo da ceratinização da superfície. Neste painel serão apresentados diferentes aspectos clínicos do papiloma oral e seu tratamento, auxiliando o cirurgião-dentista no diagnóstico e resolução clínica desta lesão.
A fibromatose gengival hereditária (FGH) é uma condição bucal rara (1:750.000), de herança autossômica dominante ou recessiva, caracterizada clinicamente pelo crescimento gengival contínuo e progressivo, indolor, não hemorrágico, que pode recobrir total ou parcialmente as coroas dos dentes. Sem predileção por sexo, pode variar a sua expressão de leve a grave em indivíduos de uma mesma família. O aumento gengival é proveniente de hiperplasia não inflamatória dos componentes do tecido conjuntivo, os eventos moleculares que levam a esse aumento ainda não são totalmente conhecidos. De acordo com a literatura consultada, a presença do dente é necessária para que a condição ocorra. Este painel descreve dois casos afetando irmãos, um menino de 6 anos e uma menina de 15 anos, da cidade de Curitiba, Paraná. Ambos apresentavam FGH generalizada, com dois terços das coroas recobertos por gengiva fibrótica. A mãe relatou ter sido portadora da forma grave da doença. O tratamento instituído foi uma associação de gengivectomia e gengivoplastia por quadrantes, possibilitando a realização dos procedimentos necessários. Amostras foram enviadas para exame anatomopatológico, confirmando o diagnóstico clínico de FGH. A família recebeu orientações sobre o caráter familial da doença e vem sendo acompanhada periodicamente, devido à tendência de recidivas.
As diversas modificações na fisiologia da gestante, aliadas à enorme quantidade de medicamentos disponíveis, exigem dos profissionais de saúde constante atualização. Em nossa clínica diária, muitas vezes necessitamos administrar ou prescrever medicamentos para gestantes, com o objetivo de controlar a dor e a infecção. Entre os principais medicamentos utilizados em odontologia estão os anestésicos locais. De acordo com Malamed (2004), um bom agente anestésico deve apresentar baixa toxicidade sistêmica, não ser irritante aos tecidos e também não causar lesão permanente às estruturas nervosas. O tempo para início da anestesia deve ser o mais curto possível e a duração de ação suficiente para a realização do procedimento, com ação reversível. Buscamos neste trabalho, abordar os cuidados especiais que a paciente grávida merece e tecer considerações sobre os anestésicos locais e vasoconstritores indicados e contra-indicados durante a gravidez e também dos principais fármacos prescritos para o controle da dor e infecção. Serão enfatizados aspectos relativos à farmacologia dos medicamentos, e seus principais efeitos indesejáveis, auxiliando o clínico na escolha adequada da droga com o objetivo de minimizar os riscos para gestante e para o futuro bebê.
