Our case shows the rectus anterior direct head may be involved in the etiology of coxa saltans. Theoretically any thickening of the tendon might activate the same pathomechanism. Physicians should consider this possible new cause of internal snapping hip when the most common ones have been excluded, especially as it may be managed easily with steroid injection.
IntroduzioneI processi degenerativi della caviglia possono interessare la componente osteoarticolare, le strutture tendinee e capsulo-legamentose o entrambe [1][2][3][4]. Le cause di un'artropatia degenerativa possono essere molteplici (Tab. 1). Tra tutti questi, i fattori estrinseci traumatici rappresentano, senza dubbio, i principali responsabili di un processo degenerativo a livello dell'articolazione tibio-tarsica. Radiologia tradizionaleL'iter diagnostico parte dall'esame radiografico con le due proiezioni standard A-P e laterale, eventualmente completato con le proiezioni oblique. Per valutare la congruenza della pinza malleolare è utile la proiezione "per il mortaio", che si ottiene intraruotando il piede di 20-25°; in tal modo si ottiene un riallineamento del malleolo peroneale con quello mediale e in queste condizioni la troclea astragalica è equidistante rispetto ai malleoli. Le alterazioni anatomo-patologiche che si verificano in corso di artrosi sono ben visibili all'esame radiografico tradizionale (Tab. 2, Fig. 1). Pertanto, già nello studio radiologico tradizionale, è possibile identificare e caratterizzare un processo degenerativo artrosico. Inoltre l'identificazione di segni specifici permette anche una valutazione della natura del processo. Per esempio, nell'artropatia neuropatica (articolazione di Charcot), già all'esame radiografico si evidenzia facilmente una maggior distruzione articolare, con sclerosi, frammentazione, collasso articolare e sublussazione (Fig. 2). La presenza di osteofiti isolati sulla superficie anteriore della tibia, nel collo dell'astragalo o in entrambe è espressione di una sindrome da conflitto anteriore [5], mentre una sindrome da conflitto posteriore può essere correlata alla presenza di un os trigonum o di un processo posteriore del talo prominente ( Fig. 3) [6-9]. La continenza della sindesmosi senza lesioni ossee è, come detto, ben valutata staticamente dalla "proiezione per il mortaio", in quanto permette di apprezzare lo spostamento mediale della tibia distale e l'aumento della distanza intermalleolare tibio-peroneale. Un ruolo importante nella valutazione dei legamenti è svolto dai radiogrammi dinamici realizzabili con apparecchiatura TELOS che, applicando mediante un dinamometro un carico determinato (15 kg), permette uno studio oggettivo dello stato legamentoso della caviglia. Vengono eseguiti due test in modo comparativo: • cassetto anteriore: serve per valutare l'integrità del legamento collaterale laterale. Nei casi in cui questo è normale, applicando una spinta anteriore sulla gamba in proiezione laterale il profilo dell'astragalo e quello della tibia restano paralleli, mentre nel caso di lesione PAA si osserva una distasi posteriore >8 mm. Diastasi più grandi depongono per una lesione anche del peroneo-calcaneare ( Fig. 4) • tilt astragalico: se lo stress è in varo si valuta l'integrità del legamento collaterale laterale.
