Background:Studies aimed at assessing whether the emboli lodged in the central pulmonary arteries carry a worse prognosis than more peripheral emboli have yielded controversial results.Aims:To explore the impact on survival and long-term prognosis of central pulmonary embolism.Patients and Methods:Consecutive patients diagnosed with acute symptomatic pulmonary embolism by means of computed tomography (CT) angiography were evaluated at episode index and traced through the computed system of clinical recording and following-up. Central pulmonary embolism was diagnosed when thrombi were seen in the trunk or in the main pulmonary arteries and peripheral pulmonary embolism when segmental or subsegmental arteries were affected.Results:A total of 530 consecutive patients diagnosed with pulmonary embolism were evaluated; 255 patients had central pulmonary embolism and 275 patients had segmental or subsegmental pulmonary embolism. Patients with central pulmonary embolism were older, had higher plasma levels of N-terminal of the prohormone brain natriuretic peptide (NT-ProBNP), troponin I, D-dimer, alveolar-arterial gradient, and shock index (P < .001 for each one). Patients with central pulmonary embolism had an all-cause mortality of 40% while patients with segmental or subsegmental pulmonary embolism (PE) had an overall mortality of 27% and odds ratio of 1.81 [confidence interval (CI) 95% 1.16-1.9]. Survival was lower in patients with central PE than in patients with segmental or subsegmental pulmonary embolism, even after avoiding confounders (P = .018).Conclusions:Apart from a greater impact on hemodynamics, gas exchange, and right ventricular dysfunction, central pulmonary embolism associates a shorter survival and an increased long-term mortality.
ResuMenEl bazo representa el mayor órgano linfopoyético, contiene el 25% de la masa linfoide total. Participa en la inmunidad celular y humoral e interviene en la renovación de los glóbulos rojos y en la eliminación de las bacterias. Las funciones esplénicas están reducidas cuando el bazo está ausente, lo que implica entre otras complicaciones, una mayor susceptibilidad para padecer una sepsis por organismos encapsulados.Se presentan 6 casos clínicos ingresados en el servicio de Medicina Interna con patología esplénica y se hace una revisión del abordaje a realizar.El espectro de lesiones esplénicas en medicina interna es muy amplio. En ocasiones se puede sospechar patología esplénica por la historia clínica, la exploración física o por citopenias en los análisis. Disponemos de diversas pruebas complementarias para completar el estudio de dichas lesiones. En caso de duda diagnóstica se puede realizar esplenectomía siendo los diagnósticos más frecuentes la cirrosis hepática y el linfoma/leucemia. Palabras clave. Bazo. Esplenomegalia. Hiperesplenismo. It participates in cellular and humoral immunity and intervenes in the renovation of red cells and the elimination of bacteria. Splenic functions are reduced when the spleen is absent, which entails, amongst other complications, greater susceptibility to suffering from sepsis due to encapsulated organisms.We present 6 clinical cases admitted to the Internal Medicine serve with splenic pathology and we make a review of the approach to be used.The spectrum of splenic lesions in internal medicine is very wide. On occasions, a splenic pathology can be suspected due to clinical history, physical exploration or because of cytopenias in the analyses. Different complementary tests are available for completing study of these lesions. A splenectomy can be carried out in case of diagnostic doubt, with the most frequent diagnoses being hepatic cirrhosis and lymphoma/leukaemia.
INTRODUCCIÓNLa enfermedad tromboembólica venosa (ETEV), y su manifestación de mayor gravedad el tromboembolismo pulmonar (TEP) es un proceso prevalente (1-3) y con frecuencia grave (4). Las estrategias diagnósticas del TEP han ido cambiando desde el aserto de que es necesario un alto índice de sospecha clínica, debido a la inespecificidad de sus manifestaciones clínicas y la fácil confusión con otros procesos cardiovasculares y respiratorios, al establecimiento de la probabili- [0212-7199 (2006) RESUMENObjetivo: Valorar la efectividad de los modelos de predicción clínica de tromboembolia pulmonar (TEP).Material y métodos: Aplicación retrospectiva de los tres modelos (canadiense, Ginebra y Pisa) a pacientes diagnosticados inequívocamen-te de TEP por arteriografía o tomografía computerizada (TAC) helicoidal.Resultados: Analizamos 120 pacientes [edad media 71 ± 13 años, varones 63 (52%)] diagnosticados 19 (16%) por arteriografía y 101 (84%) por TAC helicoidal. La distribución en el modelo canadiense fue alta probabilidad 24%, intermedia 59% y baja 17%. En el modelo Ginebra fue de alta probabilidad 21%, intermedia 69% y baja 10%. En el modelo Pisa la distribución fue de alta probabilidad 49%, intermedia 45% y baja 6%. La sensibilidad de los modelos fue respectivamente 0,59, 0,67 y 0,89.Factores que condicionaron baja probabilidad fueron en el modelo canadiense la frecuencia cardiaca, la ausencia de signos de trombosis venosa, la existencia de un diagnóstico alternativo y la baja tasa de cán-cer. En el modelo Ginebra fueron la edad, la frecuencia cardiaca y la PaO 2 . En el modelo Pisa condicionaron baja probabilidad diagnóstica la radiografía de tórax normal y la existencia de edema pulmonar.Conclusiones: En los tres modelos de predicción clínica de TEP se observan deficiencias, siendo el modelo Pisa el más adecuado para predecir la probabilidad de TEP. La probabilidad intermedia en modo alguno debe servir para excluir el diagnóstico, y aún con baja probabilidad un número de pacientes en los tres modelos tendrán TEP.
