The aim of this study was to evaluate morphological and functional malocclusion trait changes in 3- to 12-year-old children and to determine whether such functional traits at the 3, 4, and 5 years of age correlated with malocclusion severity score at 12 years of age. Two hundred and sixty-seven children (132 boys, 135 girls) were randomly selected for a follow-up study from a previous cohort of 560 subjects. Functional and morphological traits were clinically assessed. Five functional malocclusion traits: mouth breathing, atypical swallowing, thumb, pacifier sucking, and bottle feeding were assessed and evaluated. Intra-arch assessment involved measurements of incisor crowding, rotation of incisors, and axial inclination of the teeth. For inter-arch measurements, overbite, anterior open bite, overjet, reverse overjet, anterior crossbite, and buccal segment relationships were recorded. The weighted sum of recorded occlusal traits thus represented the total malocclusion severity score. The median morphological malocclusion severity score was almost the same at 3 and 12 years of age, while functional malocclusion decreased. Sucking habits (finger- or dummy-sucking, bottle feeding) until 5 years of age were statistically significantly correlated with an atypical swallowing pattern from 6 to 9 years (Spearman r = 0.178, P = 0.017), which in turn was statistically significantly correlated with the morphological malocclusion severity score (Spearman r = 0.185, P = 0.042) at 12 years of age. At an early age, the morphological severity score is related to the stage of dental development, while at a later period, malocclusion severity score is also the result of incorrect orofacial functions at an early stage of dental development.

The aim of this study was to quantify the palatal change in three groups of children: children with a unilateral posterior crossbite (TCB) who were treated, children with untreated unilateral posterior crossbite (UCB), and children without a crossbite (NCB). Study casts of 60 Caucasian children in the primary dentition (20 TCB, 20 UCB, and 20 NCB), aged 5.4 ± 0.7 years, were collected at baseline (T1) and at 1-year follow-up (T2). Both TCB and UCB groups had unilateral posterior crossbite and midline deviation. The TCB group was treated using a cemented acrylic splint expander in the upper arch. The study casts were scanned using a laser scanner and palatal surface area, palatal volume, and symmetry of the palatal vault were evaluated and compared between the three groups. At T1, the palatal volume of TCB (2698 mm(3)) and UCB (2585 mm(3)) was significantly smaller than that of NCB (3006 mm(3); P < 0.05, analysis of variance test). After treatment, the palatal volume of the TCB group (3087 mm(3)) increased and did not differ from the NCB group (3208 mm(3)), whereas the UCB (2644 mm(3)) had a significantly smaller palatal volume than the NCB or TCB groups (P < 0.05). The increase of palatal volume in the TCB group (389 mm(3)) was significantly greater than in the UCB (59 mm(3)) and NCB (202 mm(3)) groups. The symmetry of the palatal vault was greater than 90 per cent in all three groups at T1 and at T2. Treatment of unilateral posterior crossbite in the primary dentition has a significant effect, particularly on the palatal volume increase.

The aim of the present study was to evaluate the association of tongue posture with the dentoalveolar maxillary and mandibular morphology in a group of Class III subjects in comparison to a group of Class I subjects. Twenty Class III subjects (9 males, 11 females, 19.2 ± 4.6 years) and 20 Class I subjects (6 males, 14 females, 17.4 ± 1.7 years) were included in the present study. Maxillary and mandibular morphology was defined by the intermolar and intercanine distances, at both the cusps and gingival levels, and by measuring surface area and volume of the palatal vault and mouth floor assessed on three-dimensional digital models. Tongue-to-palate distances were measured on lateral cephalograms. The groups were compared using the Mann-Whitney U-test and correlations between each morphological parameter and the tongue-to-palate distances were calculated using the Spearman correlation coefficient. The mandibular intermolar width at the gingival level was significantly greater in the Class III group (P < 0.01), while the maxillary intercanine widths were significantly smaller in the Class III group (P < 0.05). The mouth floor area and volume and the respective ratios between the mouth floor and palate were significantly greater in the Class III group (P = 0.01). The tongue-to-palate distances were generally greater, i.e. lower tongue posture, for the Class III subjects. Significant correlations were seen between tongue-to-palate distances in the posterior region with the area ratio (rho = 0.44, P < 0.05). Tongue posture is significantly lower in Class III subjects and is associated with the dentoalveolar characteristics of the maxilla and mandible.

Skeletal relationships in the sagittal plane do not always correspond with dental relationships. The aim of this study was to determine in which type of malocclusion the correlation between overjet and skeletal sagittal parameters assessed by lateral cephalogram analysis is the highest. The extent to which overjet can predict skeletal relationships in the sagittal plane was also assessed. Eighty-three subjects fulfilled the inclusion criteria (40 males and 43 females, mean age 16.3 +/- 4.3 years). Overjet was measured on study casts and sagittal skeletal relationships were analysed on lateral cephalograms. ANB angle, Wits appraisal, and convexity at point A were determined. Mean values and standard deviations of measured parameters were calculated for Class I, Class II division 1, and Class III malocclusion subjects. Correlation between overjet measured on study casts and sagittal skeletal parameters measured on lateral cephalogram was calculated. Overjet as a predictor of skeletal relationships was assessed by means of linear regression analysis. A statistically significant positive correlation (P < 0.01) was found between the values of overjet and ANB (r = 0.690), overjet and Wits appraisal (r = 0.750), and overjet and convexity at point A (r = 0.608) when assessing the whole sample. When linear regression between overjet and cephalometric parameters was assessed separately in Class I, Class II division 1, and Class III malocclusion subjects, the percentage of variability was statistically significant in just four pairs. The findings show that overjet is a good predictor of sagittal relationship only in subjects with a Class II division 1 malocclusion.

Prevalence of malocclusion among prematurely born subjects is not significantly different compared with controls, and premature birth is not a risk indicator for any functional or morphological characteristics of malocclusion.
