Pediatric septic hip sequelae represent a challenging condition to the treating surgeons with conventional femoral and pelvic osteotomy being difficult to solve advanced head and neck destruction. Surgeons usually resolve to pelvic support osteotomy which is technically demanding and may require revision if performed before skeletal maturity. We report a 10 years old girl which had sequelae of septic coxitis Hunka type IVb, the patient was treated by a C-shaped pertrochanteric and Salter osteotomy and by applying Ilizarov methods of distraction histogenesis we did reduction and gradual lengthening of the femoral neck stump which was adapted to the acetabular cavity at last assessment, the clinical and radiographic parameters had improved significantly at 14 years long term follow-up and the patient was able to walk painlessly and without support. The case represents an introduction to an alternative surgical technique which makes future joint replacement more feasible.
Введение. последствия септического коксита достаточно разнообразны и могут проявляться в виде повреждения зоны роста, деформации и деструкции суставных компонентов, нарушения суставных соотношений. Для лечения этой патологии используются технически сложные и травматичные оперативные вмешательства. Цель исследования-изучить эффективность лечения детей с последствиями септического коксита методом чрескостного остеосинтеза. Материал и методы. проанализированы результаты лечения 37 детей с последствиями септического коксита с применением аппарата Илизарова. Средний возраст на момент выполнения оперативного вмешательства составил 8,2±0,5 лет (от 6 до 10 лет). пациенты были распределены на три группы с учетом анатомических особенностей. Сроки наблюдения пациентов составили от 3 до 10 лет (средний срок-4,7 года). Функциональные исходы оценивали по критериям C.L. Colton, рентгенологические показатели-по критериям E. Severin и j. Kruczynski. Результаты. Функциональные результаты лечения пациентов первой группы: хороший (12-15 баллов)-3 сустава, удовлетворительный (9-11 баллов)-5 суставов, неудовлетворительный (8 баллов)-1 сустав. Рентгенологические результаты в первой группе пациентов по критериями j. Kruczynski: хороший-5 суставов, удовлетворительный-4 сустава; по критериям E. Severin: Ia-1, Ib-1, IIa-2, IIb-3, III-2. Функциональные результаты лечения пациентов второй группы: хороший (12-14 баллов)-10 суставов, удовлетворительный (9-11 баллов)-8 суставов. Распределение результатов во второй группе по критериями j. Kruczynski: хороший-9, удовлетворительный-9; по критериям E. Severin: IIa-14, IIb-4. Распределение функциональных результатов в третьей группе в соответствии с критериями C.L. Colton: хороший (12-15 баллов)-3 сустава, удовлетворительный (9-11 баллов)-7 суставов. Распределение результатов в третьей группе по критериям j. Kruczynski: хороший-4, удовлетворительный-6; по критериям E. Severin: IIa-5, IIb-4, III-1. Выводы. применение технологии реконструкции тазобедренного сустава с применением аппарата внешней фиксации у детей с последствиями септического коксита обеспечивает достаточную стабильность сочленения, не вызывая при этом его декомпенсации и развития тугоподвижности. Использование аппарата Илизарова расширяет возрастные границы для реконструктивного лечения патологического дистензионного вывиха бедра.
