RESUMOO terço médio da face é frequentemente acometido em acidentes de trânsito gerando fraturas principalmente no complexo zigomaticomaxilar (CZM) e na região naso-órbitoetmoidal (NOE). As fraturas desta região geram comprometimento funcional pelo envolvimento do globo ocular e vias aéreas superiores, além das deformidades estéticas. O presente artigo tem como objetivo relatar um caso clínico de fratura do terço médio da face, bem como o resultado obtido com o tratamento. Paciente do sexo masculino, 51 anos, vítima de acidente motociclístico contra caminhão, evoluindo com múltiplas fraturas em terço médio da face, acometendo o CZM e região NOE, ao exame físico apresentava, dentre outros, sinais de enoftalmo direito e telecanto traumático bilateral, referia diplopia em olho direito e dificuldade respiratória. Devido ao comprometimento funcional e estético das fraturas o paciente foi submetido ao procedimento cirúrgico três dias após o trauma, sob anestesia geral, para realização de redução aberta e fixação das fraturas. As melhoras clínicas puderam ser observadas no pós-operatório imediato e se mantiveram ao longo do tempo. No acompanhamento pós-operatório, aos 60 dias o paciente evoluiu livre de comprometimento funcional e apresenta pequenos desvios estéticos. O diagnóstico e tratamento precoce proporcionam um melhor resultado clínico favorecendo a manutenção das funções e harmonia da face do paciente.
Epidermoid cyst (EC) is a benign lesion that emerges as a painless and fluctuating increase of volume. EC is a rare injury in the oral cavity and its diagnostic through clinical exams and conventional radiography provides limited results. Thus, the purpose of the current work is to report two cases of rare tongue lesions diagnosed as EC in two adult patients treated surgically. A 65-year old man and a 40-year old woman were complaining of a "little ball" in their tongue. The patients presented tongue lesions in anatomical sites with distinct clinical presentations. The adopted treatment was a resection of the lesion and, after a histopathological analysis, the final diagnostic pointed to an epidermoid cyst. In the current cases, the lesion occurred in the belly and dorsum of the tongue, and both were treated surgically. And so they became the 14 th and 15 th cases of EC in tongues found in the literature, thus confirming the relevance of the current work.
Infecções odontogênicas são condições potencialmente fatais quando não tratadas de forma precoce devido a sua rápida disseminação por espaços faciais profundos e seu alto potencial de obstrução de vias aéreas. Ocorre, com frequência, em pessoas com pobre higiene oral, ou que iniciaram tratamento e não deram continuidade, ou ainda por iatrogenias, criando assim um ambiente para proliferação bacteriana. Pacientes que apresentam infecções de origem odontogênica apresentam sinais como dor, aumento de volume e trismo, em casos mais graves podem apresentar disfagia, disfonia e dispneia. O tratamento primário é garantir vias aéreas pérvias e controle do estado hemodinâmico do paciente, na maioria dos casos, isso é obtido com drenagem cirúrgica e antibioticoterapia, e remoção da causa. O objetivo deste trabalho é relatar 02 casos de infecções odontogênicas graves e discutir o manejo rápido instituído.
A pandemia pela COVID-19 promoveu o comprometimento dos sistemas de saúde de diversos países. Este estudo retrospectivo foi realizado para avaliar o efeito provocado pela pandemia no serviço de Cirurgia e Traumatologia Bucomaxilofacial do Hospital Universitário do Oeste do Paraná, em Cascavel. Foram comparados os pacientes atendidos no período de pandemia, entre 16 de março de 2020 até 15 de março de 2021, com o mesmo período, um ano anterior, denominado pré-pandemia. Os dados foram categorizados com base no total de atendimentos, horário e data de admissão e etiologias. Variações de idade e sexo também foram avaliadas. Estatísticas foram admitidas com significância em p <0,05. Os resultados demonstram que houve redução dos casos durante a pandemia (n=321) comparado com o período pré-pandemia (n=423 p=0,03). Também houve diminuição nas admissões noturnas entre os períodos (n=126 e n=181, respectivamente) com significância (p=0,037). Em relação ao sexo, o perfil de pacientes masculinos apresentou redução (n=220 e n=303, p=0,005). A etiologia de casos de agressão física apresentou redução significativa (n=38 e n=64, p=0,016) assim como os casos de multifatorial (n=49 e n=97, p=0,011). O período de pandemia mostrou associação com a redução de pacientes com trauma bucomaxilofacial. Concluiu-se que a redução de casos admitidos no período noturno assim como de pacientes do sexo masculino pode ser atribuída como resultado das restrições de circulação que a pandemia impôs. O efeito do consequente distanciamento social, sugere uma diminuição no trauma bucomaxilofacial devido à agressão física.
