Abstract:The wound healing process has always been an excellent subject for researchers. The use of lowpower laser on wounds during the postoperative phase has increased the speed of the healing process. It has been implied that low power radiation affects cellular metabolic processes and promotes beneficial biological effects (analgesic, anti-inflammatory, and healing). Laser biostimulation appears to influence the behavior of the repair process. This paper aims at reviewing the most interesting aspects of the use of lowpower laser in the tissue-repair process. Key words: lasers, low-level laser therapy, wound healing Resumo: Os lasers de baixa potência promovem efeitos biológicos benéficos, de caráter analgésico, antiinflamatório e cicatrizante, por meio de um fenômeno de bioestimulação. A radiação emitida pelo laser terapêutico afeta os processos metabólicos das células-alvo, produzindo efeitos bioestimulantes que resultam na ocorrência de eventos celulares e vasculares, os quais parecem interferir diretamente no processo de reparo. Este trabalho visa estudar o fenômeno da bioestimulação e destacar os principais efeitos bioestimulantes do laser de baixa potência na reparação tecidual. The objective of this study was to compare the efficacy of fluoride varnish (Fluorniz®) and irradiation with a gallium-arsenide-aluminum diode laser in the treatment of cervical dentin hypersensitivity. Cervical dentin hypersensitivity (CDH) is a painful condition that is highly prevalent in the world's adult population, with one in six patients presenting this symptom. Eighty-six teeth were divided into two groups: Group 1, teeth treated with Fluorniz; Group 2, teeth irradiated with a GaAlAs laser at a 4 J/cm 2 dose. The two treatments were applied to the buccal cervical region in four sessions, at intervals of 72 to 96 h. The response of the patient to tactile and thermal-evaporative stimuli was rated on a visual analog scale. The results showed a reduction of hypersensitivity in response to tactile and thermal-evaporative stimulation at the end of treatment in both groups.In conclusion, short-term treatment with Fluorniz was found to be more effective than low-level laser radiation in reducing cervical dentin hypersensitivity.
Phenytoin-induced gingival overgrowth (PIGO) is a common complication of the continuous use of medications. This paper presents a case of PIGO hindering oral function and compromising oral hygiene and aesthetics, which was treated with a combination of nonsurgical and surgical periodontal therapies. A 39-year-old male patient was referred for dental treatment with several complaints, especially upper and lower gingival overgrowth that hindered speech and swallowing. Generalized deep probing pockets and bone loss were detected. Diagnosis of gingival overgrowth associated with phenytoin and chronic periodontitis was established. The treatment plan consisted of conservative therapy with education on oral health, motivation and meticulous oral hygiene instruction in combination with scaling and root planing. During the revaluation period, a marked reduction in the clinical parameters was noted, particularly probing pocket depth reduction. Surgical therapy for removal of gingival overgrowth was also performed to achieve pocket reduction. Supportive periodontal therapy was proposed and the patient is currently under follow-up for 4 years. Management of PIGO may be obtained by the use of periodontal procedures combined with good oral hygiene and periodontal supportive care.
Objetivo: Avaliar a eficácia do uso da solução de fluoreto de sódio a 4% para o tratamento da Hipersensibilidade Dentinária Cervical (HSDC) em comparação ao LASER de baixa potência. Método: Foi realizado um ensaio clínico controlado randomizado, constituído por um grupo teste (47 dentes) e grupo controle (47 dentes). Para o grupo teste foi realizada aplicação do fluoreto de sódio a 4%, e o grupo controle recebeu irradiação do LASER de baixa potência. As terapias dos dois grupos foram realizadas em três sessões com intervalo de uma semana entre elas. Previamente ao tratamento (T0), imediatamente após a aplicação das terapias em cada sessão (T1, T2 e T3), e nos tempos de 1 (T4), 3 (T5) e 6 (T6) meses, os elementos dentários foram submetidos aos estímulos táctil e térmico-evaporativo e as respostas foram obtidas através da Escala Visual Numérica (EVN). A análise estatística dos dados foi realizada através dos testes não paramétricos Wilcoxon, Friedman e Mann-Whitney. Resultados: Em T6, a média de dor do grupo teste passou de 5,93 para 1,73 (p < 0,05) ao estímulo evaporativo e de 3,63 para 0,804 ao estímulo táctil (p < 0,05). Para o grupo controle, a média da dor passou de 6,020 para 1,854 (p < 0,05) ao estímulo evaporativo e de 3,50 para 0,937 (p < 0,05) ao estímulo táctil. Não houve diferença entre as duas modalidades de tratamento em T6. Conclusão: O fluoreto de sódio a 4% foi tão eficaz quanto o LASER na redução da HSDC.
Implantes dentários realizados imediatamente após a exodontia contam com uma alta taxa de sucesso e permitem a reabilitação do paciente com uma quantidade menor de intervenções para a finalização do caso. O estudo em questão tem o objetivo de relatar um caso de uma paciente que foi submetida a técnica do implante imediato em área estética. Neste artigo é relatado um caso clínico de uma paciente do sexo feminino, jovem, onde houve a indicação de exodontia do elemento 11, incisivo central direito, e após a exodontia atraumática foi feita a instalação do implante dentário de forma imediata, entretanto, sem carga imediata. Foi utilizado enxerto ósseo Geistlich Bio-Oss® para preencher os espaços entre o implante e o alvéolo e para melhorar a qualidade dos tecidos moles foi feito enxerto de tecido conjuntivo concomitante. A reabilitação provisória da paciente consistiu na instalação de uma prótese adesiva, que permite a reabilitação provisória, sem colocar carga no implante recém instalado e sem realizar desgaste nos dentes adjacentes. Sabe-se que a estética do sorriso tem impacto direto na autoestima do paciente, já que os seres humanos são seres sociais que baseiam toda sua existência na comunicação e imagem, ter uma parte de destaque da face afetada negativamente pode trazer enormes prejuízos ao auto apreço e relações psicossociais. Desta forma, o uso desta técnica possibilitou a reabilitação da paciente de forma funcional e estética, tendo ganho em sua qualidade de vida.
