RESUMENIntroducción: la calidad de vida relacionada con la salud (CVRS) se centra en los aspectos relativos a la percepción de la salud experimentada y declarada por la persona, en diferentes dimensiones tales como física, mental, social, la percepción general de la salud y la satisfacción alcanzada medida en diferentes niveles.Objetivo: determinar la CVRS de los pacientes con diabetes mellitus tipo 2 que son atendidos en el Dpto. de Medicina Interna del Hospital Nacional (Itauguá, Paraguay) en 2016.Metodología: el instrumento medición utilizado fue el cuestionario Diabetes-39 que es una escala multidimensional, que es completada por el propio paciente. A mayor puntuación, peor CVRS.Resultados: fueron encuestados 101 diabéticos. La edad media fue 57±16 años (rango 29-93 años). Hubo 55 varones (54%) y 46 mujeres (46%). El estado civil predominante fue el de casado (64%) y hubo predomino de sujetos con escolaridad secundaria (47%). La media de CVRS fue 57, valor que se encuentra por encima del estándar establecido por el cuestionario.El 49% de la muestra presenta mala CVRS en contraste con 51% con buena CVRS. Los dominios más afectados por orden de puntuación fueron: energía y Dentro de las variables estudiadas, el sexo masculino se constituyó en factor relacionado a la buena CVRS.Conclusiones: 49% de la muestra presenta mala CVRS y 51% buena CVRS.Palabras claves: diabetes mellitus tipo 2, calidad de vida relacionada con la salud, cuestionarios. Methodology:The instrument used as measurement was the Diabetes-39 questionnaire which is a multidimensional scale, which is completed by the patient. The higher the score, the worse HRQL.Results: 101 diabetics were surveyed. The mean age was 57 ± 16 years (range 29-93 years). There were 55 males (54%) and 46 females (46%). The predominant marital status was married (64%) and there were predominant subjects with secondary schooling (47%). The mean HRQoL was 57, a value that is above the standard established by the questionnaire. 49% of samples presented poor HRQL compared to 51% with good HRQL. The domains most affected by ranking order were: energy and mobility (60), anxiety-worriesome (62) and sexual functioning (66).Among the variables studied, the male sex was a factor related to good HRQoL.Conclusions: 49% of the sample had poor HRQL and 51% had good HRQL.Key words: diabetes mellitus type 2, quality of life related to health, quest.
Correlación entre el cociente proteinuria/creatininuria en una orina al azar y la proteinuria de 24 horas Correlation between the random urine protein-creatinine ratio and 24-hour proteinuria Artículo recibido: 3 enero 2015 Artículo aceptado: 26 enero 2015 ResumenIntroducción: la cuantificación de proteínas en orina es un estudio que evalúa la afectación renal por ciertas enfermedades. Su medición puede realizarse también a través del cociente proteinuria/creatininuria. Objetivo: determinar la correlación entre el cociente proteinuria/creatininuria y la proteinuria de 24 horas. Metodología: se realizó un estudio descriptivo observacional, prospectivo con componente analítico de corte transverso, de muestreo no probabilístico. Se incluyó a 60 pacientes con factores de riesgo de padecer enfermedad renal crónica, que acudieron al Hospital Nacional (Itauguá) en el año 2014. Todos se realizaron análisis de orina de 24 horas y de una muestra de orina al azar para estimar el cociente proteinuria/creatininuria. Resultados: se observó que existe una correlación muy significativa (r= 0,9 p < 0,001) entre los valores del cociente de proteinuria/creatininuria en una orina al azar y la proteinuria de 24 horas, con una sensibilidad 94,1% (IC95% 79-100), especificidad 100% (IC95% 98-100%), valor predictivo positivo 100% (IC95% 96-100) y valor predictivo negativo 97,7% (IC95% 92-100). The measurement can be made through the urine protein-creatinine ratio. Objective: To determine the correlation between the urine protein-creatinine ratio and 24-hour urine protein. Methodology:A cross-sectional prospective observational descriptive study with analytical component and non-probabilistic sampling was performed. Sixty patients who had risk factors of chronic renal disease and attended the National Hospital (Itauguá) in 2014 were included. Twenty four-hour urine and a random urine sample were analyzed to estimate the protein-creatinine ratio. Results: A very significant correlation (r= 0.9 p < 0.001) was observed between the values of the proteincreatinine ratio in a random urine and 24-hour urine protein with a sensitivity of 94.1% (IC95% 79-100), specificity of 100% (IC95% 98-100%), positive predictive value of 100% (IC95% 96-100) and negative predictive value of 97.7% (IC95% 92-100). Púrpura trombocitopénica trombótica asociada a COVID-19 RESUMEN Introducción: La púrpura trombocitopénica trombótica es una patología rara, ha sido asociada a la infección por el virus SARS-CoV-2. Presentación del caso: Mujer de 27 años, consulta por cuadro de 12 días con tos, astenia, cefalea, dolor muscular, pérdida del olfato y del gusto. Se le realiza hisopado nasofaríngeo retorna positivo para SARS-CoV-2. Recibe tratamiento ambulatorio. 48 horas antes del ingreso presenta parestesias en mano derecha que asciende al miembro superior derecho, con disminución de la fuerza muscular en hemicuerpo derecho, desvío de la comisura labial hacia el lado opuesto, recuperación de la fuerza muscular 40 minutos después, es traída al Hospital Nacional. Examen Físico: FC 80 por minuto, FR 20 por minuto, PA 120-80 mmHg, temperatura 36,2°, conjuntivas pálidas. Examen sistema nervioso: normal, resto del examen físico sin datos de valor. Pruebas laboratoriales: Glóbulos rojos 2,45 por 109, hemoglobina (Hb 7,6 g/dl), hematocrito (Hto 22%), plaquetas 34.000 por mm3. Reticulocitos 12,6% (0,5-2%), urea 30 mg/dl, creatinina 0,76 mg/dl, bilirrubina total 3,05 mg/dl bilirrubina directa 0,79 mg/dl, bilirrubina indirecta 2,26 mg/dl, haptoglobina 2mg/dl, GOT 45 U/L, GPT 27 U/L, LDH 1553 Ul, PCR 2.4 mg/dl. Serología: toxoplasmosis. IgG 924,2. IgM 0,2 (negativo), rubéola: IgG 283,30, IgM 0,2 (negativo), citomegalovirus IgG 165,2 IgM 0,5 negativo. Herpes HSV1 y 2 IgG positivo IgM negativo, C3 154, C4 25 mg/dl (10-40 mg/dl), test de Coombs directo negativo, ferritina 1380 ng/ml. Anticuerpo antinuclear (ANA), anticuerpo anti DNA de doble cadena (anti DNA), negativo. Frotis de sangre periférica: esquistocitos (+++). Tomografía de cráneo normal. Tomografía de tórax: infiltrado intersticial bibasal leve bilateral. Se realiza el diagnóstico de púrpura trombocitopénica t r o m b ó t i c a ( P T T ) , r e c i b e 1 0 0 m g d e metilprednisolona, plasma fresco, glóbulos rojos, falleció 24 horas después durante la plasmaferesis de un sangrado en el sistema nervioso central. Conclusión: La PTT es una patología grave, puede presentarse en pacientes con COVID-19.
RESUMENIntroducción: la fiebre que se presenta en un paciente con lupus eritematoso sistémico (LES) es muchas veces un desafío etiológico para el Internista, por lo que resulta trascendental establecer un correcto diagnóstico, ya que las conductas terapéuticas a adoptar son claramente diferentes. Objetivos: describir las causas de fiebre en pacientes con LES del Hospital Nacional y determinar la frecuencia de actividad lúpica según la escala MEX-SLEDAI. Metodología: estudio observacional, prospectivo, realizado en pacientes de ambos sexos mayores de edad, portadores con LES que acudieron por fiebre al Hospital Nacional (Itauguá, Paraguay) entre abril y noviembre de 2016. Resultados: se incluyeron 37 pacientes con edad media 30±11 años, con predomino del sexo femenino (89%). El tiempo medio de duración de la fiebre fue 8±5 días y las causas de la misma fueron: infecciones (43%), reactivación (35%) y ambas causas (22%). Los focos principales de infección fueron pulmonar, piel y partes blandas. La media de puntajes de la escala MEX-SLEDAI fue 8±5 puntos. Los pacientes con esta escala en valores altos se caracterizaron por ANA elevado y complementos disminuidos. Conclusiones: la causa de fiebre más frecuente en pacientes con LES fue la infección. It is fundamental to make a correct diagnosis because the therapeutic choices are clearly different. Objectives: To describe the causes of fever in patients with SLE of the National Hospital and determine the frequency of the lupus activity according to the MEX-SLEDAI. Rev. virtual Soc. Parag. Med. Results: Thirty seven patients were included, they had a mean age of 30±11 years and there was a predominance of women (89%). The mean time of fever duration was 8±5 days and the causes were: infections (43%), reactivation (35%) and both causes (22%). The main infection foci were lung, skin and soft tissues. The mean score of MEX-SLEDAI was 8±5 points. The patients who had high values in this scale were characterized by high ANA and decreased complements. Conclusions:The most frequent cause of fever in patients with SLE was infection. The mean score in MEX-SLEDAI was 8±5 points.
RESUMENIntroducción: la deficiencia de hierro es una comorbilidad en pacientes con insuficiencia cardiaca que puede provocar graves consecuencias clínicas y generar mal pronóstico.Objetivos: determinar la frecuencia de anemia ferropénica en pacientes internados con insuficiencia cardiaca y describir sus características demográficas y clínicas de los pacientes.Metodología: observacional descriptivo prospectivo de corte transversal. Se incluyó a pacientes varones y mujeres, mayores de 18 años de edad, portadores de insuficiencia cardiaca, internados en el Hospital Nacional (Itauguá, Paraguay) en años 2017-2018. Se midieron variables clínicas y laboratoriales. Se consideró anemia ferropénica a todo valor de hemoglobina ≤13 g/dL en hombres y ≤ 12 g/dL en mujeres con ferritina <100 µg/L. Resultados: se incluyeron 152 pacientes con edad media 68±14 años y siendo 57% del sexo masculino. Entre las comorbilidades predominó la hipertensión arterial (95%). Se encontró 72% de sujetos con anemia ferropénica, siendo la frecuencia superior en los grados II y III de la clasificación funcional de la NYHA Conclusión: La frecuencia de anemia ferropénica en pacientes con IC fue 72% y predominó en el sexo masculino y con clase funcional II-III. La comorbilidad más frecuente fue la hipertensión arterial. Objectives: to determine the frequency of iron deficiency anemia in patients hospitalized with heart failure and to describe their demographic and clinical characteristics. Methodology: observational, descriptive, prospective, cross-sectional. Were included male and female patients, older than 18 years of age, with heart failure, admitted to the National Hospital (Itauguá, Paraguay) in the years 2017-2018. Clinical and laboratory variables were measured. Iron-deficiency anemia was considered in patients with hemoglobin value ≤ 13 g / dL in men and ≤ 12 g / dL in women with ferritin <100 μg / L.Results: 152 patients were included, with a mean age of 68 ± 14 years being 57% males. Between comorbidities, arterial hypertension was prevalent (95%). 72% of subjects with iron deficiency anemia were found, the frequency being higher in grades II and III of the NYHA functional classification Conclusion: The frequency of iron deficiency anemia in patients with HF was 72% and was prevalent in males with functional class II-III.
