SummaryBackgroundJejunal diverticulosis is a rare, usually asymptomatic disease. Its incidence increases with age. If symptomatic, diverticulosis may cause life-threatening acute complications such as diverticulitis, perforation, intestinal hemorrhage and obstruction. In this report, we aimed to present a 67-year-old male patient with jejunal diverticulitis accompanying with abdominal pain and vomiting.Case ReportA 67-year-old male patient complaining of epigastric pain for a week and nausea and fever for a day presented to our emergency department. Ultrasonographic examination in our clinic revealed diverticulum-like images with thickened walls adjacent to the small intestine loops, and increase in the echogenicity of the surrounding mesenteric fat tissue. Contrast-enhanced abdominal computed tomography showed multiple diverticula, thickened walls with showing contrast enhancement and adjacent jejunum in the left middle quadrant, increased density of the surrounding mesenteric fat tissue, and mesenteric lymph nodes. The patient was hospitalized by general surgery department with the diagnosis of jejunal diverticulitis. Conservative intravenous fluid administration and antibiotic therapy were initiated. Clinical symptoms regressed and the patient was discharged from hospital after 2 weeks.ConclusionsIn cases of diverticulitis it should be kept in mind that in patients with advanced age and pain in the left quadrant of the abdomen, diverticular disease causing mortality and morbidity does not always originate from the colon but might also originate from the jejunum.
ÖZET Amaç:Tiroid nodülleri toplumda sık görülmesine karşın bunlarda malignite olasılığı daha azdır. Malign olguların doğru bir biçimde ayırt edilebilmesi için sonografik olarak malignite kuşkulu tiroid nodülüne ince iğne aspirasyon biyopsisi (İİAB) yapılması günümüzde noninvaziv bir yöntem olarak uygulanmaktadır. Metod:±Hastanemiz radyoloji anabilim dalında 2009-2012 yılları arasında yapılan tiroid İİAB lerini yeniden değerlendirmeyi, cerrahi uygulanan hastaların sitolojikhistopatolojik bulguları ile US verilerini karşılaştırmayı ve bulguları güncel literatür eşliğinde tartışmayı amaçladık. Bulgular: Çalışmaya 280 hastaya ait 284 nodül alınmıştır. Hastaların 234 tanesi kadın (%83,57), 46 tanesi erkek (%16,42) tir. Kadın erkek oranı 5,08/1 dir. Yaş ortalaması 50,12±10,3 dir. Her hastada patolog tarafından hızlı hasta başı değerlendirme yapılmıştır. Toplam 156 (%54,9) lezyonda 1 kez; 91 (%32,0) lezyonda 2 kez ve 29 (%13,1) lezyonda 3 kez girişim yapılmıştır. Ortalama girişim sayısı 1,55'tir. Her olgu için incelenen lam sayısı ortalama 5,44 adettir. Tanısal olmayan/değerlendirme için yetersiz materyal sayısı 7'dir (%2,5). Hiçbir hastada işlem sırasında veya sonrasında komplikasyon izlenmemiştir (%0). İİAB sonuçlarına göre ameliyat edilen 34 hastanın sitolojik ve histopatolojik tanıları korele edilmiştir. İİAB'nin maligniteyi saptamada doğruluk, özgüllük, duyarlılık, negatif ve pozitif beklenen değerleri sırasıyla %85.00; %86.20; %81.81; %92.59; %69.23 olarak bulunmuştur. Sonuç: Sonuç olarak tiroid nodüllerine tanı/tedavi yaklaşımında US eşliğinde İİAB testi, hastalığın yönetiminde oldukça güvenilir, non-invaziv bir yöntemdir. Bu testin başarısı sadece sitolojik tanıya ulaşılabilmesi değil olası moleküler testler için de uygun nitelikte örnek elde edilebilmesiyle mümkündür.

Intrabiliary rupture is one of the most serious complications of a hepatic hydatid cyst. Radiological findings, especially magnetic resonance imaging and magnetic resonance cholangiopancreatography (MRI/MRCP), are helpful in the diagnosis of hydatid disease.Case ReportWe present a 48-year-old female patient with complaints of abdominal pain and jaundice. Radiological examination showed a heterogeneous lesion that contained cystic-solid components and millimetric calcifications in the liver. Adjacent intrahepatic bile ducts were dilated.ConclusionsIn geographical areas endemic for hydatid disease, cyst rapture into the bile ducts should be included in the differential diagnosis even in seronegative cases, although it is not typical for hydatid cyst to be found as a mass lesion in the liver on US in patients with right upper quadrant pain and jaundice.Detailed imaging by MRI/MRCP should be done.
Kikuchi-Fujimoto hastalığı (histiyositik nekrotizan lenfadenit), 1972'de tanımlanmış, genelde Asyalı genç bayanlarda görülen, ateş gibi sistemik bulgularla seyreden nadir bir servikal lenfadenopati nedenidir. Etiyopatogenezi bilinmemekle birlikte enfeksiyonlar ve otoimmunite suçlanmaktadır. Biz, 1,5 aydır süren ve antibiyotik ile düzelmeyen, sağ boyunda ağrılı şişlik nedeniyle başvuran, Kikuchi-Fujimoto hastalığı tanısı koyduğumuz 26 yaşında bayan hastayı radyoloji, İİA sitolojisi ve histopatolojik bulguları ile birlikte sunmayı amaçladık. İİA sitolojisi yararlı bir tanısal yöntem olmakla birlikte bu hastalıkta tanıyı kesinleştirebilmek için çoğu kez histopatolojik inceleme de gerekmektedir.
