ObjectiveRepetitive transcranial magnetic stimulation (rTMS) is an effective treatment for major depressive disorder (MDD). This study evaluated the antidepressant effect of rTMS and examined how it affected N-asetyl aspartate (NAA), choline (Cho), creatine (Cr), lactate (Lac), myoinositol (mIns), glutamate (Glu), glutathione (GSH), and glutamine (Gln) metabolite levels in the left dorsolateral prefrontal cortex (DLPFC) of MDD patients who were not receiving antidepressant medication. MethodsIn total, 18 patients (10 female, 8 male) were evaluated. Each patient underwent H magnetic resonance spectroscopy (H-MRS) before and within 3 days of completion of TMS therapy. All patients completed 20 sessions of rTMS directed at the left DLPFC over a 2-week period. The Hamilton Depression Scale (HAMD) scores of patients were calculated, and their responses to treatment were assessed within 1–3 days of completion of TMS. ResultsWe found statistically significant differences in HAMD scores before and after rTMS. Moreover, the peak metabolite ratios of NAA/Cr, GSH/Cr, and Gln/Cr were significantly higher after rTMS compared to those before rTMS. ConclusionIncreased understanding of the mechanism of action of TMS will improve its application and may stimulate development of new-generation therapeutic agents.
ÖZETAmaç: Şizofreni etiyolojisinde genetik, enfeksiyon ve enfeksiyona immün yanıt gibi çevresel faktörlerin rol oynadığı kronik psikotik bir bozukluktur. Bu çalışmanın amacı nükleer faktör-κB (NF-kB) aktivasyonu, myeloperoksidaz (MPO), antiinflamatuar sitokin interlökin-4 (IL-4) ve regulatuar sitokin transforming growth faktör-β (TGF-β) gibi bazı immün faktörleri şizofreni hastalarında ve yaş ve cinsiyet olarak eşleştirilmiş kontrol grubunda karşılaştırmaktır. Yöntem:Tedaviye dirençli 20 şizofreni hastalığı olan kişi ve yaş ve cinsiyet olarak uyumlu 20 sağlıklı kontrolde interlökin-4 (IL-4), transforming growth faktör-β (TGF-β), myeloperoksidaz (MPO) ve nükleer faktör-κB aktivasyonunun (NF-κB) plazma düzeyleri analiz edildi. Hastalık şiddeti Kısa Psikiyatrik Değerlendirme Ölçeği kullanılarak değerlendirildi (BPRS). Bulgular:Bu çalışmada antipsikotik tedaviye dirençli şizofreni hastalarında serum TGF-β düzeyleri kontrollere göre anlamlı derecede daha düşük, NF-κB düzeyleri ise anlamlı derecede daha yüksek saptandı. Serum IL-4 ve MPO düzeylerinde ise hasta ve kontrol grupları arasında anlamlı fark bulunmadı.Sonuç: Bu çalışmada dirençli şizofreni hastalarında, psikotik belirtilerin alevli olduğu dönemde TGF-β seviyesinin düşük saptanması, Th1/Th2 dengesini sağlamakta yetersiz kaldığını göstermektedir. Bu durumun şi-zofrenide dirençten sorumlu olup olmadığının araştırılması için geniş ölçekli çalışmalara ihtiyaç vardır. The aim of this study was to compare some immune factors such as nuclear factor-κB (NF-κB) activation, myeloperoxidase (MPO), the antiinflammatory cytokine interleukin-4 (IL-4), and regulatory cytokine transforming growth factor-β (TGF-β) in schizophrenia patients and an age-and gender-matched control group.Method: Plasma levels of IL-4, TGF-β, MPO, and NF-κB activation in 20 subjects with treatment-resistant schizophrenia and 20 age-and gender-matched healthy controls were analyzed. Disease severity was evaluated using the Brief Psychiatric Rating Scale (BPRS).Results: Plasma TGF-β levels were found to be significantly lower and NF-κB to be significantly higher in antipsychotic treatment-resistant schizophrenia patients than in controls in this study. AbstractÖz There have been some changes of obsessive compulsive disorder in DSM-5 in terms of its classification and description. First of all, it has been suggested that the disorder should be out of a lower cap of the anxiety disorders and with DSM-5 a new heading has become an issue like obsessive compulsive disorder and related disorders. The ones that suggest the obsessive compulsive spectrum disorders fundamentally assume that the obsessive compulsive disorder and the disorders defined as related with the obsessive compulsive disorder come from the same etiologic origin unlike the other anxiety disorders. The purpose of this compilation is to mention on how obsessive compulsive disorder has been defined all along the history and what kind of conceptual changes it went through until DSM-5.Obsesif kompulsif bozukluğun DSM-5'te yeniden sınıflandırılmasında ve tanımlanmasında değişiklikler meydana gelmiştir. İlk olarak hastalığın anksiyete bozukluklarının bir alt başlığı olmaktan çıkarılması önerilmiş ve DSM-5'le beraber obsesif kompulsif bozukluk ve ilişkili bozukluklar şeklinde yeni bir başlık gündeme gelmiştir. Obsesif kompulsif spektrum bozukluklarını önerenler esasen obsesif kompulsif bozukluk ve ilişkili olarak tanımlanan hastalıkların diğer anksiyete bozukluklarından farklı olarak aynı etyolojik kökenden geldiğini varsaymaktadır.
