Objective: Generalized peritonitis is fairly common with very high mortality of delayed diagnosis and treatment of infectious condition. In this study, we aimed to determine the effectiveness of malondialdehyde (MDA), superoxide dismutase (SOD), catalase (CAT) and C-reactive protein (CRP) levels in peritonitis severity.Methods: This study was planned prospective and randomized. Study consisted of three groups. Each group had 50 patients. Group 1, patients with generalize peritonitis ; Group 2,patients with non-abdominal inflammatory disease; Group 3, healthy individuals as a control group. The venous blood samples from patients in group 1 and 2 were collected to determine MDA, SOD, CAT and CRP levels at the operation day. The same procedures repeated postoperative first and third day. Blood samples were collected once from group 3.One-way Anova test was used to compare average of intergroup parametric variables. Repeated Anova test was used to compare intragroup parameters.Results: Demographic features and cause of laparatomy of all groups were evaluated. MDA values of group 1 in days 0, 1st and 3rd compared with group 2 and group 3 the MDA values of group 1 were higher than group 2 and 3 (p<0,0001). CAT values of group 1 in days 0, 1st and 3rd were lower than group 2 and group 3 (p<0,0001). SOD values of group 1 and 2 in day 0 were lower than group 3 (p<0,0001). CRP values of group 1 in days 0,1st and 3rd were higher than group 2 (p<0,0001).
Background: Acute appendicitis (AA) is the most common cause of surgery performed for the acute abdomen. The standard treatment for AA patients has been appendectomy for more than a century. The use of broad-spectrum antibiotics, the medical treatment option has come to the fore in the treatment of uncomplicated AA. Aim: Evaluate whether white blood cell (WBC), platelet count, mean platelet volume (MPV), platelet distribution width (PDW), red cell distribution width (RDW) and Creactive protein (CRP) values of patients diagnosed with uncomplicated AA clinically, with laboratory tests and radiologically with abdominal computed tomography (CT) could be a marker for choosing medical or surgical treatment. SUMMARYIt is a rare entity to observe the coexistence of thyroid gland pathologies and coronary artery disease, whose surgical treatment may be performed simultaneously. In this case, we present a case of a patient with substernal thyroidectomy concurrent with off-pump coronary artery bypass grafting. A 57-year-old female patient was admitted to the hospital with exertional dyspnoea, intermittent coughing and stable angina pectoris. The substernal goitre measuring 5×5×4 cm was accompanied by a 95% in-stent restenosis at the left anterior descending artery. Thyroidectomy and off-pump coronary artery bypass grafting procedures were performed simultaneously. The postoperative period was uneventful and the patient was discharged 5 days after the operation. This case indicates that off-pump revascularisation seems to be a better option in cases where surgical interventions for thyroid and coronary artery diseases are necessary instead of on-pump revascularisation where the adverse effects of the cardiopulmonary bypass are considered.
BACKGROUND
Objective:The most common cause of hydatid cyst disease is Echinococcus granulosus. Nowadays, minimally invasive methods such as laparoscopy used in the treatment of hepatic hydatid cyst (HHC). In this study, it is aimed to share the clinical, surgical and postoperative data of patients who underwent laparoscopic surgery with a diagnosis of HHC in our clinic.Methods: Data of 105 patients who underwent laparoscopic surgery for HHC between January 2011 and January 2020 were retrospectively analyzed. Patients who were diagnosed with HHC by clinical, indirect hemagglutination test, upper abdominal ultrasonography (USG) and upper abdominal tomography were included in the study. All patients received 10 mg / kg / day albendazole therapy for a total of six weeks, two weeks preoperatively and four weeks postoperatively. Demographic information, clinical characteristics, and the incidence and percentage of complications were analyzed by simple manual calculation.Results: 64 (60.95%) of 105 patients who underwent laparoscopic surgery were male and 41 (39.05%) were female. The mean age was 40.6 ± 11.7 years (18-65 age range). The mean cyst diameter was 8.4 cm in size (minimum: 6 cm, maximum: 16 cm). The mean operation time was 55 ± 11.45 minutes (Range: 42 to 70 minute). The mean hospitalization period of the patients was 3.7 days (Range 2 to 7). No recurrence was observed in the 18-month follow-up of the patients.Conclusıons: HHC is a common disease in our country, and we think that laparoscopic approach is an effective surgical treatment option in suitable patients and in experienced centers.
