Resumo Objetivo Analisar a associação entre o tipo de aleitamento e as dificuldades relacionadas à essa prática entre mulheres e crianças assistidas em um ambulatório especializado em amamentação. Métodos Estudo transversal retrospectivo realizado por meio da análise de prontuários de crianças e mulheres atendidas entre 2004 e 2016 em um ambulatório especializado em aleitamento materno. Foram excluídos os registros referentes às mulheres com gestação múltipla e àqueles não realizados em formulário padrão, totalizando 1.608 prontuários. Utilizaram-se os testes Qui-Quadrado e Kruskal-Wallis para comparar o tipo de aleitamento materno com variáveis categóricas; e com os dias de vida e idade materna, respectivamente. O teste Mann-Whitney utilizou-se para comparar a frequência do aleitamento materno exclusivo. Resultados O aleitamento materno exclusivo foi praticado por 72,6% das mulheres atendidas, nos primeiros 30 dias após o parto. Houve associação significativa entre esta prática e as dificuldades: percepção materna quanto à quantidade de leite produzida, de mamas cheias antes das mamadas, de vazamento de leite e extração manual do leite com facilidade; posicionamento materno e da criança, preensão, sucção e deglutição da criança adequados; além das variáveis: maior escolaridade, situação conjugal estável; ter tido experiência prévia com aleitamento materno, ter mamilos protrusos, ter realizado contato precoce pele a pele, ter filhos com menor média de dias de idade e que faziam uso de chupeta. Conclusão O aleitamento materno exclusivo foi o mais prevalente nos primeiros 30 dias pós-parto e diversas variáveis maternas e neonatais estiveram associadas à essa prática no primeiro atendimento em ambulatório especializado.

Objective: To estimate the prevalence and factors associated with sexual dysfunction in the postpartum period. Methods: Cross-sectional study of 200 postpartum women in their resumption to sexual activity. Data were collected in a private place, through interviews and recorded in forms, containing information regarding sexual life of postpartum women. Results: Among the women studied, it was found that 33.5%, 76.0% and 43.5% had sexual dysfunction before pregnancy, during and after delivery, respectively. The types of dysfunction most frequently identified were dyspareunia, vaginismus, dysfunction of desire, orgasmic and arousal. The significantly associated factors were Catholic or protestant religions, vaginal delivery with suture, dyspareunia during pregnancy, vaginismus before pregnancy and working hours over 8 hours/daily. Conclusion: The prevalence of sexual dysfunction was high and associated factors were religion, working hours, previous history of dysfunction and type of delivery. ResumoObjetivo: Estimar a prevalência e os fatores associados à disfunção sexual no período pós-parto. Métodos: Estudo transversal com 200 puérperas que retomaram a vida sexual ativa. Os dados foram coletados, em local privado, por meio de entrevista e registrados em formulário contendo informações pertinentes a vida sexual das puerperas. Resultados: Dentre as mulheres pesquisadas verificou-se que 33,5%, 76,0% e 43,5% apresentavam disfunções sexuais antes da gravidez, durante e após o parto, respectivamente. Os tipos de disfunção identificados com maior frequência foram a dispareunia, seguida do vaginismo, disfunção do desejo, orgásmica e excitação. Os fatores significativamente associados foram as religiões católica ou evangélica, o parto vaginal com sutura, a dispareunia durante a gravidez, o vaginismo antes da gravidez e uma jornada de trabalho além de 8 horas/diárias. Conclusão: A prevalência das disfunções sexuais foi alta e os fatores associados foram: religião, jornada de trabalho, história prévia de disfunção e tipo de parto.

Objective: to verify the factors associated with the practice of skin-to-skin contact with breastfeeding in the first hour of life and its influence on exclusive breastfeeding in the first month. Method: a retrospective cross-sectional study with medical records from a breastfeeding outpatient clinic in São Paulo, Brazil. The sample included all the medical records of the mother-baby dyad who had been consulted by nurses between 2004 and 2010. Data were collected from the medical records between 2014 and 2015. Results: 1,030 medical records were identified, 71 were excluded and the final sample was 959. The prevalence of skin-to-skin contact with breastfeeding in the first hour was 37.2%. The Apgar score between 8 and 10 in the first minute of life and the higher birth weight of the newborn were protective factors of contact with early breastfeeding; lower maternal age and cesarean delivery and forceps were risk factors for this practice. Exclusive breastfeeding was statistically higher in the group of newborns who had contact with breastfeeding in the first hour; however, there was no association with duration of exclusive breastfeeding. Conclusion: good birth conditions were protective factors for breastfeeding followed by skin-to-skin contact in the first hour, while younger maternal age and surgical deliveries proved to be risk factors. Exclusive breastfeeding in the baby´s first month of life was not associated with skin-to-skin contact followed by breastfeeding in the first hour of life.

Objective: To identify the prevalence of postpartum depression symptoms and the level of self-efficacy to breastfeed among puerperal women assisted at a Breastfeeding Incentive Center, and to analyze possible associations. Methods: A cross-sectional study with 208 women up to 60 days postpartum submitted to the Edinburgh Postpartum Depression Scale and the Self-Efficacy Scale for Breastfeeding. Results: Postpartum depression symptoms were present in 31.25% of women who presented medium (39.9%) and high (36.06%) levels of breastfeeding self-efficacy. The medium or high self-efficacy decreased the depression score by 27.4% or 38.8% respectively, while the high score on the postpartum depression scale reduced the self-efficacy score in breastfeeding by 11.84 points. Conclusion: The high prevalence of postpartum depression symptoms and self-efficacy for breastfeeding were evident in the studied population. The levels of postpartum depression symptoms and self-efficacy revealed a cause and effect association between themselves. ResumoObjetivo: Identificar a prevalência de sintomas de depressão pós-parto e o nível de autoeficácia para amamentar, entre puérperas atendidas num Centro de Incentivo ao Aleitamento Materno, e analisar possíveis associações. Métodos: Estudo transversal com 208 mulheres, até 60 dias pós-parto, submetidas à Escala de Depressão Pós-parto de Edinburgo e à Escala de Autoeficácia para Amamentar. Resultados: Sintomas de depressão pós-parto estiveram presentes em 31,25% das mulheres, que apresentaram níveis de autoeficácia para amamentar médio (39,9%) e alto (36,06%). Ter média ou alta autoeficácia diminui em 27,4% ou 38,8%, respectivamente, o escore de depressão, enquanto a elevada pontuação na escala de depressão pós-parto reduz em 11,84 pontos o escore da autoeficácia na amamentação. Conclusão: Prevalência elevada de sintomas de depressão pós-parto e de autoeficácia para amamentar foram evidenciados na população estudada. Os níveis de sintomas de depressão pós-parto e de autoeficácia revelaram associação de causa e efeito entre si.

During the COVID‐19 quarantine period, there was an increased number of patients admitted in advanced stages of labor, resulting in higher rates of vaginal deliveries compared to the same period in 2019.

Objective: To analyze breastfeeding practice among indigenous children aged between zero and two years and the factors associated with ablactation. Methods: Cross-sectional study conducted with 94 indigenous children and 91 indigenous women. Data were collected in households by applying an instrument specifically developed for the study. Logistic regression was used for the analysis. Results: A total of 60.6% of the children were breastfeeding. Exclusive breastfeeding was present in 35% of the children aged under six months. The only association of early ablactation with the variables was the ethnic group, in which the chance of early ablactation among the Poyanawa, Nawa, and Nukini ethnic groups was 3.7 times higher than the Katukinas. Conclusion: The prevalence indices of breastfeeding is below the recommendations of the WHO. Only the variable ethnic group was found to be associated with early ablactation. These data highlight the need to implement programs to promote breastfeeding among indigenous people. ResumoObjetivo: Analisar o aleitamento materno de crianças indígenas de zero a dois anos e os fatores associados ao desmame. Métodos: Estudo transversal realizado com 94 crianças e 91 mulheres indígenas. Os dados foram coletados nos domicílios, com aplicação de um instrumento construído especificamente para o estudo. Para a análise foi utilizada a regressão logística. Resultados: Estavam em aleitamento materno 60,6% das crianças. Em menores de seis meses o AME esteve presente em 35% das crianças. A única associação do desmame precoce com as variáveis foi a etnia, em que a chance de desmame precoce entre as etnias Poyanawa, Nawa e Nukini, foi 3,7 vezes maior em relação a etnia Katukina. Conclusão: As prevalências de AM encontram-se aquém das recomendações da OMS. Somente a variável etnia mostrou-se associada ao desmame precoce. Esses dados mostram a necessidade de implementações de programas de incentivo ao AM entre os indígenas.
