Objective: The purpose of this study was to compare the results of open Bankart repair versus those of modified Bristow operation in patients with recurrent anterior dislocations of the shoulder, the last of which was caused by a minor trauma. Materials and Methods: This study included 38 patients (34 (89.5%) male and 4 (10.5%) female) who presented recurrent dislocation of the shoulders with capsular laxity and who underwent an open Bankart repair or a modified Bristow operation. The mean age of the patients was 29.6 years (range, 17-60 years). The mean follow-up period was 5.5 years (range, 35 months to 9 years). A total of 25 patients (65.8%) underwent an open Bankart repair, whereas 13 (34.2%) underwent a modified Bristow operation. The treatment results were assessed using the Rowe score for instability. Results: Of the 38 shoulders assessed, 24 (63.1%) were right shoulders and 14 (36.9%) were left shoulders. Furthermore, 26 (68.4%) were the dominant shoulders of the patients, and 12 (31.6%) were the nondominant shoulders. The mean time from the first dislocation was 3.8 years (range, 10 months to 11 years). The age at which the first shoulder dislocation occurred was 20 years or younger in 7 cases (18.4%), 21-30 years in 22 cases (57.9%), 31-40 years in 6 cases (15.8%) and 41 or older in 3 cases (7.9%). The patients had experienced 4-10 recurrent dislocations in 15 cases (39.5%), 10-20 recurrent dislocations in 10 cases (26.3%) and 20 or more recurrent dislocations in 13 cases (34.2%). Tekrarlayan çıkık sayısı, 15 omuzda (%39.5) 4-10 arasında, 10 omuzda (%26.3) 10-20 arasında, 13 omuzda (%34.2) 20 ve üze-rindeydi. Açık Bankart tamiri yapılan hastalarda Rowe puanı 85.6, modifiye Bristow ameliyatında 81.9 bulundu. Rowe değerlendirme kriterlerine göre açık Bankart ve modifiye Bristow ameliyatlarının çok iyi ve iyi toplamları arasında istatistiksel olarak anlamlı bir fark bulunmadı (p>0.05).Sonuç: Açık Bankart tamiri ve modifiye Bristow ameliyatları için uygun hasta seçilmesi çok önemlidir. Uygun hasta seçimi yapıldığı takdirde bu iki ameliyat tekniği kapsüler laksisiteli hastaların tedavisinde başarılı bir tedavi yöntemidir.

BACKGROUND:There is no consensus on the optimal treatment of Tillaux-Chaput fractures. The results of our cases treated with mini-open surgery (open reduction and internal fixation) concerning efficacy and complications, we aim to compare other procedures (open, percutaneous, and arthroscopy-assisted) with the literature data and to look for an answer to the question of whether the primary treatment can be mini-open surgery. METHODS:Between the years 2014 and 2017, 22 of 30 patients with Tillaux-Chaput fractures were treated using mini-open surgery, open reduction and internal fixation with one cannulated screw. These cases were retrospectively examined concerning age, sex, side, surgery duration, complications, and American Orthopedic Foot and Ankle Score (AOFAS). Functional results were statistically evaluated 12 months after the injury. Follow-up included a clinical examination, imaging and AOFAS. RESULTS:Of the patients, 14 were male (63.6%) and eight were female (36.4%). The mean age of the patients was 13.8 years. The mean surgery duration was 21 minutes. All fractures healed after an average of 14 weeks. No complications were observed in any of the cases. The mean follow-up time was 24.7 months. The mean preoperative AOFAS score was 53.3 and the mean postoperative AOFAS score was 93.6, which was a statistically significant difference. At the last follow-up, the AOFAS score was perfect for all cases. CONCLUSION:Contrary to what is stated in the literature, the mini-open surgical procedure has many advantages. The operation is very short and the risk of nerve injury is very low. This study showed that Tillaux-Chaput fractures could be safely and efficiently treated with mini-open surgery. We recommend mini-open surgery, complete anatomical reduction, and internal fixation for successful results.

Introduction: We present the case of a 3.5-month-old boy whose right second toe was affected by hair/thread wrapped around it and this situation's urgency. Case Report: A 3.5-month-old boy was brought by his parents to the emergency clinic. The patient had a swollen, reddish-color right second toe with hyperemia. The parents did not know for how long the toe was affected. When ananmese was deepened, no disorder was learned observing at birth. No other disorder was observed by the parents until they visited the emergency department at noon. No pathology was detected on plain radiographs. By continuing physical examination, a hair-wire wrapping around the toe with linear skin lesion was explored. Lesion was over extensor tendon on the dorsal face and intracutaneous on the plantar face. The wire was meticulously removed, and the dorsal skin lesion was sutured. Proper wound care was performed. On the 10 th day, the sutures were removed. No sequelae were observed on follow-up. Based on the attitude of the parents and the cooperation of medical professionals, the case was considered to be a baby care-related accident. Conclusion: Hair-thread tourniquet syndrome of an affected limb part is a life-threatening condition. Because it is rarely observed, its diagnosis can be delayed. However, ischemia of the affected extremity should alert the physician. Whenever the syndrome is diagnosed, proper treatment must be immediately initiated.
