Objective: to describe the challenges in implementing the chikungunya surveillance and prevention system in Brazil. Methods: this was a descriptive study of suspected cases of the disease based on records held on the Notifiable Diseases Information System (Sinan) for the period 2014-2016. Results: more than 100,000 probable chikungunya cases were notified in Brazil in this period, with the largest concentration in the Northeast states (83.3% between 2014 and 2015; 91.0% in 2016); Sinan provided an excellent opportunity for closing records of cases occurring between 2014 and 2015 (85%) and high completeness of obligatory variables. Conclusion: given the imminence of the introduction of chikungunya in Brazil in 2014, advance public health preparation took place in order to minimize its effects on society; implementation of the surveillance system improved collection of information regarding the disease, however many challenges can be seen in practice, in view of increasing case incidence. This requires greater handling capacity in this sector.
RESUMO Objetivo Compreender os sentidos atribuídos pelos gestores dos Serviços da Região Metropolitana de Goiânia sobre a oferta de Práticas Integrativas e Complementares (PIC) na Atenção Primária à Saúde (APS). Método Estudo descritivo e exploratório, de abordagem qualitativa, realizado entre agosto e dezembro de 2017, com 21 gestores, mediante utilização de entrevistas semiestruturadas que foram gravadas, transcritas e analisadas com a técnica de análise de conteúdo temática. Resultados Os entrevistados demonstraram compreender o contexto em que as PIC estão inseridas, embora tenham apresentado dificuldades e insegurança na sua conceituação. Dos resultados emergiram três categorias, quais sejam: PIC para gestores; aspectos positivos e negativos da oferta das PIC para a equipe de saúde; aspectos positivos e negativos da disponibilização das PIC para os usuários dos serviços. Conclusão e implicações para a prática Conclui-se que há forte influência do modelo biomédico em detrimento da integralidade na organização dos serviços de APS, o que implica com a baixa e descontínua oferta das PIC na Região Metropolitana de Goiânia. Esta conclusão convida para a ampliação dos espaços de reflexão sobre a pluralidade de racionalidades de cuidado no Sistema Único de Saúde.
EDITORIALQualitative research makes possible the development of health knowledge, because it works with complex phenomena whose totality, besides depth, depends on the intensity. For Minayo, the objective of the qualitative research is to make explicit data, indicators, verified trends and/or construct theoretical models applicable to practice. Therefore, its purpose is to study history, representations, relationships, beliefs, perceptions and opinions, representing the result of the interpretations that subjects construct about the production and reproduction of their lives (1) .Qualitative and quantitative research is diverse in nature, but complements. One treats of the magnitude of the phenomena, and the other of its intensity. One seeks what is repeated, being treated in its homogeneity, and the other, singularities and meanings (2) . In a reflexive approach, Taquette insists that qualitative research in health develops, but still shy of its potential. At the international level, qualitative research stands out as a producer of knowledge that subsidizes the clinical practice of nurses (3) . It is appropriate for the understanding of phenomena within their intervention context, establishing connections between concepts, representations, beliefs and behaviors, respecting intersubjectivity. It is also observed how contradictory is the low productivity of studies using the qualitative approach as a method.There is a challenge to develop qualitative research capable of placing this type of research at a level suitable to converge in parallel with quantitative research. Although they are hegemonic in health, it establishes for us, researchers, and expressively translates the potential of qualitative research to the possibilities of greater access to edicts to foster research and to an increase in publication in journals.Furthermore, this edition brings together articles from the Ibero-American Congress on Qualitative Research (CIAIQ -Congresso Ibero-Americano em Pesquisa Qualitativa) held in Fortaleza State, Brazil, in 2018, in search of greater visibility to qualitative research in health. It also presents studies that address the researchers' perception about the development of health research, from a qualitative perspective, using different approaches and techniques of approach and analysis, in different practice settings. Thus, they reveal the object, objectives, method's theoretical frameworks and approach forms with the object in the qualitative perspective.A recent book addresses data analysis in qualitative health research. It points out that data analysis techniques advocate that doing science is working simultaneously with theory, method and techniques, in a perspective where this tripod is mutually conditional. Resumo O objetivo deste artigo é compreender os sentidos atribuídos por trabalhadores da Atenção Primária à Saúde ao processo de formação profissional nas Práticas Integrativas e Complementares. Estudo descritivo, exploratório, com abordagem qualitativa, com 20 profissionais, de 14 unidades de saúde de três municípios na Região Metropolitana de Goiânia. Os dados foram coletados com entrevistas semiestruturadas, transcritas e analisadas por meio da Análise de Conteúdo Temática. Da análise, emergiu a categoria temática sobre as trajetórias de formação nas Práticas Integrativas e Complementares. Nela discute-se que a formação se dá por capacitações proporcionadas pela gestão federal, municipal ou conselhos de categoria profissional, via educação à distância, semipresencial ou presencial. Além disso, são realizadas formações em instituições privadas de ensino custeadas pelos próprios profissionais. Também, fontes informais de informações são usadas para obtenção de conhecimento (internet, livros e revistas). Os resultados revelam, por um lado, a formação insuficiente e difusa, com limitação na oferta e na qualidade, por outro, a necessidade de ampliação de estratégias educacionais que melhorem a formação dos profissionais de saúde para a oferta das diferentes Práticas Integrativas e Complementares na Atenção Primária à Saúde.
Resumo: Introdução: A prática baseada na evidência tem benefícios para a melhoria dos cuidados de saúde e sustentabilidade dos sistemas de saúde. Tal como outras áreas precisa de ser aprendida. Objetivo: Compreender a perspetiva dos estudantes, do curso de licenciatura em enfermagem sobre o seu envolvimento num projeto de transferência de conhecimento para a clínica Métodos: Focus Group com oito participantes. A análise qualitativa dos achados foi efetuada com recurso ao software WebQDA® e seguiu as etapas da codificação; armazenamento/recuperação e interpretação. Seguiram-se os princípios éticos inerentes à investigação. Estudo aprovado por uma Comissão de Ética. Resultados: Emergiram quatro categorias: Pertencer, Usar evidência, Melhorar Cuidados e Desenvolver competências e respetivas subcategorias. A perceção dos estudantes é que a participação em projetos de transferência de conhecimento para a clínica gera oportunidades de aprendizagem relacionadas com o uso e comunicação de evidência, facilita a integração no serviço, a participação, o trabalho colaborativo e o desenvolvimento de competências. Conclusões: A metodologia utilizada é motivante para o estudante que é ativo na busca de evidencia e transferência do conhecimento, e que tem oportunidade de observar melhorias na prática clínica e mudanças nos contextos.
Mechanisms of the larvicidal effect of diflubenzuron on Aedes aegypti larvae, as evidenced by ultrastructural changesDengue is a viral disease transmitted by Aedes aegypti in over 100 countries in the intertropical band in the world and the main forms of control are antivectorial actions. In this paper, ultrastructural changes caused by diflubenzuron (DFB) are presented. The experiments were performed with 3 rd instar larvae of Ae. aegypti with DFB at concentrations of 0.1 and 1 µg/mL. After 24h of exposure, larvae were collected, fixed, dehydrated, embedded, cut, contrasted with uranyl acetate and 3% lead citrate and examined under the electron microscope. Ultrastructural alterations caused by the diflubenzuron inhibitory activity were observed in the cuticle and in the midgut of Ae. aegypti larvae treated with 0.1 and 1 µg/mL, for 24 hours. The alterations observed by scanning microscopy included higher number of bristles, which were thinner and longer than the controls and exhibited a coiling pattern in the intersegmental grooves. Transmission electron microscopy analysis revealed that the epicuticle almost came entirely off the new epicuticle and had no points of reinforcements commonly found in the control. The midgut cells of the larvae treated with diflubenzuron presented a spongy skeleton and in the ultrathin sections, the cells showed intense vacuolation and damage, however with secretion vesicles present and mitochondria preserved. These studies indicate that the diflubenzuron can also act in the midgut cells, being a promising product for use in Ae. aegypti control. This study showed that the DFB interferes with the process of ecdysis, prevents the release of the old cuticle that accumulates in the intersegmental spaces and strangle the segmental portions in a successive and cumulative process, blocked the changes, causing the death of the larva. It also acts destroying the midgut cells.
