RESUMOO uso de versões apenas traduzidas de instrumentos neuropsicológicos padronizados internacionais na clínica brasileira ainda é bastante freqüente. Para que uma ferramenta diagnóstica mensure os processos cognitivos a que se propõe a medir, torna-se essencial uma adaptação de suas instruções e de seus estímulos à realidade brasileira lingüística, cultural e social. O presente artigo de revisão teórica tem por objetivo refletir sobre as particularidades da adaptação de instrumentos neuropsicológicos verbais, propondo um fluxograma de procedimentos de adaptação neuropsilinguística que transcenda à mera tradução. Este fluxograma foi derivado da consulta à literatura de avaliação psicológica, avaliação neuropsicológica, avaliação fonoaudiológica e da experiência clínica e científica das autoras na construção e adaptação de ferramentas clínicas de mensuração de desempenho cognitivo por meio de estímulos verbais. Engloba procedimentos específicos envolvendo três grupos de procedimentos gerais: tradução, análise de juízes e estudo piloto. O contato contínuo com os pesquisadores autores do instrumento original é destacado como uma etapa essencial. A continuidade do processo de adaptação com procedimentos neuropsicométricos é brevemente discutida. ABSTRACT Adaptation of Neuropsychological Verbal Instruments: A Fluxogram of Procedures far Beyond TranslationA very recurrent practice in Brazilian clinics still nowadays is the adoption of translated versions of neuropsychological instruments internationally standardized. It is fundamental that diagnostic tools have their instructions and stimuli adapted to the Brazilian linguistic, cultural and social reality, so that they accurately and reliably measure the cognitive processes that they are meant to assess. This theoretical review article aims to reflect on the specificities underlying the adaptation of verbal neuropsychological instruments, proposing a fluxogram of procedures for neuropsycholinguistic adaptation which transcends mere translation. This fluxogram was derived from the literature on psychological, neuropsychological, phonoaudiological assessments, and from the authors clinical and scientific experience in the construction and adaptation of tools for the evaluation of cognitive performance by means of verbal stimuli. It encompasses specific procedures comprising three groups of general procedures: translation, judges analysis and pilot study. The continuous contact with the researchers who are the authors of the original instrument is highlighted as representing an essential phase. The ongoing adaptation process with neuropsychometric procedures is briefly discussed.
Purpose: to propose a protocol able to analyze both the sensibility as oral motor function in feeding for patients with cerebral palsy. Methods: a selection of sensibility and oral motor function evaluation protocols was done. Then four speech-language pathologists developed tasks of the protocol for the analysis of alimentary difficulties. After its elaboration, the protocol was critically analyzed by 9 speech-language pathologists with experience in orofacial motricity and care of patients with cerebral palsy, and one nutritionist. A consensual analysis was made among the authors of the protocol about the permanence or removal of the tasks following the 0.7 criterion of agreement among the judges. After this analysis the protocol was applied in three individuals with cerebral palsy, one with feeding difficulty complaint and two without complaint. Results: after the judges' analysis, 100% of them agreed with the most of the tasks established in the protocol, the proposed suggestions were: changes in nomenclature and of some foods used in some tasks, for a detailed test, and the possibility of the observation in the chewing function. Conclusion: it was proposed the "Protocolo de Rastreio de Dificuldades Alimentares", a screening tool of alimentary difficulties for individuals with cerebral palsy. Métodos: foi realizada uma seleção de protocolos de avaliação da sensibilidade e função motora oral. Em seguida, quatro fonoaudiólogas confeccionaram as tarefas do protocolo para análise das dificuldades alimentares. Após sua elaboração, o protocolo foi analisado criticamente por nove fonoaudiólogas com experiência na área de motricidade orofacial e no atendimento a pacientes com paralisia cerebral, e por uma nutricionista (juízes). Foi realizada análise consensual entre os autores do protocolo sobre a permanência ou retirada de tarefas, obedecendo ao critério de 0,7 de concordância entre os juízes. Após essa analise, o protocolo foi aplicado em três indivíduos com paralisia cerebral, sendo um com queixa de dificuldade alimentar e dois sem queixa.Resultados: após a análise dos juízes, 100% deles concordaram com a maioria das tarefas estabelecidas no protocolo, as sugestões propostas foram: alteração de nomenclatura e de alguns alimentos utilizados para que fosse possível uma análise mais minuciosa, e a possibilidade de observação de algumas provas na função de mastigação. Conclusão: foi proposto o Protocolo de Rastreio de Dificuldades Alimentares, um instrumento de rastreio de dificuldades alimentares para indivíduos com paralisia cerebral.
test for subsequent refinement and adaptation of this language assessment instrument. Methods: This study was organized into two parts. The first part of the study involved 61 subjects with educational level higher than nine years. Naming of 266 visual stimuli was elicited and concordance among participants on figure naming was also determined. Concordance of responses was assessed using Cochrane's test at a 5% level of significance. Some visual stimuli were subsequently re-designed and images were replaced. The second part of this study involved the re-testing of the new images among 110 individuals ranging in age from 18 to 75 years of both genders, stratified Conclusão:Identificaram-se quais figuras do Teste MT Beta-86 modificado poderão continuar presentes na próxima versão do instrumento e, por meio da análise do baixo reconhecimento populacional, verificaram-se quais pranchas deverão ser substituídas e quais poderão compor a nova versão do teste.
Purpose: to analyze the narrative, descriptive, conversational and procedural discourse of fluent aphasic individuals and to compare their performance with healthy individuals. Methods: the authors selected by a systematic sample 22 fluent aphasic individuals complaining of discursive difficulty and that presented language disorders poststroke. The individuals are or were in treatment in the institution of origin. The aphasic individuals were matched in age and education. Both groups were subjected to a protocol of oral discursive tasks to the evaluation of narrative, descriptive, conversational and procedural discourse. Results: in most of the investigated variables there were statistically significant differences in all types of discourse in the quantitative and qualitative analysis. Conclusion: aphasic individuals presented better performance in narrative discourse and greater difficulty in the other discourse tasks, although when comparing their discourse with the healthy individuals' discourse they presented greater difficulty in all the oral discursive genres. RESUMOObjetivo: analisar os discursos narrativo, descritivo, conversacional e procedural de indivíduos afásicos fluentes e compará-los com indivíduos saudáveis. Métodos: foram selecionados, por meio de amostragem sistemática, 22 indivíduos afásicos fluentes com queixas de dificuldade discursiva e que apresentam alteração de linguagem pós-acometimento em Sistema Nervoso Central decorrente de Acidente Vascular Encefálico, que estão ou já estiveram em tratamento na instituição de origem. Estes foram pareados a indivíduos saudáveis de mesma idade e escolaridade e ambos os grupos foram submetidos a um protocolo de tarefas discursivas orais para avaliação dos discursos narrativo, explicativo, conversacional e procedural. Resultados: houve diferenças estatisticamente significantes na maioria das variáveis investigadas em todos os tipos de discursos tanto na análise quantitativa quanto na qualitativa. Conclusão: indivíduos afásicos apresentaram maior facilidade no discurso narrativo e maior dificuldade nos demais discursos, porém comparando-os aos indivíduos saudáveis apresentaram maior dificuldade em todos os gêneros discursivos orais.
