O artigo analisa o processo de desinstitucionalização resultante do movimento de Reforma Psiquiátrica, discutindo, em especial, o lugar destinado pelas famílias das pessoas com transtornos mentais (PCTM), a partir das alterações nas funções do Estado neste processo. Baseia-se em revisão bibliográfica. No que concerne à trajetória histórica da família brasileira e, mais especificamente, da família no contexto da saúde mental, que deslocam os papéis e lugares dos diferentes grupos familiares na direção do processo de inserção comunitária das PCTM, constatou-se que não basta fechar os manicômios ou reduzir o número de leitos psiquiátricos, se antes não forem modificados os significados desses aparatos nos cernes familiar, comunitário e social. Para que a Reforma Psiquiátrica se efetive, a família tem que ser vista como agente fundamental de transformações da percepção de loucura, bem como ser incluída de forma responsável no processo de cuidado, recebendo suporte e partilhando atribuições juntamente com o Estado e a comunidade.
Estudo sobre a trajetória das políticas sociais no Brasil com recorte ao período de 1930 aos dias atuais, utilizando o método de pesquisa bibliográfica, de modo a mostrar mais especificamente o resgate histórico da política de saúde mental brasileira enquanto política de saúde, identificando suas principais características e os desafios que permeiam esta política no contexto social atual. De acordo com os autores estudados, percebeu-se que o sistema de proteção social brasileiro, mesmo diante dos significativos progressos quanto a ampliação dos direitos sociais, ainda é definido como um sistema de não redistributividade, reflexo da sua construção que foi caracterizada pela centralização política e financeira no governo federal e nas ações sociais, fragmentação institucional, exclusão da população à participação política, o autofinaciamento social, a privatização e o clientelismo que ainda persiste em muitos segmentos sociais. Reflexos estes que também perpassam pelas demais politicas públicas, incluindo a de saúde, bem como saúde mental, que está envolta por grandes desafios na sua trajetória de construção.
Com o objetivo de repensar a família na política de saúde mental, álcool e outras drogas no Brasil, investe-se na reconfiguração dos lugares destinados aos familiares na perspectiva proibicionista, que é pautada na demonização da substância e pela criminalização dos usuários de substâncias psicoativas (SPAs); em confronto com a construção de outras possibilidades contornadas pelo paradigma da redução de danos/atenção psicossocial, que é firmada no cuidado humanizado e cidadão dos usuários de álcool e outras drogas. Trata-se de uma pesquisa teórica e descritiva, baseada em revisão bibliográfica, enfatizando as principais tendências dos estudos frente às posições ocupadas pela família no cuidado do consumidor de substâncias psicoativas. Os resultados da presente pesquisa sinalizam para os tensionamentos acerca dos deslocamentos nos lugares da família no contexto de avanço do ideário neoliberal, que reforça a tendência familista, em que estas assumem a principal responsabilidade pelo bem-estar de seus membros, sobrecarregando-a.
