Background: The aim of this study was to investigate the prevalence of the middle mesial canal (MMC) and radix entomolaris (RE) in mandibular rst permanent molars in a western Chinese population using cone-beam computed tomography (CBCT). Methods: A total of 1174 CBCT images of the mandibular rst molars were collected from West China Hospital of Stomatology, Sichuan University. The following information was recorded and evaluated: the detection rate and location of the MMC and RE, the curvature of the RE, the canal con guration and bilateral symmetry. Results: The detection rates of the MMC and RE were 3.41% and 25.04%, respectively, as calculated by individuals, and 1.79% and 22.15%, respectively, as calculated by total teeth. The average curvature in the buccolingual (BL) orientation (40.63±14.3 9°) was signi cantly larger than that in the mesiodistal (MD)orientation (17.64±7.8 2°) (p<0.05). Of 587 patients, 71.72% (421/587) had bilateral symmetry according to the root canal morphology. The incidence of three-rooted mandibular rst molars was higher in males than in females, while the incidence of two-rooted mandibular rst molars was higher in females than in males. Conclusions: Our results showed that the RE could be detected in almost 1/4 of the western Chinese population; thus, RE detection requires special attention and careful assessment in endodontic treatment.
Introdução: dens in dente é uma anomalia rara de desenvolvimento, caracterizada pela invaginação dos tecidos dentários, podendo variar de um ligeiro aumento da fosseta do cíngulo a um profundo sulco, que pode se estender até o ápice dentário. Objetivo: o objetivo do presente artigo foi relatar um caso clínico e descrever as peculiaridades dos achados clínicos e radiográficos de um dens in dente Tipo III A, com ênfase na descrição de sua morfologia completamente atípica, além das dificuldades que foram encontradas no diagnóstico e durante o tratamento endodôntico. Caso clínico: o paciente compareceu ao ambulatório de uma clínica de Endodontia com queixa de dor e leve edema. Ao exame clínico e radiográfico, observou-se imagem sugestiva de dens in dente no elemento #22, sendo, então, proposto tratamento endodôntico convencional, com estudo tomográfico. Resultados: após 1 ano e 6 meses de acompanhamento, o paciente encontrava-se assintomático. Notou-se reparo satisfatório, com formação óssea, apesar da sobre-extensão do cone. Conclusão: foi possível perceber a importância da tomografia computadorizada de feixe cônico como auxiliar no diagnóstico e planejamento da conduta frente a casos de dens in dente, principalmente por aumentar as chances de sucesso, evitando a perda precoce de elementos dentários.
Introdução: Reabsorção radicular interna é a perda progressiva de dentina intrarradicular como resultado de atividades clásticas. Geralmente, mantém-se assintomática e é diagnosticada durante o exame radiográfico de rotina. O correto diagnóstico é de extrema importância para evitar a terapêutica inadequada, já que, em alguns casos, sua progressão pode resultar na perda do elemento dentário. Objetivo: O objetivo do presente trabalho é relatar um caso clínico de perfuração radicular causada por uma reabsorção interna inflamatória em incisivo central superior, em terço médio da raiz, com integridade do terço apical. Métodos: O tratamento foi realizado usando técnica de obturação com cones de guta-percha e cimento endodôntico à base de resina epóxi no terço apical e agregado trióxido mineral na área da reabsorção, com auxílio do microscópio operatório. Resultado: Após um período de três anos de acompanhamento clínico e radiográfico, pode-se confirmar o processo de reparo, demonstrando que é possível obter sucesso em casos de reabsorção perfurante. Conclusão: Mesmo em casos em que há comunicação da polpa dentária com tecidos periodontais, é possível salvar o elemento dentário com o diagnóstico preciso e tratamento correto.
Root fractures can involve the dentin, cementum, and pulp and commonly can occur as oblique fractures with varying orientations. The aim of this study was to demonstrate the maintenance of pulp health in a tooth with a fractured root without any endodontic treatment and to discuss the advantage of cone-bean computed tomography (CBCT) compared with traditional radiographs in the diagnosis of oblique root fractures. Intra-oral radiography of tooth 11 revealed a horizontal root fracture at the level of the apical third, while the sagittal CBCT slice reveals a complete fracture line running obliquely from the apical third on the buccal aspect through the cervical third on the palatal aspect. Four years follow-up, the tooth has kept its pulp vitality, no tooth discoloration or discrepancy in arch position, without endodontic treatment. This result illustrates spontaneous healing of root fracture including preservation of pulp health. In addition, it confirms the importance of exams in 3 dimensions to correctly locate the fracture and assist in the treatment decision.
