PurposeTo examine the impact of simulated medical consultations using standardized patients (SPs) on the empathy levels of fourth- and sixth-year students at the Unicamp medical school in Brazil.MethodThroughout 2011 and 2012, the authors conducted this study with two classes of fourth-year (n = 124) and two classes of sixth-year (n = 123) medical students. Students completed the medical student version of the Jefferson Scale of Physician Empathy before and after simulated medical consultations with SPs, followed by an in-depth debriefing dealing with the feelings of the patient about the disease, such as fear, guilt, anger, and abandonment; the feelings of the doctor towards the patient; and other topics as they arose.ResultsThe simulation activity increased the empathy scores of the fourth-year students (from 115.8 to 121.1, P < .001, effect size = 0.61) and of the sixth-year students (from 117.1 to 123.5, P < .001, effect size = 0.64).ConclusionsAlthough the study results were obtained via self-report—a limitation—they suggest that the effective simulation of medical consultations with SPs may improve medical students’ empathy levels. One unexpected result was that this activity, during the debriefing, became a forum for debating topics such as the doctor–patient relationship, the hidden curriculum, negative role models, and emotionally significant experiences of students in medical school. This kind of activity in itself may influence young doctors to become more empathetic and compassionate with their patients and foster a more meaningful way of practicing medicine.
Objective: Understanding the meanings of the training experiences of postgraduate students of the Multidisciplinary Residency Program in Health linked to a federal educational institution. Methods: An exploratory and descriptive study with qualitative approach carried out with postgraduate students of a Multidisciplinary Residency Program in Health. The data collection was held through a semistructured interview. The data obtained in the speeches were categorized by the content analysis technique. Results: Four thematic categories were unveiled: the residency and multidisciplinary practice, the residency as practice of teamwork, recognition of the resident's work by users and the residency experience in the context of health work. Conclusion: The Multidisciplinary Residency Program is understood as a significant opportunity of learning and having contact with professionals from different fields where the comprehensive care is provided in professional practice, with the awareness that the assistance should contemplate the social, environmental and psychological aspects of individuals. ResumoObjetivo: Compreender os significados das experiências de formação dos pós-graduandos do programa de Residência Multiprofissional em Saúde vinculado a uma instituição federal de ensino. Métodos: Pesquisa exploratória, descritiva com abordagem qualitativa, realizada com pós-graduandos de um Programa de Residência Multiprofissional em Saúde. Realizada coleta de dados através de entrevista semiestruturada, sendo os dados obtidos nos discursos categorizados através da técnica de análise de conteúdo. Resultados: Foram desveladas quatro categorias temáticas: a residência e prática multiprofissional, a residência como prática do trabalho em equipe, reconhecimento do trabalho do residente pelo usuário e a experiência da residência no contexto do trabalho em saúde. Conclusão: O Programa de Residência Multiprofissional é compreendido como uma oportunidade significativa de aprendizado e contato com profissionais de diferentes áreas, onde se efetua o cuidado integral na prática profissional com a concientização de que a assistência deve contemplar os aspectos sociais, ambientais e psicológicos do indivíduo. Apesar dos avanços tecnológicos, a história médica, o exame físico e a relação médico-paciente continuam as mais importantes ferramentas diagnósticas e terapêuticas. No entanto, deficiências em habilidades clínicas entre alunos têm sido relatadas. Competência clínica deve ser considerada não apenas a demonstração de comportamentos isolados, mas, sim, a habilidade integrada de pensar, sentir e agir na prática real, um determinado contexto. Por isso, ferramentas de observação direta são bastante úteis para avaliar o que os estudantes fazem ou deixam de fazer durante as consultas. O instrumento de avaliação estruturada parece ser adequado tanto para os avaliadores quanto para os avaliados, uma vez que pode ser usado como um guia de observação e também como uma ferramenta de feedback, ampliando as oportunidades de aprendizagem no cenário clínico. Para que esse instrumento seja aplicado de forma eficaz, justa e confiável, os avaliadores precisam ser adequadamente preparados. A B S T R A C TDespite the technological advances, medical histor y, physical examination and patient-doctor relationship continue being the most important diagnostic and therapeutic tools. However, deficiencies in clinical skills among students have been related. Clinical competence is contextdependent; it is more than a demonstration of isolated behaviors, but an integrative ability to think, feel and act in real practice. Tools for direct obser vation are ver y helpful in evaluating what students are and are not able to do during clinical encounters. So, structured assessment seems to be a valuable instrument both for evaluators and students. It can be used as an observational guide and as a tool for feedback, and enhances the learning opportunities in clinical settings. To effectively apply this method, facult y should be provided with effective training to increase fairness and reliability of the assessment strategy.
Muitos estudos recentes incluem a auto-avaliação e a avaliação feita por pares na nota final do aluno em estágio clínico. Por muito tempo as avaliações dos estudantes de Medicina têm sido feitas exclusivamente pelos professores de forma subjetiva. Entretanto, a avaliação dos pares tem sido apontada como um bom indicador do futuro desempenho profissional, sendo considerada consistente e confiável, fornecendo informações que não poderiam ser medidas pelos métodos tradicionais. De forma complementar, a auto-avaliação também contribui para o processo de aprendizagem contínua, pois auxilia os alunos a identificarem seus pontos fortes e fracos. Embora nenhum desses dois métodos de avaliação seja rotineiramente ensinado durante o curso, esse exercício é uma valiosa intervenção para o desenvolvimento e habilitação profissional do médico. Dessa forma, as escolas médicas deveriam ajudar os alunos a compreender os princípios básicos dessas estratégias educacionais, oferecendo treinamento adequado e apoio institucional, incluindo a confidencialidade das informações.
OBJETIVO: Comparar quatro métodos de avaliação com base nas notas obtidas por alunos do quarto ano de Medicina em estágio clínico. MÉTODO: Ao final do estágio clínico em Ginecologia e Obstetrícia, Clínica Médica e Pediatria, em unidades de atenção primária, 106 alunos do quarto ano foram avaliados com base em quatro métodos: Prova Teórica (Pt), Portfólio (Pf), Avaliação Estruturada do Atendimento Clínico (AE) e Conceito Global Itemizado (CGI), (escala 0-10). A análise estatística utilizou coeficiente alfa de Cronbach, teste de Wilcoxon para dados pareados, coeficiente de correlação de Pearson, análise de componentes principais e distância euclidiana. RESULTADOS: As medianas mais baixas foram de PT, e as mais altas, de AE. Houve diferença significativa nas distribuições das notas entre todos os pares de métodos (P < 0,001). A correlação mais forte e a maior concordância ocorreram entre as notas de AE e CGI. A análise de componentes principais contrastou as notas de Pt e Pf com as de AE e CGI. CONCLUSÃO: Cada método parece enfocar diferentes aspectos da competência clínica. AE e CGI parecem avaliar competências semelhantes, em oposição a Pf e Pt. Portanto, nenhum método deve ser utilizado isoladamente para avaliar os alunos em estágio clínico.
This is a descriptive exploratory study with a quantitative approach. It aims to characterize the environment of the Intensive Care Center (ICC) in regard to its physical area, material resources and equipments, and to identify factors concerned to the work of nurses. It investigates environmental factors that contribute to an aesthetically harmonious, functional and humanized space and that favor the performance of nursing work. The units that make up the ICC meet the recommendations of the National Health Surveillance Agency (ANVISA) in the evaluated items. he following favorable work conditions were highlighted: thermal conditions, color of the ceiling, walls and floors, luminosity. The following unfavorable work conditions were highlighted: outdoor spaces, privacy and individuality of clients and noise. The conclusion is that the facilities of the units meet the minimum recommendations of ANVISA. Both favorable and unfavorable environmental work conditions were identified. The creation of better environmental conditions allows a better staff performance, influencing positively quality, safety, and job satisfaction.
