Fast-track drug designation of safe regimens represents an emerging method of development and approval of new medications targeting debilitating diseases including inflammatory bowel diseases (IBD). The goal of accelerated drug approval pathways is to shorten the time between application and approval of therapies that treat diseases with significant morbidity and mortality. Recently, fast-track drug approval approaches were supported by data deriving from central reading of images, a method of clinical data interpretation that has significantly benefited patients with gastrointestinal disorders. Biological agents and other emerging therapies in IBD represent “game-changing” or “treat-to-target” drugs and have satisfied quite successfully some of the patients’ unmet needs. The development of biosimilars is an area where the Federal Drug Administration and the European Agency for Evaluation of Medicinal Products seem to have different approval processes. Biosimilars, including those for IBD, promise cost reductions and wide access to biologic therapies by patients, advantages similar to those already offered by generic drugs. Given the rapid development of IBD drugs and patients’ needs, a consensus among the academic community, clinicians, researchers, sponsors, patients and regulatory authorities is required to standardize better the IBD trials and create a productive environment for fast-track approval of any “changing-game” IBD drug.

ΕΙΣΑΓΩΓΗ: Τα χρόνια νοσήματα, όπως είναι ο παιδικός σακχαρώδης διαβήτης (Σ.Δ.), αποτελούν πρόκληση για τα παιδιά που αντιμετωπίζουν το νόσημα αλλά και για τις οικογένειές τους. Ο Σ.Δ. τύπου 1 χαρακτηρίζεται από μια σύνθετη διαχείριση της θεραπευτικής αγωγής που έχει σωματική και ψυχολογική επίδραση σε ολόκληρη την οικογένεια. ΣΚΟΠΟΣ: Σκοπός της παρούσης διδακτορικής διατριβής είναι η ανίχνευση κοινωνικοδημογραφικών παραγόντων που επηρεάζουν τη ψυχική κατάσταση των γονέων με παιδιά που πάσχουν από σακχαρώδη διαβήτη, όπως αυτή εκφράζεται από την κλίμακα ψυχοπαθολογίας SCL – 90, την κλίμακα εξωτερικής ντροπής (OAS), την κλίμακα για το βίωμα της ντροπής (ESS), την πνευματικότητα και το οικογενειακό περιβάλλον.ΥΛΙΚΟ ΚΑΙ ΜΕΘΟΔΟΣ: Η συλλογή των δεδομένων πραγματοποιήθηκε την άνοιξη του 2016 στα παιδιατρικά νοσοκομεία Αθηνών και Πατρών και στην παιδιατρική κλινική του νοσοκομείου Ιωαννίνων. Στην έρευνα συμμετείχαν 285 γονείς των παιδιών με σακχαρώδη διαβήτη που δέχθηκαν να συμμετάσχουν στην έρευνα. Τα ερευνητικά εργαλεία που χρησιμοποιήθηκαν ήταν: α) η Κλίμακα Οικογενειακού Περιβάλλοντος (Family Environment Scale, Form R-FES), β) το Ερωτηματολόγιο Επιθετικότητας και Κατεύθυνσης της Επιθετικότητας (Hostility and Direction of Hostility Questionnaire - HDHQ), γ) η Κλίμακα Ψυχοπαθολογίας (Symptom Checklist 90-R - SCL-90), δ) η Κλίμακα Αυτοεκτίμησης του Άγχους του Spielberger (State - Trait Anxiety Inventory - STAI), ε) η Κλίμακα Εξωτερικής Ντροπής(Other As Shamer Scale-OAS), στ) η Κλίμακα για το Βίωμα της Ντροπής (Experience of Shame Scale-ESS) και ακόμη ένα ερωτηματολόγιο το οποίο περιλάμβανε ερωτήσεις για τα κοινωνικοδημογραφικά στοιχεία τους.ΑΠΟΤΕΛΕΣΜΑΤΑ: Η παρούσα εργασία είχε δύο στόχους, ο πρώτος εκ των οποίων ήταν η ανίχνευση των παραγόντων που επηρεάζουν την ψυχοπαθολογία των γονέων και ο δεύτερος η διευκρίνιση της επιρροής της ντροπής ως ενδιάμεσος παράγοντας μεταξύ των δημογραφικών παραγόντων και της ψυχοπαθολογίας των γονέων.Στα πλαίσια αυτών των στόχων, βρέθηκε πως η ψυχοπαθολογία των γονέων όπως αυτή εκφράζεται από το γενικό δείκτη συμπτωμάτων, την κλίμακα για το βίωμα της ντροπής και την κλίμακα εξωτερικής ντροπής είναι ασθενώς αρνητικά συσχετισμένη με τη διάρκεια της ασθένειας γεγονός που υποδεικνύει πως με την πάροδο του χρόνου οι γονείς αποδέχονται τις αλλαγές στην προσωπική και τη οικογενειακή τους ζωή που συμβαίνουν λόγω της ασθένειας του παιδιού τους, δημιουργώντας αυτοματισμούς στην καθημερινότητά τους,μειώνοντας με τον τρόπο αυτό τις συνέπειες του αρχικού ψυχοσυναισθηματικού σοκ τόσο σε επίπεδο ψυχοπαθολογίας όσο και σε επίπεδο εσωτερικής και εξωτερικής ντροπής.Στο ίδιο πνεύμα βρέθηκε πως η ηλικία του γονέα έχει άμεση αρνητική επίδραση στην εξωτερική ντροπή και έμμεση αρνητική επίδραση στην σωματοποίηση. Στην παρούσα έρευνα επιβεβαιώνεται η διαφορετικότητα ανδρών και γυναικών καθώς το φύλο «Άνδρας» του γονέα είχε άμεση μειωτική επίδραση στην εξωτερική ντροπή, και αυξητικές επιδράσεις στην κατάθλιψη, το άγχος και τη σωματοποίηση.Επιπλέον, ο τόπος διαμονής, το εκπαιδευτικό επίπεδο, το φύλο του παιδιού και η προέλευση του ονόματος του παιδιού συμμετέχουν σε αλληλεπιδράσεις που επηρεάζουν την ψυχοπαθολογία των γονέων όπως αυτή εκφράζεται στις κλίμακες που μελετήθηκαν.ΣΥΜΠΕΡΑΣΜΑΤΑ: Η αναγνώριση της επιρροής των κοινωνικοδημογραφικών παραγόντων στην ψυχοπαθολογία των γονέων αναδεικνύει ως αναγκαίες στοχευμένες παρεμβάσεις βοηθώντας τους να γίνουν περισσότερο αποτελεσματικοί στην υποστήριξη των παιδιών τους στην αντιμετώπιση της ασθένειας του σακχαρώδη διαβήτη. Background: Disease Management (DM) is an approach to health care that coordinates resources across the entire health care delivery system and throughout the course of a disease. Objectives:The purpose of the present study was to evaluate the DM implementation in a country like Greece, with distinct geographical characteristics and non-symmetrical distribution of health care services. Methods:Recent bibliographic data about DM were gathered from electronic databases emphasizing on the Greek status and the Greek Health Care System (GHCS). Results:The present article discusses the current role, the future contribution and the expected results of DM in Greece. DM still is not well applied in Greece. This is due to the economical, geographical and organisational particularities of the country. There is a big gap between the GHCS and the requirements of the Institute Of Medicine. The percentage of Greek population receiving screening services is low. Posttreatment guidelines for patients are also limited. Numerous initiatives have taken place for the creation of national disease registries but remain incomplete. Despite the increased prevalence of some diseases, few seek health care services, which can be primarily attributed to erroneous beliefs, so the need for education and prevention programs is increasing. Conclusion:We will be able to count for a high level health care system in Greece when the philosophy of DM changes and modifi cations is applied properly, especially when patient education and behavioral commitment will evolve as the major contributors to the successful treatment.

Background: The age profile of the population has shifted globally. Age discrimination has been documented, particularly in the areas of health and employment.Aim: This study aims at examining nursing students' attitudes toward older people and dehumanization of older people among nursing students.Methods: A cross-sectional design was employed in this study. A structured questionnaire was used to collect data, which included a sociodemographic variables questionnaire, the Kogan's Attitude towards Old People Scale (KAOP) scale, a modified version of the Human Nature & Human Uniqueness Scale, the Ten-Item Personality Inventory (TIPI) and the Toronto Empathy Questionnaire.A total of 410 undergraduate nursing students participated in this study.Descriptive statistics were applied while inferential statistics such as Pearson correlation, and regression analysis with the stepwise method were also performed. The significant level was set as p ≤ .05. SPSS-25 was used for the analysis of data. Results:The total mean score of the KAOP scale was 128.84 ± 14.32, indicating that nursing students had a slightly positive view of older individuals. Nursing students scored higher on the mechanistic dehumanization scale than on the animalistic dehumanization one. A strong negative association was found between Mechanistic and Animalistic dehumanization with the KAOP scale and empathy. Year of study, age, and gender were identified as predictor variables for mechanical dehumanization. Conclusion:Personality traits, demographic variables, and nursing students' level of empathy are factors that influence their views about older people and the dehumanization of older people.
