XI. 53.)Vor einiger Zeit berichteten Bally, Schiaaler & Reichsteinl) uber die Glykoside der Samen von S t r o p h a n t h u s mirabilis Gilg. Damals stand nur eine sehr kleine Menge reifer Samen zur Verfugung, so dass nur eine Untersuchung an einzelnen Samen mit Rilfe der Papierchromatographie erfolgen konnte. Inzwischen ist uns aber, wieder durch die Bemuhungen von Lt. Col. W. A . Venour3), eine grossere Menge reifer Samen zugekommen, die eine praparative Untersuchung ermoglichte, uber deren Resultate wir im folgenden berichten.
Apresentação do Caso: Paciente 63 anos, feminina, diabética, hipertensa, obesa e com cirrose hepática provavelmente relacionada a esteato-hepatite não alcoólia. Admitida com quadro de hiporexia, ascite, anasarca, dispneia aos mínimos esforços e ortopneia mesmo com uso de cateter de O2. Apresentava histórico de abuso de AINE, além de tratamento irregular de DM2. RX e TC de tórax com derrame pleural bilateral, moderado à esquerda. TC de abdome mostrando ascite volumosa. Gasometria arterial com hipoxemia (PaO2 de 58mmHg). Ecocardiografia (ECO) com microbolhas evidenciando a presença de shunt arteriovenoso intrapulmonar. Ao exame físico apresentava, em decúbito dorsal, frequência respiratória (FR) de 20 irpm e saturação arterial de oxigênio (SaO2) de 99% e, quando sentada, FR 12 irpm e SaO2 de 93% (platipneia e ortodeoxia). Após ter sido iniciado tratamento com diurético e albumina a paciente apresentou melhora do edema e da ascite, diminuição da dispneia, porém com persistência da platpneia e da ortodeoxia. Discussão: A síndrome hepatopulmonar (SHP) é uma grave complicação caracterizada pela tríade: doença hepática (cirrose e/ou hipertensão portal), hipoxemia arterial e dilatações vasculares intrapulmonares (DVIP). Está presente em 4-32% dos pacientes. A dispneia é o sintoma pulmonar mais comum da SHP. Alguns pacientes podem apresentar platpneia, ou seja, dispneia quando assume a posição supina, que é aliviada com o decúbito dorsal, e/ou ortodeoxia, caracterizada pela diminuição da SaO2 maior que 3 mmHg quando assume a posição ereta. A presença desses sintomas reflete a gravidade da dilatação vascular intrapulmonar, a qual predomina nos terços inferiores de ambos os pulmões. A ortodeoxia é um achado clínico característico, mas não patognomônico da SHP, acometendo até 88% dos pacientes. Na síndrome a gasometria arterial revela elevado gradiente alveolar-arterial de oxigênio e queda na PaO2 (<70mmHg). O ECO com microbolhas é o teste ideal para determinação de vasodilatação intrapulmonar. A síndrome pode ser progressiva e incapacitante. O transplante hepático (TxT) costuma revertê-la, portanto, a SHP que no passado fora considerada como uma contraindicação para o TxT, hoje é uma indicação para o mesmo. Comentários Finais: Quando que observarmos a presença de platpneia e ortodeoxia em um paciente com cirrose devemos considerar e pesquisar a presença de SHP, uma vez que por si só a síndrome é uma indicação para o TxT.
