Introdução: O câncer de mama representa a neoplasia mais frequente entre as mulheres, com elevada morbimortalidade. Com o advento de novos medicamentos, houve um aumento na sobrevida global; entretanto, o trastuzumabe, um anticorpo monoclonal utilizado no tratamento, pode promover cardiotoxicidade, que deve ser avaliada e monitorada durante o tratamento. O objetivo deste artigo é descrever um relato de caso de paciente que apresentou cardiotoxicidade associada ao uso de trastuzumabe e a importância do monitoramento e da identificação precoce da cardiotoxicidade por meio do monitoramento pelo ecocardiograma (ECO). Relato do caso: Paciente, sexo feminino, 63 anos, hipertensa, obesa, com câncer de mama, apresentou cardiotoxicidade associada ao uso de trastuzumabe, com redução da fração de ejeção do ventrículo esquerdo (FEVE) e desenvolveu diabetes após o tratamento. A paciente fez acompanhamento regular pelo ECO durante o tratamento, tendo sido esse processo fundamental para a detecção precoce e o manejo adequado da cardiotoxicidade associada ao uso do trastuzumabe. Foi utilizado um algoritmo na determinação da causalidade. Conclusão: Este estudo mostra a importância do acompanhamento da FEVE pelo ECO em pacientes submetidas à quimioterapia cardiotóxica, e do monitoramento de possíveis alterações metabólicas após o tratamento oncológico.
Introdução: A cardiotoxicidade é um dos efeitos mais significativos no tratamento oncológico. Entre as implicações mais consideráveis, destaca-se a insuficiência cardíaca com disfunção ventricular sistólica ou diastólica. Relato do caso: Paciente do sexo feminino, 39 anos, com linfoma não Hodgkin difuso de grandes células B, submetida à quimioterapia com rituximabe, ciclofosfamida, doxorrubicina, vincristina e prednisona (R-CHOP), que apresentou cansaço aos pequenos esforços e aumento da pressão arterial. O ecocardiograma demonstrou disfunção ventricular esquerda leve com hipocinesia difusa e redução da fração de ejeção do ventrículo esquerdo (FEVE), associadas à possível cardiomiotoxicidade da doxorrubicina. O caso foi identificado em uma pesquisa de prontuário e analisado segundo o algoritmo de Naranjo e, em seguida, foi determinada a gravidade das reações adversas cardiovasculares. Conclusão: Este estudo ressalta a importância da busca ativa de eventos cardiovasculares em prontuários de pacientes submetidos à quimioterapia cardiotóxica e do papel da farmacovigilância nesse contexto.
BACKGROUND: Liver transplantation represents the best therapeutic modality in end-stage chronic liver disease, severe acute hepatitis, and selected cases of liver tumors. AIMS: To describe a double retransplant in a male patient diagnosed with Crohn's disease and complicated with primary sclerosing cholangitis, severe portal hypertension, and cholangiocarcinoma diagnosed in the transplanted liver. METHODS: A 48-year-old male patient diagnosed with Crohn's disease 25 years ago, complicated with primary sclerosing cholangitis and severe portal hypertension. He underwent a liver transplantation in 2018 due to secondary biliary cirrhosis. In 2021, a primary sclerosing cholangitis recurrence was diagnosed and a liver retransplantation was indicated. Recipient's hepatectomy was very difficult by reason of complex portal vein thrombosis requiring extensive thromboendovenectomy. Intraoperative ultrasound with liver doppler evaluation was performed. Two suspicious nodules were incidentally diagnosed in the donor's liver and immediately removed for anatomopathological evaluation. RESULTS: After pathological confirmation of carcinoma, probable cholangiocarcinoma, at frozen section, the patient was re-listed as national priority and a new liver transplantation was performed within 24 hours. The patient was discharged after 2 weeks. CONCLUSIONS: The screening for neoplasms in donated organs should be part of our strict daily diagnostic arsenal. Moreover, we argue that, for the benefit of an adequate diagnosis and the feasibility of a safer procedure, the adoption of imaging tests routine for the liver donor is essential, allowing a reduction of the costs and some potential risks of liver transplant procedure.
Background and Objective: Pancreatic adenocarcinoma remains a dismal disease and is expected to become an even greater burden in the near future. This review focuses on the different surgical aspects for pancreaticoduodenectomy (PD), distal and total pancreatectomy (TP), incorporating lessons from both the western and eastern visions in treating pancreatic cancer.
Methods:We conducted an extensive literature review through PubMed, prioritizing papers published in the last 5 years, but older emblematic papers were also included. We included articles that explored the treatment of pancreatic adenocarcinoma, with focus on the surgical aspect and strategies to improve outcomes. References of selected articles were also reviewed to identify any missed studies. Only papers in English were included.
The aim of this report is to present a case of a breast cancer patient who showed a reduction in the left ventricular ejection fraction and developed the metabolic syndrome after doxorubicin and trastuzumab use. It was analyzed if those drugs may be related to cardiotoxicity or be a possible cause for metabolic syndrome. With increased cancer survival, many patients may still develop metabolic syndrome, which has a multifactorial nature A 46-year-old female patient with infiltrated breast cancer exhibiting reduced left ventricular ejection fraction after chemotherapy neoadjuvant with doxorubicin and then with isolated trastuzumab. Chemotherapy was discontinued, and it was prescribed tamoxifen. The patient was referred to the cardiologist and nutritionist for a follow-up of the metabolic syndrome. Importance of early identification and monitoring of cardiotoxicity, avoiding the cardiovascular complications of cancer treatment and the preventing interruption of therapy.
