Introducción: el estado epiléptico (EE) es una emergencia neurológica con alta morbi-mortalidad asociada, el tratamiento se debe fundamentar en las guías internacionales.
Objetivo: describir la frecuencia de uso de los anticonvulsivantes empleados en el manejo del EE en dos hospitales de cuarto nivel en Colombia.
Materiales y métodos: serie de pacientes mayores de 14,5 años, con diagnóstico de EE establecido por el servicio de Neurología, tratados entre agosto de 2013 y julio de 2014.
Resultados: se incluyeron 84 casos, con una mediana de edad de 52,5 años rango intercuartílico (RIQ 27-71,5); el 64,3% de los pacientes (n=54) tenían antecedentes de epilepsia. El tipo de estado epiléptico más frecuente fue tónico-clónico en el 57,1% (n=48) y la causa principal epilepsia no controlada en el 53,6% (n=46). Para el manejo de primera línea, la mitad de los pacientes recibieron combinaciones de benzodiazepinas con otro anticonvulsivante, los más usados fueron fenitoina y ácido valproico. Todos los fármacos antiepilépticos de primera línea se administraron por vía endovenosa. En 32 pacientes (38,1%) se requirió el manejo en la unidad de cuidados intensivos. El resultado, muerte de 17 pacientes (20,2%) y secuelas neurológicas sin muerte en 6 pacientes (7,2%).
Conclusión: los fármacos antiepilépticos clásicos, entre los que se encuentran la fenitoina y el ácido valproico, continúan siendo la elección en la primera línea para el tratamiento del EE. La combinación de ácido valproico y levetiracetam puede ser efectiva en el EE refractario. Las medidas alternativas durante el tratamiento del EE no fueron observadas.
Nervous system involvement in patients with AIDS is frequent either due to direct infection by the HIV-1 or to opportunistic infections or neoplasms. In the present study we evaluate the epidemiologic characteristics of patients in whom the first AIDS manifestation was an indicative neurologic disease (IND) and the influence of such a presenting form in the patients' survival. Out of 1250 reported cases, 252 (20.2%) presented with one of the INDs, according with the CDC AIDS definition criteria. Neither sex nor age differences were found between patients presenting with and without an IND. IVDUs were more likely to present with an IND than homosexual/bisexual men (p = 0.024). Cerebral toxoplasmosis (CT) was the only IND with a significant proportional increase over time. Although some of the IND have a specific treatment, as a whole patients presenting with an IND lived shorter than those presenting with any of the other indicative disorders (p less than 0.0001). The incidence of IND is greater than elsewhere, mainly because of CT. The increment of CT may be in part due to the introduction of the new AIDS definition criteria.
Introducción: el síndrome de Guillain-Barré (SGB) es una polineurorradiculopatía desmielinizante aguda inmunomediada. El síntoma principal es la debilidad muscular. El objetivo del presente estudio es describir las características del episodio agudo, las secuelas neurológicas a largo plazo y cómo estas afectan la situación laboral y las actividades de tiempo libre. Materiales y métodos: estudio realizado en dos fases: primera, descripción retrospectiva de casos; segunda, corte transversal donde se interrogaron las secuelas neurológicas y sus implicaciones. Resultados: se identificaron 63 casos de SGB con edad promedio de 50,79 años (DE 17.8), más frecuente en hombres (61,9%) y cuyo promedio de la escala Hughes en el episodio agudo fue 3,2 (DE ± 10,1). Las principales complicaciones durante la fase aguda fueron neumonía (7,94 %) y úlceras por presión (3,17 %). Se logró contactar vía telefónica a 28 pacientes, 82,1% presentó al menos un síntoma como secuela, 60,7% parestesias, 57,1% fatiga, 50% calambres y 46,4% dolor de características neuropáticas. El 32,1% tuvo cambios o retiro de su trabajo por las secuelas; 21,43% debió modificar sus actividades de tiempo libre. Conclusión: existe alta prevalencia de síntomas residuales discapacitantes después del episodio agudo de SGB con predominio de síntomas sensitivos, coincidiendo con estudios realizados en países desarrollados. Estas alteraciones ocasionan un impacto negativo en la actividad laboral y en la de tiempo libre de los pacientes.
