Vitamin D deficiency and diabetes mellitus are two common conditions and they are widely prevalent across all ages, races, geographical regions, and socioeconomic conditions. Epidemiologic studies have shown association of vitamin D deficiency and increased risk of chronic diseases, such as cancer, cardiovascular disease, type 2 diabetes, and autoimmune diseases, such as multiple sclerosis and type 1 diabetes mellitus. The identification of 1,25(OH) 2 D receptors and 1-α-hydroxilase expression in pancreatic beta cells, in cells of the immune system, and in various others tissues, besides the bone system support the role of vitamin D in the pathogenesis of type 2 diabetes. Observational studies have revealed an association between 25(OH) D deficiency and the prevalence of type 1 diabetes in children and adolescents. This review will focus on the concept of vitamin D deficiency, its prevalence, and its role in the pathogenesis and risk of diabetes mellitus and cardiovascular diseases. RESUMOA deficiência de vitamina D e o diabetes melito são enfermidades comuns na população e são altamente prevalentes em todas as raças, idades, regiões geográficas e situação socioeconômica. Estudos epidemiológicos mostram uma associação entre hipovitaminose D com o aumento do risco de doenças crônicas, tais como câncer, doença cardiovascular, diabetes melito do tipo 2 e doenças autoimunes como a esclerose múltipla e o diabetes mellitus do tipo 1. A identificação de receptores da 1,25(OH) 2 D e da expressão da 1 α-hidroxilase nas células betapancreáticas, em células do sistema imunológico e em uma variedade de células do organismo além do tecido ósseo, suporta o papel da vitamina D na patogênese do diabetes tipo 2 e do tipo 1. Dos processos articulados de transição demográfica e epidemiológica em curso nas últimas décadas no país, decorre o crescimento da população idosa, particularmente do número de idosos longevos, com idade acima de 80 anos. Este processo de envelhecimento da população impõe uma reorganização do Estado e da sociedade, principalmente em relação aos sistemas de saúde e previdenciário. OBJETIVO: Analisar o perfil socioepidemiológico e o grau de autonomia e independência de idosos longevos na cidade de Recife-PE. MÉTODO: Trata-se de um estudo epidemiológico descritivo, do tipo corte transversal, de amostra aleatória, composto por 227 idosos. Estes responderam ao questionário Brazil Old Age Schedule (BOAS) modificado em entrevistas domiciliares nos seis distritos da cidade. RESULTADOS: Na maioria dos idosos, verificaram-se a feminização, o baixo nível de escolaridade, a viuvez como estado conjugal e a aposentadoria como principal fonte de renda. Os idosos residiam comumente com filhas e netos, e os principais problemas de saúde foram hipertensão, doença de coluna, problemas de visão, incontinência urinária e osteoporose. O grau de autonomia e independência dos idosos longevos foi considerado bom, uma vez que não houve relato de impedimentos em realizar as atividades da vida diária (AVD) pela maioria dos idosos. CONCLUSÕES: Com a tendência de ampliação do número de idosos longevos nos próximos anos, é necessário implementar políticas públicas específicas, adequando programas e ações que contribuam efetivamente para a obtenção da qualidade de vida na velhice avançada.
This investigation reports for the first time the establishment of immortalized clones of dopamine-producing nerve cells in culture. Freshly prepared single-cell suspensions from fetal (12-day-old) rat mesencephalic tissue were transfected with plasmid vectors, pSV3neo and pSV5neo, using an electroporation technique. Cells were plated in tissue culture dishes which were precoated with a special substrate and contained modified MCDB-153 growth medium with 10% heat inactivated fetal bovine serum. The immortalized cells were selected by placing the transfected cells in a selection medium (modified MCDB-153 containing 400 micrograms/ml geneticin). The survivors showed the presence of T-antigens and were non-tumorigenic. Two cell lines, 1RB3 derived from cells transfected with pSV3neo, and 2RB5 derived from cells transfected with pSV5neo revealed only 1 to 2% tyrosine hydroxylase (TH)-positive cells. Repeated single-cell cloning of these cell lines by a standard technique failed to increase the number of TH-positive cells in any clones. Using three cycles of growth, alternating between hormone-supplemented, serum-free medium and serum-containing medium produced a cell line (1RB3A) that was very rich in TH-positive cells. The recloning of 1RB3A yielded clones some of which contained over 95% TH-positive cells. These cells produced homovanillic acid, a metabolite of dopamine, and may be useful not only for neural transplant but also for basic neurobiological studies.
Background: Diabetes and the problem of the diabetic foot specifically are a severe burden on the public healthcare system. Amputations caused by this condition are still common in our setting (Brazil), although the true magnitude of the problem is not known with certainty. Lower limb amputation rates have come to be seen as an indicator of the quality of preventative care of the diabetic foot. Objective: To identify associations between amputations and factors related to people, to morbidities and to primary care received. Methods: This was a cross-sectional study of a sample of 137 patients with diabetic feet admitted to a large hospital in the city of Recife, Pernambuco, Brazil. Logistic regression analysis was conducted. Results: Associations with amputation were detected for the following variables: age over 60; resident of the Metro zone; income of three minimum salaries or less; presence of gangrene on admission; glycemia ≥ 126 mg/dL; smoking; not receiving information about results of glycemia testing; not having feet examined, and not being given guidance on caring for feet at consultations during the previous year. Conclusions: Factors related to Primary Care, such as time since onset of ulcers, information about results of glycemia testing and lack of guidance on how to care for their feet, were associated with occurrence of lower limb amputations. ResumoContexto: O diabetes e especificamente o problema do pé diabético representam grave adversidade ao sistema de saúde pública. As amputações resultantes deste agravo ainda são frequentes em nosso meio, embora não se conheça ao certo a sua magnitude. A taxa de amputações de membros inferiores tem sido considerada um indicador da qualidade dos cuidados preventivos do pé diabético. Objetivo: Identificar a existência de associação entre amputações e fatores relacionados às pessoas, à morbidade e à atenção básica recebida. Métodos: Estudo transversal que incluiu uma amostra de 137 portadores de pé diabético internados em hospital de grande porte da cidade do Recife. Realizou-se análise de regressão logística. Resultados: Verificou-se associação para as variáveis: idade de 60 anos ou mais; procedência da Região metropolitana; renda de até três salários mínimos; presença de gangrena à admissão; glicemia de 126 mg/ dL ou mais; tabagismo; não receber informação dos resultados da glicemia; não ter os pés examinados, e não receber orientação sobre cuidados com os pés nas consultas do ano anterior. Conclusões: Fatores relacionados à Atenção Básica, tais como o tempo de ocorrência da úlcera, a informação dos resultados do exame de glicemia e a falta de orientação sobre cuidados com os pés, estiveram associados com a ocorrência de amputações de membros inferiores.Palavras-chave: pé diabético; amputação; atenção básica.
The implementation analysis of the arterial hypertension care by the Family Health teams in Recife city (Pernambuco, Brazil) was based on a normative evaluation of the structure and work process and analysis of some context elements. An interview was applied to a randomized sample of doctors and nurses of 72 Family Health teams and the hypertension programme coordinator, and official documents were analyzed. A score system was used to classify the performance of the implementation level in excellent (when 90- 100% of activities were implemented); adequate (70-79%); inadequate (50-69%); and critical (<50%). The results show a significant weakness in the arterial hypertension care by the Family Health teams in Recife. The absence of specific programme coordination is related to the low performance observed. The implementation level was considered inadequate and some problems were identified due to the poor infrastructure, deficient inputs, low professional qualification for hypertension care and incipient use of information for planning the actions.
This paper strives to identify the current impact of Non-Transmissible Chronic Diseases (NTCDs) on sickness and disability benefits paid out by Brazil's National Social Security Institute (INSS) between 2000 and 2002. A total of 17,970 new cases were studied, registered at the two local agencies in Recife, Pernambuco State, Northeast Brazil. Initially the cases were divided up according by major diseases groups, following the CID-10 classification. Osteomuscular diseases (OMDs) and cardiovascular diseases (CVDs) were among the main reasons for granting sickness benefits. Among the disability benefits, CVDs, mental disorders (MDs), and OMDs, were the main reasons. In terms of specific diseases within the major DCNT groups, the main reasons for granting sickness benefits were high blood pressure, diabetes mellitus, arthrosis, breast and intestinal cancer, mood disorders and schizophrenia. For disability benefits, the main causes were cerebrovascular diseases, diabetes mellitus, cancer of the gastro-intestinal tract and schizophrenia. Most (66%) of the recipients were men between 39 and 58 years of age, and the initial value of the benefit was of up to three minimum wages per month.
OBJETIVOS: avaliar a implantação da atenção à saúde da criança em dois municípios do estado de Pernambuco, Brasil. MÉTODOS: pesquisa avaliativa, tipo análise de implantação. Consideradas três unidades de análise: unidade do Programa Saúde da Família (PSF) com profissionais treinados na Estratégia Atenção Integrada às Doenças Prevalentes da Infância (AIDPI), unidade sem esse treinamento e unidade tradicional. Para obtenção do grau de implantação foi realizada observação do manejo de casos e estrutura. A percepção dos profissionais e usuários foi apreendida por meio de entrevista e discussão em grupos focais. RESULTADOS: grau de implantação satisfatório para unidades do PSF, insatisfatório para unidade tradicional no Município 1 e insatisfatório para unidades do Município 2. Os profissionais consideraram bons a resolubilidade dos serviços e o relacionamento interpessoal, mas dificuldades no sistema de referência e falta de integração das equipes das unidades tradicionais. Os usuários relataram boa resolubilidade, mas dificuldades no acesso e nas relações interpessoais. CONCLUSÕES: a variação no grau de implantação apresentou como determinantes deficiências no manejo de casos e potencialidade da atenção prestada pelo enfermeiro. Não foi possível definir influência da AIDPI. Há melhor satisfação profissional e dos usuários para unidades do PSF, porém problemas no acesso e integralidade da atenção.
