Изучение факторов риска развития детского церебрального паралича (ДЦП) дает возможность формирования новых подходов к профилактике и раннему выявлению заболевания.Цель исследования. Оценить значимость влияния перинатальных факторов на формирование церебрального паралича у детей.Материалы и методы: ретроспективное изучение первичной медицинской документации и анкетирование матерей.Результаты. В группе детей с ДЦП антенатальный период сопровождался наличием акушерской патологии, выкидышей и абортов у матерей в анамнезе; интранатальный -слабой или стремительной родовой деятельностью, тугим обвитием пуповиной вокруг шеи, асфиксией; постнатальный -недоношенностью, малым весом при рождении, низкой оценкой по шкале Апгар, желтухой, судорогами, апноэ, ИВЛ.Заключение. Детей с наличием вышеуказанных факторов необходимо отнести в группу риска по развитию ДЦП. Ключевые слова: дети, факторы риска, детский церебральный паралич.
ОБЗОРЫ № 3 2016Д етская инвалидность -одна из острейших ме-дико-социальных проблем современного обще-ства. По данным экспертов ВОЗ, в мире количе-ство детей в возрасте до 16 лет с ограничением жизненных и социальных функций составляет около 10 %, т.е. более 120 млн [1].По данным Министерства здравоохранения РФ, в 2010 г. были зарегистрированы 71 429 детей с детским це-ребральным параличом в возрасте до 14 лет и 13 655 детей в возрасте 15-17 лет. Существуют единичные публикации о более высокой распространенности заболевания -до 10 на 1000 новорожденных [3].Основные факторы, определяющие формирование ДЦП, можно разделить, исходя из периода их воздей-ствия, на три основные группы. К первой группе следует отнести повреждающее действие агентов, связанных с па-тологическим течением беременности, аномалиями раз-вития и внутриутробными заболеваниями ЦНС, реализу-ющих свой негативный потенциал антенатально. Вторая группа -это факторы, действующие интранатально, т.е. зависящие от процессов, развившихся в головном мозге ребенка во время родов. Третья группа -постнатальные экзо-и эндогенные воздействия, приводящие к альтера-ции ЦНС [2,4,5,6,7,].Соотношение пренатальных и перинатальных факто-ров поражения мозга при ДЦП, по мнению различных ав-торов [5,8], колеблется. Дородовые формы церебрального паралича варьируют от 35 до 60%, интранатальные -от 27 до 54%, постнатальные -от 6 до 25%. В 80% наблюдений поражение мозга, вызывающее церебральный паралич, происходит в периоде внутриутробного развития плода, а в последующем внутриутробная патология отягощается интранатальной. Тем не менее, в каждом третьем случае причину церебрального паралича установить не удается.Описано более 400 факторов, негативно влияющих на ход нормального внутриутробного развития. Причиной же возникновения церебральной патологии в 70%-80% случаев является воздействие на мозг не одного вредного фактора, а комплекса.К группе антенатально действующих повреждающих агентов, прежде всего, относят острые или хронические экстрагенитальные заболевания матери, в первую очередь гипертоническая болезнь, пороки сердца, анемия, ожире-ние, сахарный диабет, эпилепсия, олигофрения, гипоти-реоз, токсемия, прием лекарств во время беременности, профессиональные вредности, употребление алкоголя, наркотиков, стрессы, психологический дискомфорт, фи-зические травмы во время беременности, инфекции (ци-УДК 616.831-009.1-02-092 Э.В. Цель исследования-разработать методику и создать нейросетевой программный комплекс для прогнозирования риска формирования церебрального паралича у детей. Материал и методы. В исследовании приняли участие дети с диагнозом «церебральный паралич», подтвержденным данными клинических, инструментальных и лабораторных методов исследования, а также дети, не имеющие диагностированных заболеваний центральной нервной системы. Кроме изучения первичной медицинской документации использовали анкетирование матерей с последующим заполнением индивидуальной карты. Статистическая обработка данных проводилась с использованием точного теста Фишера, прикладного пакета программ «Statistica Neural Networks». Программное обеспечение разрабатывали на языке программирования Java с использованием модуля Encog 3.4. Результаты и их обсуждение. На основе искусственной нейронной сети разработана методика оценки влияния 20 перинатальных факторов на риск формирования детского церебрального паралича. Для практического применения создан программный комплекс на языке программирования Java, позволяющий проводить быструю оценку риска развития детского церебрального паралича. Выводы. Были выделены статистически значимые признаки, влияющие на риск формирования церебрального паралича у детей. Разработан нейросетевой классификатор. Рассчитаны показатели значимости параметров, которые имели наибольшее влияние на риск формирования церебрального паралича у детей. Также было выполнено тестовое прогнозирование разработанной экспертной системы с использованием контрольной выборки пациентов.
