El derrame pleural en el mieloma múltiple surge como complicación evolutiva en un 6% de los pacientes (3,6,10), siendo su etiología variada (Insuficiencia cardiaca por amiloidosis, embolismo pulmonar, insuficiencia renal crónica, infecciosa y neoplá-sica), sin embargo la afectación mielomatosa pleural con derrame es inusual (7), encontrándose en menos del 0,8% de casos (6,9,10) siendo en más del 80% de los casos Mieloma múltiple Ig A (3,6,10) quizás como resultado de su mayor tendencia a invadir estructuras extraóseas.Describimos el caso de un paciente con múltiple Ig λ que debutó como derrame pleural.Varón de 55 años de edad con antecedentes de lipomatosis en tórax, hallux valgus en pie izquierdo con dedo en martillo, síndro-me dispéptico, intervenido de pólipos laríngeos y actualmente uno en cuerda vocal izquierda en edema de Reincke, hernia discal L 4 -L 5 . Trabajador de caldería en contacto con lana mineral y amianto, ex-fumador desde hace 14 años de 60 cigarros/día y bebedor de 1,1 l/día de vino.Consultas por un dolor de aparición brusca en base de hemitó-rax izquierdo, de una semana de evolución, que se irradia hacía escápula izquierda, aumentando en decúbito y mejora en bipedestación, con sensación subjetiva de malestar general, sin tos ni expectoración ni disnea. En la exploración destacaba la existencia de piorrea, faringe eritematosa y seca, con disminución del murmullo vesicular en tercio medio y abolición en el tercio inferior de hemitórax izquierdo, con abolición de las vibraciones vocales en dicha zona; el resto de la exploración física fue normal. La analítica mostraba proteínas totales=9,08 g/dl con el resto de los parámetros dentro de la normalidad, tanto el sistemático como la bioquímica sanguínea y la analítica de orina; VSG 1ª hora=10 mm, β2-microglobulina=3,2 mg/dl. Hemostasia y ECG sin alteraciones, y en la Rx de tórax y tomografías apareció derrame pleural izquierdo; GSA Fi O 2 21% con Ph=7,35, pCO 2 =43 mmHg; saturación O 2 =83%; HCO 3 -mosm/l, y pruebas funcionales respiratorias con resultado de obstrucción ligera.Mantoux + de 16 mm; microbiología y anatomía patológica del estupo negativos; ecocardiograma con hipertrofia concéntrica ligera y septal moderada, y serología virus hepatitis A, B y C, VIH, CMV, VEB negativa.El proteinograma detectó la presencia de una banda monoclonal en la región de las gammaglobulinas que correspondió a una Ig G lambda, con Ig M e Ig A disminuidas.La proteinuria de 24 h, el uroproteinograma y la inmunofijación en orina fueron negativas.Ante la sospecha de mieloma múltiple se realizó inmunoelectroforesis en suero detectando paraproteína Ig G λ.La punción esternal y mielograma mostraron un infiltrado de células plasmáticas del 31% formado por plasmoblastos, proplasmocitos, y palasmocitos maduros.La punción pleural y estudio citológico mostró un líquido serohemático, con proteínas=6,5 g/dl, LDH=548 U/l, glucosa=110 mg/dl, colesterol=122 mg/dl, ADA=39 (VN<45U/l)), linfocitos=67%, neutrófilos=30%, la citología detectó abundantes células plasmáticas atípicas. Hemos leído con interés la excelente revisión "Una mirada general a las enfermedades pulmonares intersticiales y una específica a la fibrosis pulmonar idiopática" publicada recientemente en su revista por Marcos et al 1 . Como bien señalan los autores, la presencia de crepitantes, que suelen ser secos y en las bases pulmonares, puede sugerir la presencia de una enfermedad pulmonar intersticial. Asimismo, existe una importante demora entre el inicio de los sínto-mas y el diagnóstico final de la fibrosis pulmonar idiopática.Nos gustaría hacer algunas reflexiones, desde el punto de vista de nuestra práctica clínica habitual, sobre algunas características de los crepitantes y su distribución que puede ayudar en el diagnóstico diferencial. Desde el punto de vista de la terminología, los crepitantes son ruidos anormales adventicios, discontinuos, que se superponen a los ruidos normales. Es frecuente que se refleje en la historia clínica la presencia de "crepitantes secos" o "crepitantes húmedos". Estos son 2 de los 4 términos descriptivos que utilizó Laënnec como hallazgos de la auscultación con su estetoscopio cilíndrico de madera. Esta clasificación como "secos o húmedos", que la mayoría de nosotros usamos en numerosas ocasiones, no se recomienda por algunos autores 2 . Una clasificación posterior los ha diferenciado en crepitantes finos y gruesos. Los gruesos son de tono más grave y tienden a ser menos reproducibles que los finos, modificándose con la tos. Suelen ser el resultado de la acumulación de secreciones en los bronquios. Los crepitantes finos son de tono relativamente alto, inspiratorios y no se modifican con la tos. Muchos investigadores han explorado la potencial especificidad de ciertas características de los crepitantes para entidades específicas, con varios escenarios en los que los hallazgos de la auscultación podrían orientar hacia un diagnóstico 3 . En la sarcoidosis, los crepitantes son menos frecuentes que en la fibrosis pulmonar idiopática. Así, en un estudio 4 publicado en la década de los 70, en el que se intentaba correlacionar los hallazgos clínicos, radiológicos y patológicos, únicamente un 20% de pacientes con sarcoidosis u otras enfermedades granulomatosas presentaban crepitantes en la auscultación frente a un 60% con neumonías intersticiales. Esta baja incidencia de crepitantes en la sarcoidosis ha sido también descrita por otros autores. Así, en la sarcoidosis, podemos encontrarnos con un paciente con una alteración radioló-gica muy expresiva y una auscultación relativamente "limpia". Al contrario, en la asbestosis los crepitantes pueden aparecer precozmente, con radiografía de tórax y pruebas de función respiratoria normales 5,6 . En sujetos con exposición a las fibras de asbesto, la auscultación de crepitantes puede ser un signo temprano de asbestosis.Más frecuente, en nuestra práctica clínica habitual, es que los pacientes con enfermedad pulmonar intersticial difusa tengan factores de riesgo vascular o una cardiopatía conocida asociada.
