Введение. Множественная миелома (ММ) -плазмоклеточная злокачественная опухоль, которая может протекать с формированием костных плазмоцитом. Костная плазмоцитома представляет собой опухолевый пролиферат, развивающийся в костномозговой полости, который может разрушать корковый слой кости с выходом опухоли в окружающие ткани. Гипотетически биологические особенности опухолевого субстрата могут влиять на клиническое течение ММ. Цель исследования -сопоставить клинико-лабораторные параметры впервые диагностированной ММ в зависимости от наличия или отсутствия костных плазмоцитом. Материалы и методы. В ретроспективное исследование включены 40 пациентов с впервые выявленной ММ в возрасте от 24 до 63 лет. У 21 пациента выявлены костные плазмоцитомы. У других 19 больных ММ протекала без формирования плазмоцитом. В качестве индукционной терапии всем пациентам проводилось лечение бортезомибсодержащими схемами, у 10 пациентов терапевтические схемы кроме бортезомиба включали иммуномодуляторы. Трансплантационный этап лечения проведен 34 пациентам. Результаты. при сравнении лабораторных показателей в дебюте ММ у пациентов с костными плазмоцитомами отмечены менее выраженное поражение костного мозга, более высокая концентрация гемоглобина и малая секреция парапротеина. при сравнении клинических параметров ММ оказалось, что частота достижения значимого противоопухолевого ответа после индукции достоверно ниже у пациентов с костными плазмоцитомами: 28,6 % против 68,4 % (p = 0,038). Выполнение трансплантации аутологичных гемопоэтических стволовых клеток (ауто-ТГСК) позволило углубить противоопухолевый ответ в группе пациентов с костными плазмоцитомами: на +100-й день ауто-ТГСК значимый противоопухолевый ответ документирован уже у 52,6 % пациентов. Для достоверной оценки противоопухолевого ответа у больных с костными плазмоцитомами важно учитывать размеры костной плазмоцитомы в динамике. при оценке ответа только по данным иммунохимического исследования частота достижения значимого противоопухолевого ответа завышена, что не является адекватным отражением клинической ситуации. Заключение. ММ, протекающая с костными плазмоцитомами, в своем течении демонстрирует ряд клинико-лабораторных особенностей. Ауто-ТГСК является высокоэффективным методом лечения данной когорты больных. Оценка ответа у больных с костными плазмоцитомами только по данным иммунохимического исследования недостаточна и может служить причиной выбора неверной терапевтической тактики.Ключевые слова: множественная миелома, костная плазмоцитома, трансплантация аутологичных гемопоэтических стволовых клеток, иммунохимическое исследование сыворотки крови и мочи, противоопухолевый ответ
Introduction. Autologous haematopoietic stem cell transplantation (auto-HSCT) is a highly effective treatment for multiple myeloma (MM). Auto-HSCT allows a signifi cant improvement of haematological response leading to higher overall survival and quality of life in MM patients. Nonetheless, the majority of patients develop relapse.Aim — a comparison of clinical MM relapses developing at variant terms after auto-HSCT.Patients and methods. A retrospective study enrolled 65 MM patients aged between 39 and 64 years. All patients had auto-HSCT during 2009–2019, all had achieved complete response (CR) or very good partial response (VGPR) and all since developed immunochemical MM relapse in laboratory evidence. Patients were divided in two cohorts by relapse term, the early (within 12 months of auto-HSCT) and late relapse.Results. Early immunochemical relapse was diagnosed in 13 (20 %), late relapse — in 52 (80 %) patients. The dependence between relapse term and depth of post-auto-HSCT antitumour response has been determined. The proportion of CR patients was signifi cantly higher in late than in early relapse (55.8 vs. 23 %). In follow-up, 60 patients (92.3 %) were initiated on antirelapse therapy, all early relapse and 90.3 % late relapse patients. On day +100 of auto-HSCT, CR patients had later relapse vs. VGPR individuals (median 24 vs. 19.9 months, p = 0.08) with signifi cantly weaker paraprotein secretion resembling the clinical course of monoclonal gammopathy of unclear signifi cance (MGUS).Conclusion. Auto-HSCT allows long-term control of the disease. A signifi cant prognostic factor is antitumour response on +100 day of auto-HSCT. Patients attaining CR have later relapse progressing in a MGUS-like manner. Patients with late indolent relapse can be managed long-term without antitumour therapy.
Background:Multiple myeloma (MM), complicated by bone plasmacytomas, is characterized by some features. The influence of bone plasmacytomas on the survival of MM patients is studied worldwide. A well-known factor of unfavorable prognosis in solitary bone plasmacytoma is the size: a large bone plasmacytoma has one of the linear dimensions exceeds 5 cm (D. Knobel et al.,2006). Learning of the impact of the size of bone plasmacytoma on the prognosis and course of the disease in patients with MM represents a considerable interest. Aims:to evaluate the impact of a size of bone plasmacytoma on the frequency of achieving a deep antitumoral response and survival in patients with MM. Methods:Retrospective single-center study from 2019 to 2022 included 58 patients (M:F-28:30) with MM aged 26 to 83 years (median 57 years), who were diagnosed with bone plasmacytomas at the onset of the disease according to low-dose whole body CT. All patients underwent routine laboratory tests, cytological examination of the bone marrow. Cytogenetic study by FISH was performed in 48 patients. Induction therapy included PAD or VCD in 40 patients, 17 patients received VRD, and 1 patient -DaraVMP. 30 patients underwent auto-HSCT.Statistical analysis was performed using the χ 2 test for qualitative variables. Overall survival (OS) and progression-free survival (PFS) curves were built using the Kaplan-Meier method.
