Цель аналитического обзора-оценить опыт применения костных ксеноматериалов в травматологии и ортопедии. Методы. Поиск литературных источников проводился в открытых электронных базах данных научной литературы PubMed и eLIBRARY. Глубина поиска-20 лет. Результаты. Обнаружено 13 источников, в которых описан опыт клинического применения костнопластических материалов костного ксеногенного происхождения в травматологии и ортопедии. Отмечено, что наибольшая эффективность (от 92 до 100%) обнаруживается в случаях применения ксеноматериала для замещения дефектов при внутрисуставных переломах и ревизионном эндопротезировании. Случаи неудовлетворительных исходов связаны с отсутствием интеграции материала и его отторжением. Наименьшая эффективность (от 41,9 до 46,1%) отмечена при реконструктивных операциях на стопе. Отмечено отсутствие эффекта после применения ксеногенных костных материалов для замещения дефектов при псевдоартрозе. Основное встречаемое осложнение-инфицирование материала. Расчетный процент осложнений при применении ксеноматериалов после суммирования данных литературы составил 7,53% (18 случаев из 239, ДИ 5-95%: 4,53-11,21). Выделено два направления улучшения технических и биологических характеристик костных ксеноматериалов: 1. Модификация исходного ксеноматрикса (развитие технологии очистки, изменение структуры и химического состава матрикса кости); 2. Введение в объем матрикса дополнительных элементов (биологически активные вещества, стволовые клетки). Отмечено, что ксеноматериалы в травматологии и ортопедии могут быть более востребованными за счет уточнения и расширения показаний к их применению. Заключение. Ксеногенный костный материал в современной практике травматологии и ортопедии нашел свое применение для восполнения недостатка костной ткани при ревизионных операциях по эндопротезированию, а также для замещения дефектов кости при некоторых видах переломов. Этот материал относительно безопасен, а возможность его модифицирования позволяет улучшать его биологические характеристики.
