Resumo: Introdução: Os movimentos sociais, organizados em torno da reforma sanitária, contribuíram para a institucionalização do processo de formação em saúde no país que deve estar em consonância com as reais necessidades de saúde da população e, dessa forma, promover a inclusão e equidade na perspectiva da social accountability. Isso se torna ainda mais importante no contexto da pandemia da Covid-19, já que se vivencia uma nova prioridade de saúde. Entretanto, o que se tem observado ao longo da pandemia da Covid-19 é um despreparo profissional para um cuidado integral em saúde que considere as pessoas e comunidades historicamente invisibilizadas. Objetivo: Analisar criticamente as questões das diversidades em relação às Diretrizes Curriculares Nacionais para os Cursos de Graduação em Medicina (DCN). Desenvolvimento: Por meio de três blocos temáticos - “Análise comparativa das DCN de 2001 e 2014 na perspectiva da diversidade”,“Como podemos problematizar as questões da diversidade a partir das DCN de 2014?” “As DCN de 2014 e o que precisamos tornar mais evidente na busca pela diversidade no ensino médico” -, aprofundou-se o debate crítico e reflexivo sobre a educação médica a partir da perspectiva de diversidade. Constatou-se a necessidade de uma articulação formativa e assistencial com as demais políticas públicas em saúde, principalmente aquelas relacionadas a populações marginalizadas. Conclusão: A pandemia da Covid-19 mostra-se como uma oportunidade de a mídia e a sociedade como um todo olharem para as desigualdades sociais em saúde, considerarem a relevância do SUS e enfatizarem os múltiplos apontamentos contemporâneos que evidenciam que os projetos pedagógicos e os componentes curriculares dos cursos de Medicina precisam ser atualizados e se comprometer com a construção de uma proposta de ensino e cuidado em saúde que valorize a diversidade e diminuição das iniquidades em saúde.
Introduction: The number of medical schools in Brazil, as well as the number of vacancies offered at these schools, has grown considerably in the last few years. Since 2013, this increasehas aimedat reaching especially the rural and underserved areas of the country. Objective: Considering that there are many different interests concerning this debate and that this reality directly influences the education and health policies of the country, the aim of this study was to evaluate the number and the distribution of the medical courses,as well as vacancies in these schools in 2020, presenting an updated overview of the Brazilian medical schools. Methods: This was a cross-sectional study, based on data gathered from the Brazilian Ministry ofEducation and Institute of Geography and Statistics (IBGE) website. The utilized variables were the number of courses, number of vacancies offered in each course, characteristics of the cities where the medical schools are located, such as population size, Human Development Index (HDI) and distance to the capital city of each state. Results: Among the institutions that have already initiated their activities, there are 328 active courses, offering 35.480 vacancies for Medical School applicants. There is a difference when analyzing public or private institutions and paid or tuition-free institutions. There is a greater offer of paid courses (74,1%) and of courses located in the countryside (69,8%). Among the courses in the countryside, 27,8% of the vacancies are offered within 100 km of the capital city. Only 7,9% of the annual vacancies are offered in cities with a medium HDI, and the remainder are offered in cities with high or very high HDI. The increase in HDI is related to the higher proportion of private courses offering medical vacancies. It was observed that there is no correspondence between the absolute number of vacancies and the population of the North region, differentfrom what occurs in the other regions of the country. Conclusions: Medical training is under many influences, such as economic and political trends. This discussion needs to consider the regionalization and democratization of access. It was observed that public institutions tend to be located in municipalities that are farther away from the capitals. Even though there is now greater homogeneity between the regions, the Southeast still concentrates almost half of the vacancies in medical courses. Also, the increase in the number of vacancies in private courses brings up the reflection about the socioeconomic profile of medical students who have the opportunity to gain access to this level of education.

Resumo: Introdução: A atual pandemia da Sars-CoV-2 (Covid-19) deixou ainda mais evidentes a desigualdade social e o impacto dos determinantes sociais nos indicadores de saúde. Por conta disso, os profissionais de saúde, para cuidar das pessoas e populações, devem desenvolver ações que promovam a equidade, papel que também cabe às instituições de ensino e saúde. As Diretrizes Curriculares Nacionais para os Cursos de Graduação em Medicina trazem em seu artigo 3º que a formação do(a) graduando(a) deverá ocorrer com “responsabilidade social e compromisso com a defesa da cidadania”. Relato de Experiência: Este relato mostra como o debate da determinação social do processo de saúde e doença vem ocorrendo ao longo dos eventos nacionais de educação médica, especificamente nos Congressos Brasileiros de Educação Médica (Cobem), e como isso resultou na construção coletiva e colaborativa do grupo de trabalho Populações (In)Visibilizadas e Diversidades da Abem (GT-PIVD). Discussão: Os Cobems têm ampliado as oportunidades para debates sobre determinantes sociais por meio de atividades relacionadas às questões LGBTI+, racial, da mulher e indígena. O GT-PIVD, criado em 2019, reúne um coletivo de educandos(as) e educadores(as) com o objetivo de promover esse debate na formação médica e nos eventos da Abem, além de ser uma comunidade de trocas de experiências sobre as populações (in)visibilizadas e diversidades. Conclusão: Algumas iniciativas, como a “Carta de Porto Alegre”, sobre equidade de gênero e diversidade sexual, e a criação do GT-PIVD, são produtos dos Cobem. O GT-PIVD, por meio de sua comunidade de práticas virtuais, tem se tornando um espaço importante de trocas de experiências entre educandos(as) e educadores(as) sobre racismo, sexismo, LGBTIfobia e outras violências, inclusive durante a pandemia da Covid-19.
Introdução: A vasa prévia (VP) é uma condição obstétrica caracterizada pela inserção anômola do funículo umbilical fetal. Consequentemente, tem-se a anteriorização dos vasos fetais em relação à sua apresentação no seguimento uterino inferior. É uma condição rara, que acomete 1 a cada 2.500 gestantes, podendo ocasionar graves complicações. O diagnóstico precoce por meio da ultrassonografia (USG) durante o pré-natal é o principal método para prevenir intercorrências na gestação. Objetivo: Este trabalho visou salientar a importância do seguimento obstétrico e a realização da USG para identificação precoce dessa condição, além de disponibilizar as atualizações sobre o manejo proposto por diferentes entidades da área. Material e Métodos: Realizou-se uma revisão sistemática da literatura nas bases de dados PubMed, Literatura Latino-americana e do Caribe em Ciências da Saúde (LILACS), Scientific Electronic Library Online (SciELO) e UpToDate. Foram selecionadas as publicações que abordaram o diagnóstico, o prognóstico e a conduta nessas alterações, no período de 2011 a 2020. Priorizaram-se os artigos recentes e coerentes com a prática médica atual, com o maior nível de evidência, além dos consensos de sociedades médicas. Resultados e Conclusão: Diante desse cenário obstétrico, definem-se dois tipos de VP. O primeiro decorre da disposição dos vasos fetais desprotegidos entre placenta e cordão umbilical, que se encontram sobre o colo uterino ou próximo a ele. O segundo ocorre quando a placenta é lobulada e os vasos fetais que conectam os lobos se encontram próximos ao colo uterino. Entre os fatores de risco, pode-se citar: placenta prévia, cesariana anterior, cirurgia uterina prévia, lobulação placentária, gestação por fertilização in vitro e gestação múltipla. Em função da vulnerabilidade na disposição dos vasos fetais, a principal complicação decorre da sua ruptura, ocasionando desde hemorragia exacerbada à morte fetal, ou mesmo comprometimento da circulação fetal em caso de pressão mecânica sobre eles. Com o avanço na qualidade diagnóstica da USG de rotina durante o pré-natal, facilitou-se o diagnóstico precoce, o que possibilitou intervenção adequada com cesariana eletiva entre 34 e 36 semanas e garantiu aumento da sobrevida fetal. Além disso, a detecção prévia dessa afecção diminui a necessidade de assistência de urgência ao recém-nascido. Para tanto, o manejo do quadro clínico da gestante deve ser individualizado e baseado em condutas atualizadas e com alto nível de evidência. Diante das variadas complicações da VP, torna-se necessária a suspeição pelo médico diante de sinais clínicos, como sangramento vaginal associado à bradicardia fetal para intervenção adequada. Inclusive, é fundamental a adesão das gestantes às consultas de pré-natal, incluindo seguimento ultrassonográfico, assim como a realização dos exames de rotina preconizados, com o intuito de se diagnosticar precocemente tanto essa condição quanto outras e prolongar a gestação o maior tempo possível.
