La histoplasmosis es una enfermedad causada por el hongo dimorfo Histoplasma capsulatum, que involucra al tejido linfático, pulmones, hígado, bazo, glándulas suprarrenales, riñones y piel. En los últimos años ha emergido como una enfermedad oportunista, principalmente en aquellos individuos con inmunidad celular alterada, como es el caso de los pacientes con SIDA. Cerca de 70% de los pacientes sudamericanos con histoplasmosis y SIDA presenta lesiones cutáneas o mucosas. En Argentina y Paraguay, la histoplasmosis es la micosis sistémica más frecuente en individuos con infección por el VIH.Presentamos el caso de una paciente con diagnóstico de histoplasmosis sistémica cuya única manifestación es la lesión cutánea, confirmado con la anatomía patológica y, fundamentalmente, con el cultivo, constituyendo una presentación clínica inusual.

La vasculitis leucocitoclástica es una vasculitis de vasos pequeños, ha sido reportada en pacientes con COVID-19 (1)(2)(3)(4)(5) .Se presenta la imagen de lesiones de vasculitis en una mujer de 59 años con cuadro de 4 semanas de evolución con lesiones violáceas, necróticas en la cara (Figura 1), extremidades, abdomen. La paciente un mes antes había presentado COVID-19 (Figura 2 y Figura 3). Todas las serología para enfermedades autoinmunes fueron negativas, crioglobulina y criofibrnogeno (negativo), serología HIV, toxoplasmosis, citomegalovirus, herpes (negativo). Serología para SARS-COV-2 IgM-IgG (positiva). La biopsia de piel: vasculitis leucocitoclasia. Recibe tratamiento con pulsos de metilprednisolona con regresión de las lesiones.
Objetivos: determinar el nivel de actividad de la artritis reumatoide mediante el DAS 28 en pacientes ambulatorios del Hospital Nacional.Métodos: se realizó un estudio observacional, descriptivo de corte transversal donde se incluyó a 82 pacientes ambulatorios con diagnóstico de artritis reumatoide del Hospital Nacional. Se midieron variables objetivas con el valor de la eritrosedimentación, el número de articulaciones tumefactas, variables subjetivas como el número de articulaciones dolorosas y la valoración global de la salud estimada por el paciente.Resultados: la edad estuvo comprendida de 19 a 74 años con una media de 49 ± 13 años. El 81% fue del sexo femenino y 56% se dedicaba a los que haceres domésticos. El nivel de actividad fue moderado en 45% y 31% presentaban remisión.Conclusión: el nivel de actividad de los pacientes con artritis reumatoidea en los pacientes ambulatorios fue moderado en 45% y 31% en remisión.
