AbstrakMagnesium sulfat sebagai antagonis reseptor N-methyl-D-aspartate (NMDA) dan penghambat saluran kalsium, memiliki efek antinosiseptif dan antihiperalgesia. Penelitian dilakukan secara acak, terkontrol, buta ganda bertujuan untuk menilai efek pemberian bolus magnesium sulfat intravena terhadap nilai visual analog scale (VAS) dan jumlah kebutuhan analgetik petidin pada 30 pasien wanita dengan status fisik ASA I-II, usia 18-60 tahun, yang akan menjalani operasi abdominal ginekologi elektif dengan anestesi umum di ruang operasi bedah sentral Rumah Sakit Dr. Hasan Sadikin Bandung pada Juni-September 2011. Subjek penelitian dibagi menjadi dua kelompok yang akan mendapat bolus dan rumatan MgSO 4 intravena (grup M) atau NaCl 0,9% (grup S). Hasil penelitian menunjukkan nilai VAS dan jumlah pemberian analgetik petidin pada grup M secara statistik lebih rendah dibandingkan dengan grup S (p<0,05). Simpulan penelitian adalah pemberian bolus magnesium sulfat intravena perioperatif mampu menunjukkan nilai VAS saat mobilisasi pascabedah yang lebih rendah serta mengurangi kebutuhan analgetik pertolongan petidin pada pasien pascabedah abdominal ginekologi dalam anestesi umum.Kata kunci: Analgetik pascabedah, magnesium sulfat, nilai VAS saat mobilisasi, operasi abdominal ginekologi The Effect of Perioperative Magnesium Sulphate Infusion on VAS (Visual Analog Scale) Scores and Postoperative Analgesic Requirements in Patients Undergoing Gynaecological Abdominal Surgery with General Anaesthesia AbstractMagnesium sulphate is N-methyl-D-aspartate (NMDA) receptor antagonist and calcium channel blocker with antinociceptive and antihyperalgesia effects. A randomized, double blind, controlled study was conducted to evaluate the effect of perioperative magnesium sulphate infusion on visual analog scale (VAS) scores and cumulative rescue analgesic petidin consumption in 30 ASA physical status I-II female patients, aged 18-60 years, scheduled for gynaecological surgery under general anaesthesia in central operating theatre Dr. Hasan Sadikin Hospital-Bandung within June-September 2011. Subjects were divided into two groups that received either intravenous bolus and maintenance of MgSO 4 (M group) or 0.9% normal saline (S group). The results showed that postoperative VAS score during movement and the number of analgesic pethidin were significantly lower in M group compared to S group (p<0.05). In conclusions, intravenous bolus of magnesium sulphate perioperative are able to demonstrate the lower value of VAS during mobilization and reducing the amount of analgesic rescue petidin postoperative abdominal gynaecological surgery.
Sejak tahun 2007 penyebab lain ensefalitis diketahui adalah autoimmun selain virus sebagai penyebab terbanyak (69%) dengan angka kematian di dunia 8-18,45%. Ensefalitis anti-NMDAR adalah ensefalitis autoimun akibat reaksi antibodi terhadap antigen membran ekstraseluler, subunit NR1, yaitu reseptor glutamat NMDA di sinapsis susunan saraf pusat. Kasus ensefalitis NMDAR sangat jarang dijumpai, berdasarkan penelitian yang teridentifikasi hanya 1% pasien usia 18-35 tahun yang dirawat di Unit Perawatan Intensif (UPI), namun dengan penatalaksanaan yang tepat dapat meningkatkan prognosis pasien. Pasien laki-laki, usia 36 tahun dikonsulkan ke UPI RSHS setelah 10 hari rawatan di Neuro High Care (NHC) pada bulan Agustus 2019 dengan penurunan kesadaran disertai riwayat kejang berulang dengan diagnosis ensefalitis anti-NMDAR. Kombinasi obat anti-viral, anti-epilepsi, dan metilprednisolon yang diberikan sebelumnya di ruangan tidak memberikan perbaikan klinis bermakna. Selama perawatan di UPI pasien diberikan terapi plasmafaresis dan menunjukkan perbaikan secara signifikan. Bangkitan kejang berulang dapat berupa kejang parsial, kejang generalisata dan status epileptikus. Manifestasi kejang diduga terjadi karena terbentuknya antibodi yang menyerang reseptor glutamat NMDA dengan target utama NR1. The American Society for Apheresis merekomendasikan plasmaferesis sebagai pilihan terapi utama. Simpulan, terapi plasmafaresis dalam tata laksana kasus ensefalitis anti-NMDAR merupakan pilihan tepat karena terbukti efektif dan efisien dalam perbaikan klinis pasien melalui mekanisme penurunan titer antibodi terhadap reseptor NMDA.
Penatalaksanaan jalan napas merupakan hal fundamental bagi ahli anestesi. Kegagalan penatalaksanaan jalan napas mengakibatkan kematian. Berbagai modalitas tersedia untuk penatalaksanaan jalan napas sulit, mulai dari alat sederhana seperti stylet dan bougie hingga alat canggih seperti video laryngsocope dan fiberoptic. Fiberoptik masih menjadi standar baku kesulitan intubasi, namun penggunaannya masih terbatas karena harganya mahal dan penggunaannya sulit. Purwarupa camera–bougie merupakan modalitas baru yang diharapkan dapat mengatasi keterbatasan tersebut dan menjembatani antara bougie yang sederhana dan fiberoptik yang sangat canggih. Tujuan penelitian adalah membandingkan angka keberhasilan intubasi dan waktu intubasi antara bougie dan purwarupa camera–bougie pada maneken simulasi kesulitan intubasi di RSUP Dr. Hasan Sadikin Bandung. Penelitian analitik eksperimental ini dilakukan pada 41 peserta didik Program Pendidikan Dokter Spesialis (PPDS) Anestesiologi dan Terapi Intensif Fakultas Kedokteran Universitas Padjadaran/RSUP Dr. Hasan Sadikin Bandung semester 5 sampai dengan 11 yang melakukan intubasi dengan bougie dan purwarupa camera–bougie secara bergantian dengan metode randomisasi permutasi blok pada maneken simulasi kesulitan intubasi. Penelitian dilakukan dari tanggal 16–24 Oktober 2018. Keberhasilan dan waktu intubasi dicatat dan dianalisis secara statistik dengan Uji Mc. Nemar dan Wilcoxon. Keberhasilan intubasi dengan bougie sebesar 39% dan purwarupa camera-bougie 100% (p<0,001). Waktu intubasi dengan bougie dan purwarupa camera-bougie sebesar 18,81 (12,19) detik dan 7,0 (1,47) detik (p<0,001). Simpulan, purwarupa camera–bougie meningkatkan keberhasilan intubasi dan memperpendek waktu intubasi pada maneken simulasi kesulitan intubasi.Comparison of Success Rate and Duration of Intubation between Bougie and Bougie–Camera Prototype in Simulated Difficult Airway ManikinAirway management is fundamental for anesthesiologist. Fiberoptic is still the gold standard for difficult intubation but its expensive price and complicated handling limit its use. Bougie–camera prototype is one of the new modalities that is expected to overcome these limitations and bridge the gap between simple bougie and very sophisticated fiberoptic. The aim of this study was to compare the success rate and duration of intubation between bougie and bougie–camera prototype in simulated difficult airway manikin at Dr. Hasan Sadikin General Hospital Bandung. This experimental analytic study was conducted on 41 fifth semester anesthesiology residents of the Faculty of Medicine, Universitas Padjadjaran/Dr. Hasan Sadikin General Hospital who performed intubation with bougie and bougie–camera prototype alternately on simulated difficult airway mannequin using permutation block randomization method. This study was held during the period of 16–24th of October 2018. The success rate and duration of intubation were recorded and analyzed statistically by Mc. Nemar and Wilcoxon tests. The rate of successful intubation with bougie was 39% and 100% with bougie–camera prototype (p<0.001). Duration of intubation with bougie and bougie–camera prototype was 18.81 (12.19) seconds and 7.0 (1.47) seconds (p<0.001). The conclusion of this study is bougie–camera prototype increases the success rate of intubation and shortens the duration of intubation on simulated difficult airway on mannequin.
Wolff-Parkinson-White (WPW) syndrome is a commonly undiagnosed cardiac rhythm anomaly in a previously healthy patient who may precipitate malignant arrhythmia under surgical stress. We report successful management of a reconstruction surgery patient who developed cardiac arrest under general anesthesia due to undiagnosed WPW syndrome and a malignant arrhythmia during subsequent emergency surgery. A male patient with no previous history of the co-existing disease, age 23 years old underwent 14 hours of leg reconstruction with a posterior back flap. At the end of the surgery, the patient developed malignant arrhythmia that worsens to pulseless ventricular tachycardia. High-quality resuscitation was conducted and resulted in the return of spontaneous circulation. The patient had to undergo emergency surgery the next day, and another episode of intraoperative malignant arrhythmia was treated with propafenone and diltiazem. The patient underwent ablation postoperatively and, on the 14th day, was discharged without any residual complications. In conclusion, WPW may appear asymptomatic in a healthy young patient. Good anesthesia management and monitoring, knowledge of selective antiarrhythmic drugs and high-quality resuscitation skills can provide an optimal outcome in an unpredicted intraoperative crisis.
