This study aimed to develop a short version of an instrument to detect cognitive impairment in stroke patients, investigate which cognitive dimensions best discriminate between stroke patients and healthy adults and to graphically analyze the relationships among the neuropsychological variables and groups. This pilot study included 94 adults (49 post-stroke and 45 neurologically healthy) who answered the Brief Neuropsychological Assessment Battery NEUPSILIN for patients with expressive aphasia (NEUPSILIN-Af) to assess orientation, perception, memory, praxis, executive functions, oral language, and academic achievement (written language and arithmetic). The IRT Rasch model for dichotomous data indicated the exclusion of items that could not be used to discriminate performances. ROC curves indicated that only the orientation, oral language, academic achievement, and executive function dimensions could be used to differentiate between the clinical and healthy groups. Graphical analysis indicated that independently of the relation among variables, orientation and executive functions tasks are essentials in the neuropsychological assessments. This study contributes to the development of specific and sensitive neuropsychological instruments to assess stroke patients and to better understand the common deficits present in this clinical population.
ObjectiveThis paper reviews the methodological characteristics of studies on rehabilitation of expressive aphasia, describing the techniques of rehabilitation used.MethodsThe databases Medline, Science Direct and PubMed were searched for relevant articles (January 1999 to December 2011) using the keywords Expressive / Broca / Nonfluent Aphasia, combined with Language or Speech Rehabilitation / Therapy / Intervention.ResultsA total of 56 articles were retrieved describing rehabilitation techniques, including 22 with a focus on lexical processing, 18 on syntax stimulation, seven with the aim of developing speech and nine with multiple foci.ConclusionA variety of techniques and theoretical approaches are available, highlighting the heterogeneity of research in this area. This diversity can be justified by the uniqueness of patients' language deficits, making it difficult to generalize. In addition, there is a need to combine the formal measures of tests with measures of pragmatic and social skills of communication to determine the effect of rehabilitation on the patient's daily life.

Fontes dos dados:Foi realizada revisão da literatura sobre o tema proposto na MEDLINE nos últimos 5 anos e foram incluídas referências de livros-texto considerados como base para a compreensão dos distúrbios da linguagem e do aprendizado na infância.Síntese dos dados: Sabe-se que as causas de alterações de linguagem e de dificuldades de aprendizagem podem ser variadas, apesar de existirem muitos estudos indicando fatores neurológicos para tais problemas.Conclusão: Ressalta-se a importância de uma adequada investigação buscando um correto diagnóstico para, então, direcionar o melhor tratamento indicado para cada caso. As dificuldades de linguagem referem-se a alterações no processo de desenvolvimento da expressão e recepção verbal e/ou escrita. Por isso, a necessidade de identificação precoce dessas alterações no curso normal do desenvolvimento evita posteriores conseqüências educacionais e sociais desfavoráveis 1 .O objetivo deste estudo é instrumentalizar os profissionais da saúde, em especial o pediatra, para que possam agir no diagnóstico e na prevenção primária dos distúrbios de linguagem oral e escrita. -Purpose: To identify brain dominance for language functions with DLT and correlate these results with those obtained from fMRI in patients suffering from intractable temporal lobe epilepsy. Method: This study reports on 13 patients who underwent pre-surgical epileptic evaluation between April and October 2004 at the Epilepsy Surgery Program, Hospital Sao Lucas, PUCRS. In DLT, dominance was assessed through a consonant-vowel task, whereas in fMRI patients performed a verb generation task. Results: Our results identified a correlation between the fMRI lateralization index and the DLT ear predominance index and reply difference index (r=0.6, p=0.02; Pearson Correlation Coefficient), showing positive correlation between results obtained from fMRI and DLT. Conclusion: DLT was found to significantly correlate with fMRI. Método: Foram estudados 13 pacientes com epilepsia refratária de lobo temporal, que realizaram avaliações pré-cirúrgicas no período de abril a outubro de 2004 no Programa de Cirurgia de Epilepsia do Hospital São Lucas da PUCRS. Realizada investigação da dominância hemisférica para linguagem através do TED Consoante-Vogal e da RMf pela geração de verbos. Resultados: Verificou-se a existência de correlação entre os índices de lateralidade (RMf) e os índices de predomínio de orelha e de diferença de resposta (TED) (r=0,6, p=0,02). Há diversos de estudos sobre a relação entre nutrição e cognição ou nutrição e emoção. Porém a ciência da nutrição transcende o nutrir e o comer. Envolve fatores interligados que precisam ser verificados e utilizados para melhor compreensão do indivíduo e das populações atendidas pelo nutricionista e da proposição de modos de resolução e/ou melhora destas demandas. Para tanto, existem instrumentos que incrementam a avaliação alimentar e nutricional e proporcionam melhor compreensão da formação do hábito alimentar. Este ensaio teórico sobre instrumentos neuropsicológicos utilizados para avaliação de adultos e idosos contribui para a avaliação nutricional, especificando as funções cognitivas e suas relações com os fatores nutricionais e emocionais do indivíduo. Realizou-se levantamento dos instrumentos de avaliação neuropsicológica ou que possuam construtos cognitivos e emocionais. Verificou-se a avaliação cognitiva e a relação da emoção com a comida. Pensa-se que este seja um guia básico para o nutricionista incrementar sua avaliação para além da tradicional.
RESUMOObjetivo: Identificar as regiões cerebrais responsáveis pelas funções de linguagem através de Ressonância Magnética funcional (RMf) em pacientes com epilepsia refratária de lobo temporal. Metodologia: Foi realizada RMf utilizando-se o paradigma de geração de verbos. Os pacientes ouviam palavras concretas e eram orientados a pensar na sua finalidade, sem verbalizar (ex: faca-cortar). Utilizou-se o cálculo de índice de lateralidade (IL) e a verificação do grau de ativação das áreas classicamente envolvidas na linguagem. Resultados e conclusões: Constatou-se que 92,3% dos pacientes investigados apresentaram linguagem em hemisfério esquerdo, e 7,7%, linguagem mista. Observou-se que existe maior freqüência de pacientes com ativação nas regiões de Broca esquerda, Córtex Pré-motor esquerdo, área de Wernicke esquerda e Córtex Auditivo esquerdo. ABSTRACTIdentifying language cerebral functions: A study of functional magnetic resonance imaging in patients with refractory temporal lobe epilepsy Purpose: The purpose of this study was to identify the brain regions for language functions with the functional Magnetic Resonance Image (fMRI) in patients suffering from intractable temporal lobe epilepsy. Methods: During fMRI, patients performed a verb-generation task. They heard concrete words through headphones and were asked to think of their utility (e.g. pencil -to write), without verbalizing or making any facial or tongue movements, keeping silent and with eyes shut. A laterality index (LI) was calculated and we verify the brain regions activities for language. Results and conclusions: Our results identified that that 92.3% of the patients analyzed by fMRI presented left hemisphere language and 7.7% presented mixed language representation. It was observed more patients with activation in left Broca area, left pré-motor cortex, left Wernicke area and left hearing cortex.
