El sistema respiratorio es el principalmente afecto en la infección por SARS-CoV-2, produciendo el síndrome respiratorio agudo severo como principal complicación en COVID-19, además los pacientes con enfermedad severa desarrollan insuficiencia multiorgánica; entre estos destacamos el sistema nervioso, por su potencial neurotropismo. Los datos clínicos recientes revelan que los pacientes pueden manifestar síntomas como anosmia, disgeusia, deterioro de conciencia, cefalea, convulsiones y enfermedad cerebrovascular (ECV), incluida la hemorragia cerebral y el accidente cerebrovascular isquémico. Se describen dos pacientes varones físicamente activos con infección complicada por SARS-CoV-2 sin comorbilidades significativas para el desarrollo de hemorragia intracraneal, ni antecedentes de traumatismo craneoencefálico o malformaciones anatómicas documentadas. Ambos ingresaron por el área de emergencia y en su estancia en UCI desarrollaron hemorragia intracraneal diagnosticada por tomografía computarizada. Los hallazgos paraclínicos en los dos casos fueron compatibles con un estado protrombótico, como posibles etiologías del sangrado en ambos pacientes.
Introducción: Existe evidencia de la asociación entre valores de hemoglobina glicosilada (HbA1C) con hemorragia intracerebral espontánea (HICE). Sin embargo, los reportes en pacientes latinoamericanos son escasos. Objetivo: Determinar si HbA1c es factor de riesgo de HICE. Métodos: Se realiza un estudio de casos y controles. Los casos fueron pacientes hospitalizados con HICE, los controles fueron pacientes diabéticos ambulatorios del mismo grupo etario. Calculamos la asociación entre factores de riesgo de HICE mediante la regresión logística. Resultados: Se identificaron 45 casos y 45 controles. Los valores de HbA1c en el grupo con HICE (mediana 6.8%, RIQ=5.8–7.5) fueron superiores que los controles (mediana 5.8%, RIQ=5.5–6.2%; P<0.001). La HbA1c fue factor de riesgo asociado con HICE (OR=2.75, 95% IC=1.41-5.41). De los pacientes con HICE, solo el 29% había recibido diagnóstico previo de diabetes. La tasa de mortalidad hospitalaria en los pacientes con HICE fue de 37.8%, siendo 15% en los pacientes con diabetes y 46% en aquellos sin diagnóstico de diabetes. Conclusiones: Valores elevados de HbA1c se asociaron con el desarrollo de HICE. Es necesario mejorar las políticas públicas de detección temprana de diabetes ya que podrían reducir el impacto de hemorragia intracerebral espontánea en la población general.
