Introducción: Existe evidencia de la asociación entre valores de hemoglobina glicosilada (HbA1C) con hemorragia intracerebral espontánea (HICE). Sin embargo, los reportes en pacientes latinoamericanos son escasos. Objetivo: Determinar si HbA1c es factor de riesgo de HICE. Métodos: Se realiza un estudio de casos y controles. Los casos fueron pacientes hospitalizados con HICE, los controles fueron pacientes diabéticos ambulatorios del mismo grupo etario. Calculamos la asociación entre factores de riesgo de HICE mediante la regresión logística. Resultados: Se identificaron 45 casos y 45 controles. Los valores de HbA1c en el grupo con HICE (mediana 6.8%, RIQ=5.8–7.5) fueron superiores que los controles (mediana 5.8%, RIQ=5.5–6.2%; P<0.001). La HbA1c fue factor de riesgo asociado con HICE (OR=2.75, 95% IC=1.41-5.41). De los pacientes con HICE, solo el 29% había recibido diagnóstico previo de diabetes. La tasa de mortalidad hospitalaria en los pacientes con HICE fue de 37.8%, siendo 15% en los pacientes con diabetes y 46% en aquellos sin diagnóstico de diabetes. Conclusiones: Valores elevados de HbA1c se asociaron con el desarrollo de HICE. Es necesario mejorar las políticas públicas de detección temprana de diabetes ya que podrían reducir el impacto de hemorragia intracerebral espontánea en la población general.
