Según la cultura popular latinoamericana, el empacho es una enfermedad intestinal causada por ingesta excesiva de alimentos, fríos o mal cocidos. La manipulación abdominal o “sobada” es uno de los tratamientos populares utilizados para el empacho y consiste en un masaje abdominal, donde se frota, se estira la piel y se aplica presión sobre el abdomen.Objetivos: Caracterizar la evolución clínica y las complicaciones secundarias al retraso diagnóstico, en pacientes pediátricos con apendicitis aguda, con antecedente de empacho y manipulación abdominal.Pacientes y Método: Estudio observacional descriptivo, prospectivo de pacientes pediátricos con apendicitis aguda complicada con antecedente de manipulación abdominal, captados en la sala de cirugía pediátrica entre noviembre del 2019 y junio del 2020 en el Hospital Mario Catarino Rivas. Se empleó estadística descriptiva para el análisis.Resultados: Se estudiaron 10 pacientes, rango de edad 6 a 16 años (mediana 14 años), 50% procedente de zonas rurales. Los principales síntomas que se presentaron previo a la manipulación abdominal fueron: vómitos, náuseas y anorexia, posterior a lo cual se agregó diarrea espontanea o estreñimiento y aumento de la intensidad del dolor. Las complicaciones fueron perforación intestinal (5/10), plastrón apendicular (3/10), absceso intraabdominal (3/10), y apéndice en fase gangrenosa (1/10). La mediana de tiempo entre síntoma inicial y hospitalización fue 60 horas [IQR, 33,0-72,0].Conclusión: Se describe la influencia del empacho y su respectivo tratamiento en el deterioro clínico debido al retraso diagnóstico y del tratamiento de la apendicitis aguda, dando como resultado distintas complicaciones.
A new virus belonging to the Coronavirus (COV) family burst into the Wuhan region of China causing severe acute respiratory syndrome of high lethality. COV not only is pathogenic that mainly affects the human respiratory system but also has neuroinvasive capabilities. The transmission mechanism is person-to-person; also air propagation and other mechanisms have been described. The main entrance mechanism is through the binding of the virus' S protein with angiotensin-converting enzyme receptors, these are present in multiple systems including the nervous system. The spread of SARS-CoV-2 in systemic circulation or through the cribriform plaque of the ethmoid bone during an early or later phase of infection can lead to brain involvement. Multiple cases with general neurological manifestations have been reported worldwide, such as headache and confusion, but also more specific cases of cerebrovascular disease, encephalitis, and anosmia, among others described in this review. Specialists are encouraged, within patients with neurological manifestations, to investigate epidemiological links to establish the relationship between a neurological manifestation probably secondary to COVID-19. Diagnostic tests in patients with such manifestations are recommended to establish an association. It is recommended to conduct studies oriented to the different neurological manifestations, and also to clinicians to stay alert with them to improve both the evolution and the prognosis of the patient.
La enfermedad de Hirschsprung es una malformación del sistema nervioso entérico, caracterizada por falta de células ganglionares en plexo submucoso y mientérico en pared distal del colon. En el 80% solo existe afectación del recto-sigma, sin embargo, pueden encontrarse casos de aganglionosis total. Reporte de Caso: Se presenta pre-escolar masculino de 4 años de edad, procedente de La Iguala, Lempira, Honduras, con historia de constipación y distensión abdominal de dos años de evolución, que ha empeorado progresivamente, y se atenua- ba con el uso de enemas cada dos días para poder defecar. En mayo de 2016 llega al Hospital Mario Catarino Rivas, donde se le realizó una laparotomía exploratoria de emergencia debido a la presencia de deterioro clínico por constipación y distensión abdominal, acompañada de vómitos con restos alimenticios y fiebre alta de una semana de evolu- ción. En dicha operación se realizó colectomía par- cial izquierda, además se realizó una colostomía terapéutica y se diagnósticó enfermedad de Hirs- chsprung mediante biopsia, además del hallazgo incidental de apendicitis, se realizó apendicectomía obteniendo en general, una evolución satisfactoria. El estudio histopatológico de la biopsia del recto es el estándar de oro para realizar el diagnóstico. La ausencia de células ganglionares en el plexo submucoso con la tinción hematoxilina y eosina establece el diagnóstico. El tratamiento de la enfermedad de Hirschsprung es de tipo quirúrgico y se busca la eliminación del segmento colónico afectado, de manera que se pueda lograr una anastomosis del colon proximal y distal al área agangliónica.
La apendicitis aguda (AA) en niños usualmente es confundida con empacho, enfermedad que, según creencia popular, se produce por ingesta de alimentos mal cocidos o fríos causando síntomas gastrointestinales. El tratamiento empírico es la manipulación abdominal, que consiste en masaje con distintas maniobras sobre la pared abdominal, acompañado de remedios caseros. Se desconoce el efecto de dichas terapias en la evolución de AA. Objetivo: Determinar la asociación entre manipulación abdominal y AA complicada en pacientes pediátricos. Pacientes y Método: Estudio de casos y controles en menores de 18 años, con cuadro de abdomen agudo, intervenidos quirúrgicamente por AA, en un centro hospitalario de Honduras. Se definió como casos los pacientes con AA complicada y controles aquellos con AA simple. Se empleó un modelo regresión logística binaria para determinar los factores de riesgo asociados a complicaciones de AA. Resultados: Se enrolaron 31 casos y 31 controles, mediana de edad 11 años. El 58,1% procedía de una zona urbana. El 77,4% de los casos y 9,7% de los controles tuvo antecedente de manipulación abdominal. El síntoma inicial más frecuente fue dolor abdominal. En análisis univariado el sexo masculino, tiempo de evolución prolongado, uso de remedios caseros, leucocitosis > 20,000/mm3 y manipulación abdominal fueron factores de riesgo para AA complicada. En modelo multivariado, la manipulación abdominal fue el principal factor de riesgo (OR 15,94 [3,40-74,59]). Conclusión: Creencias culturales como el empacho y sus respectivos tratamientos como la manipulación abdominal y los remedios caseros, son factores de riesgo para la complicación de un cuadro instaurado de AA, así mismo contribuyen al retraso diagnóstico.
Resumen Objetivo: El objetivo de este estudio es presentar un caso clínico de un paciente con lesión del tracto biliar en el cual se realizó una reconstrucción mediante una hepáticoyeyunostomía (HY) en Y de Roux. Decripción del caso: Una joven de 25 años, con antecedentes de colecistectomía abierta, refiere múltiples episodios de vómitos, fiebre, tinte cutáneo amarillo y dolor abdominal, de una semana de evolución. La exploración física demostró tinte ictérico generalizado y ligera hipersensibilidad del hemiabdomen derecho a la palpación. El paciente fue ingresado como síndrome ictérico obstructivo secundario a la alta sospecha de coledocolitiasis residual y colangitis aguda grado II. Resultados: Se confirmó una sección completa del conducto hepático común con fuga de todo el contraste. Se realizó una anastomosis hepática yeyunal en Y de Roux para reparar el conducto hepático común. En este caso, la evolución clínica de la paciente sugiere que tenía tanto estenosis como sección completa. Discusión: Los pacientes con estenosis a nivel del conducto hepático común, en los casos en los que el conducto hepático común se confunde con el conducto cístico, presentarán la clásica tríada de Charcot con dolor en el hipocondrio derecho, fiebre e ictericia. Conclusión: Una LVB puede simular otro cuadro clínico porque sus manifestaciones clínicas dependen del tipo de lesión; que puede ser una estenosis o una sección incompleta/completa a cualquier nivel de la vía biliar.
