Five years of the National Policy of Humanization: the trajectory of a public policyResumo O artigo apresenta o cenário de emergência e a trajetória da Política Nacional de Humanização -HumanizaSUS, que como política pública foi construída para enfrentar e superar os desafios enunciados pela sociedade brasileira quanto à qualidade e à dignidade no cuidado em saúde; redesenhar e articular iniciativas de humanização do SUS e enfrentar problemas no campo da gestão e da organização do trabalho em saúde. Destacando-a como política pública, os autores analisam suas principais apostas e ofertas teórico-metodológicas para o enfrentamento de problemas na gestão e nas práticas de atenção em saúde, apontando para a necessidade de se combinar estratégi-as nos planos macro e micropolítico, sem dissociá-las. A análise enfoca mudanças na composição e nas estratégias de ação, as quais incluem a mobilização social, o apoio a instâncias de gestão, os serviços e as equipes de saúde e o desenvolvimento de processos de formação de apoiadores institucionais. Apresenta alguns resultados obtidos nos seus primeiros cinco anos de existência, bem como limites e perspectivas, considerando o desafio da Política de Humanização do SUS se constituir efetivamente em política pública, o que não pode ser alcançado sem a mobilização das forças sociais que se agenciam para além do Estado. Palavras-chave Humanização da assistência, Gestão em saúde, Política pública em saúde Abstract This article presents the context of the emergence and implementation of the National Policy of Humanization (NPH), which was devised as a public policy to address and overcome the challenges perceived by Brazilian society regarding the quality and dignity of healthcare. The aims include redesigning and joint initiatives for the humanization of care, and providing solutions for problems in the field of management and organization of the health workforce. Highlighting it as a public policy, the authors analyze the main focus and theoretical and methodological approaches to cope with problems in management and practice of healthcare, signaling the need to combine strategies in both macro and micro policy areas. The analysis focuses on changes in the composition and strategies of action, which include social mobilization, support to management systems, services and health teams and the development of training processes for institutional stakeholders. The article presents results obtained in the first five years, as well as its limitations and prospects, especially the challenge facing NPH to be seen as a public policy, which cannot be achieved without the mobilization of social forces beyond the State.
Resumo: Avaliou-se a atenção à gestação de alto risco, incluindo o acesso, o funcionamento e a utilização dos serviços de saúde, desde a atenção primária à saúde (APS) até a atenção especializada. Trata-se de pesquisa avaliativa ancorada em modelo analítico que utiliza a triangulação de diferentes fontes de informações e análise comparativa da atenção à gestação de alto risco em quatro metrópoles brasileiras. As categorias de análise selecionadas foram: acesso a consultas e exames, vínculo entre os componentes das redes de atenção à saúde e entre usuário e profissional e o cuidado oferecido. Caracterizaram-se os contextos de atenção à gestação de alto risco por meio de indicadores dos sistemas de informação em saúde e de estudo descritivo qualitativo produzido por informantes-chave, além de questionário aplicado a 1.886 gestantes dos serviços especializados, encaminhadas pela APS. A triangulação dos dados obtidos por fontes secundárias, contextualização das redes de atenção e inquérito junto às gestantes de alto risco permitiram abarcar a complexidade dos arranjos organizacionais da rede de atenção à saúde entre os municípios. Foram identificadas diferenças significativas no acesso, vínculo e cuidado na atenção à gestação de alto risco nas quatro metrópoles estudadas. Campinas (São Paulo, Brasil) destacou-se como o município que alcançou melhor desempenho nos indicadores relacionados a vínculo e acesso das gestantes à APS. São Paulo e Porto Alegre (Rio Grande do Sul) apresentaram desempenho regular no conjunto dos indicadores, ao passo que Fortaleza (Ceará) demonstrou necessidade de melhoria nas três categorias analisadas.
Construction of the Brazilian National Health System (SUS) is inscribed as a process of striving to affirm healthcare as a substantive social value for Brazilian society. The SUS has produced reforms in the healthcare sector, while at the same time giving rise to ethical, cultural and political changes. Over two decades of experimentation, the SUS has achieved changes and conserved characteristics that have marked Brazil as one of the countries with the greatest inequality of access. The National Humanization Policy (NHP) was constructed with the dual recognition that the SUS works well in some respects, but in other respects, there are problems and contradictions that need to be addressed. To achieve this, the NHP organizes a set of concepts, methods and devices to confront the problems that remain limits on healthcare services and practices.
The constitution of Brazil directs that the country's health system, the Unified Health System (Sistema Unico de Saúde), be politically and administratively decentralized. Nevertheless, handing over competencies, responsibilities, and resources to subnational levels, especially to municipal governments, has been a slow process, lasting almost two decades. Advances have been brought about by the Unified Health System, which, from a analytical perspective, is a public and universal system. Despite that, the decentralization process needs to overcome norms that keep all levels of management dependent on Brazil's federal Government. The subnational levels have consistently faced difficulties in performing their macromanagement functions with autonomy, especially when it comes to financing and to the establishment or organization of health care networks. Boldness and responsibility will be needed to prevent Brazil's health decentralization process from leading to fragmentation. New political agreements between different levels of government, with a reassignment of responsibilities and the enhancement of a culture of technical cooperation, are fundamental requisites to making the Unified Health System have a health policy that is truly public and universal.
