Objetivo: compreender as necessidades dos familiares de crianças internadas em UTI Pediátrica. Método: estudo descritivo qualitativo apoiado na análise temática com dados coletados por entrevistas não estruturadas tendo como sujeitos oito familiares de crianças internadas em UTI Pediátrica de hospital de referência no Estado do Maranhão. Resultados: revelados 11 temas agrupados em cinco necessidades (segurança e conforto; emocionais; comunicação; adaptação e suporte). Conclusão: o adoecimento e a hospitalização aliados ao atendimento de necessidades de familiares revelou um fosso entre um sistema centrado na doença e o centrado na pessoa que englobaram o campo físico, emocional, espiritual e financeiro do familiar acompanhante. Essa perspectiva demanda ampliação do foco para além da doença alcançando a família e suas necessidades. Assim, os profissionais da UTI deverão desenvolver habilidades e competências para identificar e suprir tais necessidades, rompendo com a fragmentação do cuidado e sugerem encontro que envolve o contexto, a criança e a família.Descritores: Enfermagem; Família; Unidade de Terapia Intensiva Pediátrica.
Resumo Objetivo Analisar a estrutura das unidades básicas de saúde (UBS) e o processo de trabalho das equipes de atenção básica em saúde no cuidado à criança no Brasil. Métodos Estudo ecológico com dados dos três ciclos do Programa de Melhoria do Acesso e Qualidade da Atenção Básica 2012-2018, por Unidade da Federação e suas macrorregiões. Foram analisados sete indicadores estruturais e 13 processuais. Utilizou-se o teste t de Student para comparar as médias dos indicadores entre as macrorregiões nacionais. Resultados Participaram dos três ciclos do programa 85.845 equipes, agrupadas em 68.320 UBS. No último ciclo avaliativo (2017/2018), apresentaram maiores médias percentuais de adequação entre os indicadores estruturais: funcionamento da unidade (99%), equipamentos/materiais (82%), disponibilidade de vacinas (74%) e dispensação de medicamentos (70%). População descoberta (68%) e agendamento para especialistas (52%) corresponderam aos menores percentuais de adequação dos indicadores processuais. Conclusão Os indicadores de processo apresentaram melhores adequações que os indicadores estruturais.

AbstractObjective: To develop and implement a nursing patient history planning care in pediatric intensive care. Methods: A qualitative and descriptive study, with support of a bibliographical research and convergent care, conducted in the pediatric intensive care unit of a university hospital, involving the participation of 13 nurses. Group meetings, workshops and participant observation were the methodological resources for conducting the research. Results: Fourteen Basic Human Needs and scientific evidence underpin the development of the nursing patient history, composed of seven blocks arranged in a checklist format with open spaces for recording complications and nurse impressions. An instruction guide was provided to facilitate its use. Implementation was preceded by a test phase and historical validation. Conclusion: The instrument was developed based on scientific evidence, and included contributions from the nurses. The participatory involvement of nurses favored the implementation and use of the nursing patient history in care practices. ResumoObjetivo: Construir e implantar o Histórico de Enfermagem para planejamento dos cuidados em terapia intensiva pediátrica. Métodos: Estudo descritivo de abordagem qualitativa com suporte da Pesquisa Bibliográfica e da Convergente Assistencial. Realizado na terapia intensiva pediátrica de um hospital universitário envolvendo a participação de 13 enfermeiros. Encontros grupais, oficinas e observação participante foram os recursos metodológicos para condução da pesquisa. Resultados: Catorze evidências científicas e as Necessidades Humanas Básicas embasaram a construção do Histórico constituído por sete blocos organizados no formato check-list com espaços livres para registro de intercorrências e impressões do enfermeiro. Para facilitar sua utilização, foi construído guia de instruções. A implantação foi precedida pela fase de teste e validação do histórico. Conclusão: O instrumento foi construído fundamentado em evidências científicas e contou com contribuições dos enfermeiros do serviço. O envolvimento participativo dos enfermeiros favoreceu a implantação e utilização do Histórico nas práticas de cuidado.
ResumoIntrodução: Nas Unidades de Terapia Intensiva Pediátricas, o enfermeiro exerce o cuidado à criança, administra, organiza, controla e favorece práticas de cuidar. É um ambiente circundado de situações variadas e complexas que demandam atenção do profissional. Objetivo: Compreender as demandas de atenção que emergem do processo de cuidado do enfermeiro em Unidade de Terapia Intensiva Pediátrica. Casuística e Métodos: Estudo exploratório, descritivo e qualitativo realizado por meio de entrevista não estruturada com 11 enfermeiros da Unidade de Terapia Intensiva Pediátrica de um Hospital Universitário em São Luís e que desenvolviam atividades por, no mínimo, seis meses na unidade. A análise dos dados foi apoiada nos pressupostos da Análise Temática que consistiu na extração de 642 núcleos de sentido e a construção de quatro temas. Resultados: Os temas revelados foram: Demandas Técnico-Instrumentais que caracterizaram o trabalho do enfermeiro voltado para a realização de tarefas, procedimentos e atribuições; Demandas para Gestão do Trabalho que revelou como os enfermeiros organizam o processo de trabalho e seus fatores limitantes; Demandas para Gestão do Cuidado que demonstrou a enfermagem integrando um ambiente de relações num processo dinâmico de organização do cuidado; Demandas Estruturantes do Processo de Cuidado direcionadas às peculiaridades do cuidado à criança, à família e o cuidado continuado almejando a alta e a cura da criança. Conclusão: O cuidado em terapia intensiva pediátrica sugere um ser/fazer compartilhado, corresponsável e interativo, requer da enfermagem conhecimento técnico e científico, disponibilidade física e emocional, ética e respeito pela vida; não somente relacionado à criança, mas também à família e à equipe.Descritores: Enfermagem Pediátrica; Unidades de Terapia Intensiva Pediátrica; Cuidados de Enfermagem; Atenção. Objective: Understand the demands on care emerging from the nursing care process at a Pediatric Intensive Care Unit. Patients and Methods: We carried out an exploratory, descriptive, and qualitative study using unstructured interviews involving 11 nurses who work in the Pediatric Intensive Care Unit at the University Hospital in the city of São Luis, State of Maranhão, Brazil. To participate in the study the nurses should be on duty for at least six months in the unit. Objetivo: Identificar as necessidades dos familiares de crianças durante o período de hospitalização em Unidade de Terapia Intensiva Pediátrica. Metodologia: Estudo exploratório, transversal com abordagem quantitativa. Utilizou-se como instrumento para coleta de dados o Inventário de Necessidades e Estressores de Familiares em Terapia Intensiva, validado e adaptado no Brasil. A coleta de dados foi realizada de setembro de 2011 a abril de 2012, com 66 familiares de crianças internadas em terapia intensiva. As necessidades com média ≥ 3 foram consideradas atendidas. Resultados: Para os familiares, 76,7% das necessidades foram atendidas com destaque para as de segurança atendidas em 85,7%, seguida pelas necessidades de informação com 75% e suporte com 61,5%. As necessidades não atendidas de acordo com os familiares foram as de proximidade com 55% e a de conforto com 33,3%. Conclusão: Os resultados encontrados possibilitaram uma reflexão sobre a relação enfermeiro-familiar e sua atuação na assistência a criança hospitalizada em tratamento intensivo pediátrico. Conhecer as necessidades dos familiares viabiliza a construção de um plano de cuidado singular para criança e seu familiar nas dimensões de segurança, informação, suporte, proximidade e conforto. E as não atendidas por meio de ajustes podem ser resolvidas com articulações e mudanças nas rotinas.
