Introduction: Cancer of the lip, mouth and pharynx is a serious health problem. High incidence rates are found worldwide. In Brazil, the Southern and Southeastern regions have the highest incidences in the country. Objective: To describe 5 and 10-year survival rates in patients with cancer of the lip, mouth and pharynx at a referral center in Florianopolis, Santa Catarina, Brazil. Methods: Retrospective cohort study using data from patients diagnosed between January 1 st and December 31 st , 2001, with follow-up until December 31 st , 2011. Descriptive analysis was performed and survival was estimated by Kaplan-Meier method. Cox semiparametric model was used to estimate death risk. Results: Survival rates at 5 and 10 years were 33.3 and 26.9%, respectively. Advanced clinical stage in the diagnosis increased death risk by 2.88 and 2.51, respectively. Sex, ethnicity, level of education, previous diagnosis and treatment, as well as age, did not show significant association. Conclusion: Survival rate at 5 years was 33.3% and, at 10 years, was 26.9%. Advanced stage was an independent risk factor for death due to cancer of the lip, mouth and pharynx in both periods analyzed.
BackgroundOral health problems can generate considerable negative effects on the quality of life of individuals living with HIV. The aim of this study was investigate the oral health status and its impact on the quality of life of 1 to 18 years-old living with HIV-1 under follow-up at referral centers in Southern Brazil.MethodsA cross-sectional study involving individuals under follow-up (n = 36) was carried out. The individuals living with HIV-1 and their guardians underwent individual interviews using validated questionnaires for assessing oral health-related quality of life according to age group. Clinical oral examinations were performed to establish oral health status, in terms of caries and treatment need, HIV-1-related gingival as well as stomatological changes. Medical records were reviewed searching for clinical history of the infection and the presence of HIV-1-related diseases. Association studies between frequent/very frequent oral health-related impact on quality of life and independent variables were performed using Fisher’s exact test.ResultsThe prevalence of frequent/very frequent oral health-related impact on quality of life was 69.0%. The prevalence of caries was 75.9%. Gingival changes were present in 20.7% of the individuals. Dental treatment was needed in 72.4% of the patients. HIV-1-related disease was present in 55.2%. The variables significantly associated with the prevalence of frequent/very frequent impact on oral health-related quality of life were dental treatment need (p = 0.037) and being more than 12 years of age (p = 0.041).ConclusionsIndividuals living with HIV-1 with need for dental treatment and those over 12-years of age reported a statistically higher frequency of frequent/very frequent oral health-related impact on quality of life.
O artigo analisa o desenvolvimento da capacidade avaliativa em um caso exemplar de uma Secretaria de Estado da Saúde (SES) participante do Projeto de Fortalecimento da Capacidade Técnica das Secretarias Estaduais de Saúde em Monitoramento e Avaliação da Atenção Básica. Estudo de caso de abordagem mista, com informações provenientes de documentos, entrevistas semiestruturadas e da avaliação da atenção básica realizada pela SES no período de 2008-2011. Empregou-se a análise de processos para identificar os eventos lógicos que contribuíram para o desenvolvimento da capacidade avaliativa, com duas categorias: eventos de construção da capacidade avaliativa e eventos de construção de estrutura organizacional. A pactuação em instâncias deliberativas para a continuidade da avaliação, a coleta e análise dos dados realizada pela SES, a iniciativa de mudança de indicadores e reestruturação da matriz avaliativa e a comunicação dos resultados aos municípios formaram a cadeia lógica de eventos. A análise triangulada demonstrou que o objetivo de desenvolver a capacidade avaliativa foi alcançado.
RESUMO Este artigo verifica a disseminação e o uso das pesquisas financiadas pelo Departamento de Ciência e Tecnologia do Ministério da Saúde. Trata-se de um estudo de seis casos com análise imbricada, em que se verifica que a política de descentralização do fomento contribuiu para a redução da desigualdade na produção de conhecimento em saúde, embora haja desigualdade no volume de recursos alocados. A disseminação dos resultados foi o limitador da incorporação dos resultados de pesquisa. Os formatos de acompanhamento das pesquisas devem ser melhorados. O fator determinante na incorporação de conhecimentos e tecnologias pelo sistema de saúde é a intencionalidade técnica e política para o uso dos resultados.PALAVRAS-CHAVE Uso da informação científica na tomada de decisões em saúde. Objective: to build a model to evaluate use of the National Program for Improving Primary Health Care Access and Quality (PMAQ-AB) in Brazil. Methods: document analysis and literature review followed by a consensus workshop with specialists, 19 specialists were invited who were selected because of their teaching or research experience, professional activities or publications related to the subject; validation took place on an online platform where participants were asked to analyze the evaluation proposal and to give their opinion based on the categories 'I totally agree', 'partially agree', and 'disagree'. Results: 15 specialists confirmed their participation; at the end of three rounds, the specialists emphasized the clarity of the proposal and the clarity of the object of evaluation presented in both the Logic Model and the Theoretical Logic Model; the specialists fully agreed with the Evaluation Matrix and confirmed its theoretical consistency. Conclusion: this model will be applied to evaluate the use of PMAQ-AB in Primary Health Care management and health team actions.
