O objetivo da investigação foi conhecer a prática da enfermeira em auditoria em saúde. A concepção de análise adotada foi a da hermenêutica-dialética, e foi desenvolvida em três lócus: a auditoria interna de uma organização hospitalar; a auditoria externa de um comprador privado de serviços de saúde, e o sistema de auditoria do âmbito estadual do Sistema Único de Saúde (SUS), na Bahia. Foram entrevistadas nove enfermeiras auditoras. Na auditoria do SUS, as enfermeiras expressaram satisfação no exercício desta prática e valorização de seu papel profissional. Na auditoria privada - interna e externa às organizações de saúde - as ações das enfermeiras se direcionam para atender aos interesses de seus contratantes, e pouco se relacionam com a assistência prestada pela equipe de enfermagem e com as necessidades dos usuários dos serviços.
Objective: To analyze the characteristics of nurses' work process in different countries. Method: We have used the integrative review method and selected 84 publications (articles, theses and dissertations) in national and foreign thesis banks and databases. We analyzed the evidence based on dialectical materialism. Results: The rejection of managerial tasks hides the singularity of nurses' work, due to the failure to understand the inseparable nature of managerial and healthcare tasks, given that it is what provides the expertise to coordinate the nursing work process and guide the healthcare work processes. The social and technical division is present in the work process in all countries studied, albeit in different ways. The nurse's position in the healthcare work process is subordinated to that of the physician. Conclusion: The characteristics are similar. The rejection of the dual nature of the work by nurses themselves due to alienation results in the non-recognition of their own work. A divisão técnica e social está presente no processo de trabalho em todos os países estudados, ainda que sob formas distintas. A bill for Public Sector Administrative Reform in Brazil introduces administrative strategies based on broad delegation of authority and "a posteriori" demands for results, thereby highlighting the issue of evaluation in public organizations. This article presents a methodological proposal to evaluate management capability in so-called social organizations, a kind of non-government public entity created within this Reform. Drafting of the bill included the definition of an underlying management concept and its dimensions, which oriented the construction of variables, indicators, parameters, and an analytical plan for evaluation. The final remarks include the argument that the conceptual choices and procedural decisions aim to produce an evaluation design that would guarantee the results' reliability and their sensitivity for approaching the phenomenon under evaluation, as well as the methodology's applicability and reproducibility in different contexts.
Analisamos a participação política da enfermeira na gestão compartilhada do Sistema Único de Saúde. Pressupondo que a enfermeira possui uma participação política na gestão da saúde, identificamos a sua concepção quanto à participação política; os espaços onde esta é exercida e analisamos, através de uma categorização pré-definida, como se processa esta participação. É um estudo de caso exploratório e qualitativo, num município baiano com uma gestão considerada como bem sucedida. A coleta se fez através de entrevistas semi-estruturadas com enfermeiras gestoras e da observação pontual. Os resultados, analisados a partir das categorias pré-definidas e pela técnica de análise temática, apontam para uma visão restrita das enfermeiras quanto ao seu papel político na gestão do Sistema Único de Saúde; de uma participação política incipiente; de pouca valorização quanto a esta forma de participação, embora as enfermeiras ocupem espaços diferentes, relevantes e de caráter político-técnico no sistema de saúde municipal.
This is an integrative review that examines the national scientific production on health promotion and nurse's practice in the field of primary health care. With the development of the study, it was found that the discussion of this theme is little explored in nursing and that health promotion is still understood as synonymous of health education and / or prevention of disease. It is concluded that health education is an essential strategy to achieve the health promotion, understanding that one is not restricted to the other. Moreover, it is necessary that nurses recognize health promotion as a field of practice can transform social reality and their own practice.
Objective: to analyze the intensity of nursing work in public hospitals. Method: cross-sectional, quantitative study, carried out in 22 public hospitals. The sample was composed of 265 nurses and 810 nursing technicians and assistants. Data were collected through a questionnaire and analyzed with Exploratory Factor Analysis. The calculation of the distribution of the work intensity by category was done using a score ranging from -1 to +1 standard deviation of the data. Fisher’s exact test (0.05 <p≤0.10) was used to observe the significance between groups according to the employment bond. Results: work intensity contributed to the explanation of precarization of work, with a value of 13% for nurses and 51.2% for technicians and assistants. For the technicians and assistants, the variables with the highest factor loadings were ‘work requires more than she can do’ (0.6696) and ‘takes on multiple tasks due to staff shortages’ (0.6156). Among nurses, the highest factor loadings were observed in the variables ‘time pressure at work (0.6779) and ‘Work pace’ (0.6651). Conclusion: the variables analyzed indicate that work intensity occurs differently among nursing workers, and is revealed by the versatility, understaffing and flexibility of work.
Objective: To identify the contribution of all the dimensions involved in job insecurity among nursing professionals in Brazilian state public hospitals. Method: This was a quantitative descriptive study conducted between March 2015 and February 2016, with nurses, nursing technicians and nursing aides. The data were analyzed using exploratory factor analysis. Results: The sample consisted of 265 nurses (n=161 with job security and n=104 outsourced) and 810 nursing technicians and nursing aides (n=597 with job security and n=213 outsourced). Among the nurses, “Working conditions” accounted for 46.8% of their job insecurity. Among nursing technicians and nursing aides “intensity of work due to work process organization” best explained the construct of job insecurity (51.2%). Conclusion: Job insecurity is expressed differently depending on the type of nursing professional. The difference in the distribution of the dimensions showed that job insecurity is related to the hierarchical position of each nursing professional in the technical division of nursing work and the role of each professional in the work process.
