Introdução: A Síndrome Coronariana Aguda (SCA) é responsável por impacto físico e psicoemocional. É apontada como uma das principais síndromes que contribuem para o prejuízo da Qualidade de Vida (QV). Estudos comprovam que as modificações dos hábitos de vida, controle de fatores de risco e a prática de atividade física causam impacto na QV destes indivíduos, tornando-se ao longo dos anos um desfecho relevante. Objetivo: Verificar a QV de indivíduos após sete anos do Evento Coronariano Agudo. Metodologia: Trata- se de um estudo transversal, composto por indivíduos diagnosticados com SCA e internados no período de abril de 2006 a janeiro de 2007 na Unidade de Terapia Intensiva Cardíaca de um hospital privado em Salvador. Foi realizado entrevista e aplicado o questionário EuroQol 5D, por contato telefônico. Resultados: Foram coletados 30 participantes, com idade média de 66,9 ± 11,1 anos, predominantemente do sexo masculino 17 (56,7%). Houve variação de 0% a 100% no escore total do questionário EuroQol 5D, apresentando média de 68,2%. Quanto aos domínios, em relação ao nível dois (problemas moderados) houve maior frequência dos domínios: Mobilidade 11 (36,7%), Dor/mal estar 13 (19,4%) e Ansiedade/depressão 12 (17,4%). Enquanto no nível três (problemas extremos) foi observado: Ansiedade/depressão seis (9%) e Dor/mal estar três (4,5%). Conclusão: Conclui-se que há nesta população um impacto na qualidade de vida, principalmente nos domínios ansiedade/depressão e dor/mal estar após sete anos do evento agudo. A elevada taxa de fatores de risco pode estar associada a maiores escores nos domínios avaliados.
HTLV-1 creates a chronic health condition that involves moderate to severe pain with a negative impact on quality of life (QoL). There is no consensus on which attitudes to pain are more related to the worsening of QoL in HTLV-1 infected patients. The aim of this study was to investigate the correlation between QoL and multidimensional aspects of pain in patients with HTLV-1. A cross-sectional study was conducted in Salvador, Bahia, Brazil. The study included individuals diagnosed with HTLV-1. The Short Form 36 Questionnaire was used to analyze QoL, and the Brief Pain Inventory was used to assess multidimensional aspects of pain. The mean pain intensity was 4.88±3.06 on the visual pain scale, and the average impact on QoL corresponded to a loss of approximately 40%. Moderate to high correlations between pain intensity and all domains of QoL were observed and compared reaction attitudes for general activity, mood, ability to walk, ability to work, relationships, sleep, and ability to enjoy life (r>0.40; p<0.05). Moderate correlations were found between all domains of QoL, pain intensity, and reactive attitudes to pain. The greatest pain intensity impacts involved difficulty to walk and to work, and interpersonal relationships in the emotional aspect of QoL.
A mobilização precoce é um método viável para a prevenção de problemas físicos e mentais gerados pelo imobilismo, sendo a eletroestimulação neuromuscular (EENM) utilizada como coadjuvante ao exercício ativo e na prevenção da perda da força muscular em pacientes críticos.Objetivo:revelar as evidências científicas sobre os efeitos da EENM em pacientes críticos internados nas unidades de terapia intensiva.Método: trata-se de um estudo de revisão sistemática com ensaios clínicos randomizados, sendo os artigos selecionados na base de dados PUBMED, LILACS e portal CAPES. Os critérios de inclusão foram: abordar sobre a EENM no paciente crítico≥18 anos de idade, obter escore de qualidade metodológica na Escala de JADAD ≥3, publicados na língua inglesaou portuguesa.As variáveis analisadas foram o perfil clínico, o tempo de aplicação, a quantidade de sessões, os parâmetros utilizados e o desfecho da aplicação da EENM.Conclusão: a aplicação da EENM poderárecuperar músculos enfraquecidos ou retardar o processo de perda muscular, no entanto, os parâmetros publicados encontrados não foram suficientes para sistematizar a técnica.
INTRODUÇÃO: A prática regular de atividade física está associada com melhora do estado de saúde, aumento da capacidade funcional, aumento da força muscular e redução da mortalidade por doenças cardíacas. Apesar dos benefícios de a prática regular de exercício físico estarem consolidados na literatura, as adaptações na força e resistência dos músculos inspiratórios são controversas. OBJETIVO: Testar a hipótese que não há diferença da força e resistência dos músculos inspiratórios entre indivíduos ativos e sedentários. MÉTODOS: Estudo observacional de corte transversal. Avaliou-se indivíduos entre 18 e 30 anos, ambos os sexos e saudáveis. Os voluntários foram divididos em ativos e sedentários de acordo a classificação da American College of Sports Medicine (ACMS). Os indivíduos tiveram a força máxima dos músculos inspiratórios (FMI) determinada através do dispositivo POWERbreathe® K5 inspiratory muscle trainer, que intula esta variável como Sindex. A resistência dos músculos inspiratórios foi avaliada través de um teste incremental. Para comparação das médias foi aplicada o teste t de student para distribuição simétrica, p< 0,05. O estudo foi aprovado pelo comitê de ética em pesquisa com CAAE : 37781014.4.0000.5544. RESULTADOS: Foram avaliados 92 indivíduos, destes 55 (60%) foram classificados como ativos e 57 (62%) do sexo masculino. Ao realizar a comparação do Sindex entre ativos e sedentários (128±26; 119±24 cmH2O; p=0,85) e da exaustão no teste incremental (65±16% e 60±16%;p=0,095), respctivamente. CONCLUSÃO: Os indivíduos ativos não apresentam músculos inspiratórios mais fortes e resistentes quando comparados com sedentários.
Brazil is home to the highest absolute number of human T-cell lymphotropic virus type-1 (HTLV-1)-infected individuals worldwide; the city of Salvador, Bahia, has the highest prevalence of HTLV-1 infection in Brazil. Due to the complex nature of several diseases associated with this retrovirus, a multidisciplinary health care approach is necessary to care for people living with HTLV-1. The Bahia School of Medicine and Public Health’s Integrative Multidisciplinary HTLV Center (CHTLV) has been providing support to people living with HTLV and their families since 2002, striving to ensure physical and mental well-being by addressing biopsychosocial aspects, providing clinical care and follow-up, including to pregnant/postpartum women, as well as comprehensive laboratory diagnostics, psychological therapy, and counseling to family members. To date, CHTLV has served a total of 2,169 HTLV-infected patients. The average patient age is 49.8 (SD 15.9) years, 70.3% are female, most are considered low-income and have low levels of education. The majority (98.9%) are HTLV-1 cases, and approximately 10% have been diagnosed with tropical spastic paraparesis/HTLV-1-associated myelopathy (TSP/HAM), while 2.2% have infective dermatitis and 1.1% have adult T-cell lymphoma. In all, 178 pregnant/postpartum women [mean age: 32.7 (±6.5) years] have received care at CHTLV. Regarding vertical transmission, 53% of breastfed infants screened for HTLV tested positive in their second year of life, nearly 18 times the rate found in non-breastfed infants. This article documents 20 years of experience in implementing an integrative and multidisciplinary care center for people living with HTLV in Bahia, Brazil. Still, significant challenges remain regarding infection control, and HTLV-infected individuals continue to struggle with the obtainment of equitable and efficient healthcare.
Objectives: Identify the effects of inspiratory muscle training (IMT) on walking capacity, strength and inspiratory muscle endurance, activities of daily living, and quality of life poststroke. Design: Double-blind randomized trial. Setting: The Sarah Network of Rehabilitation Hospitals. Subjects: Adult poststroke inpatients with inspiratory muscle weakness. Interventions: The Experimental Group (EG) ( n = 23) underwent IMT for 30 minutes/day, five times/week over six weeks. The Control Group (CG) ( n = 27) performed sham IMT. Both groups underwent standard rehabilitation. Main measures: Primary outcome was post-intervention six-minute walking test (6MWT) distance. We also measured maximal inspiratory pressure (MIP), maximal expiratory pressure (MEP), inspiratory muscle endurance, activities of daily living (functional independence measure – FIM), and quality of life at baseline and post-intervention. Three months after intervention, we measured MIP, walking capacity and quality of life. Results: Baseline characteristics were similar, with mean age 53 ± 11 years and FIM 74 ± 10p. Both groups similarly increased the walking capacity at six weeks (63 vs 67 m, P = 0.803). Compared to the CG, the EG increased the inspiratory endurance (22 vs 7 cmH2O, P = 0.034) but there was no variation in MEP (14 vs 5 cmH2O, P = 0.102), MIP (27 vs 19 cmH2O, P = 0.164), FIM (6 vs 6, P = 0.966) or quality of life (0 vs 0.19, P = 0.493). Gains in both groups were sustained at three months. Conclusion: Adding IMT to a rehabilitation program improves inspiratory muscle endurance, but does not further improve MIP, 6-MWT distance, activities of daily living or quality of life of individuals after stroke beyond rehabilitation alone.
