Zusammenfassung Hintergrund Die Pilotstudie „Gesundheit und Unterstützung in Zeiten von Corona“ (Technische Universität Dortmund) erhob vom Mai bis Juli 2020 Veränderungen von Unterstützung und Wohlbefinden Älterer infolge der COVID-19-Pandemie. Ziel der Arbeit Ziel war es, empirische Erkenntnisse zu den sozialen und mentalen Folgen der Pandemie für in Privathaushalten lebende Personen der Altersgruppe 40+ Jahre zu gewinnen. Betrachtet wurden durch die Pandemie bedingte Änderungen im Erhalt und im Leisten von Unterstützung (u. a. persönliche Pflege, Hilfe im Haushalt) und Betreuungsprobleme sowie Veränderungen des Wohlbefindens. Material und Methoden Mithilfe deskriptiver und multivariater Analysen wurde untersucht, wie sich im Zuge der Pandemie Unterstützungsmuster änderten, ob Betreuungsprobleme entstanden, und ob sich in diesem Zuge des Wohlbefinden (Lebenszufriedenheit und Einsamkeit) veränderte. Ergebnisse Im Zuge der Pandemie zogen sich insbesondere Ältere und Hochaltrige aus der Unterstützung für andere zurück. Frauen berichteten häufiger von Betreuungsproblemen mit älteren Angehörigen. Das Wohlbefinden verringerte sich insgesamt, am deutlichsten aber bei Frauen und Hochaltrigen. Die multivariaten Analysen verdeutlichen, dass Betreuungsprobleme mit älteren Angehörigen im Zuge der Pandemie mit geringerem Wohlbefinden einhergingen. Diskussion Unsere Pilotstudie zeigt deutliche Änderungen in Unterstützungsmustern und im Wohlbefinden der Befragten. Viele berichten von mehr Einsamkeit und geringerer Lebenszufriedenheit als vor der Pandemie – insbesondere Frauen, die Unterstützungsleistungen für andere erbringen. Sorgearbeit wird durch die Pandemie und die Maßnahmen zu ihrer Bekämpfung erschwert. Zukünftige Kontaktbeschränkungen sollten mit Bedacht eingesetzt werden und dies im Blick haben.
Background Given the nature of the spread of SARS-CoV-2, strong regional patterns in the fatal consequences of the COVID-19 pandemic related to local characteristics such as population and health care infrastructures were to be expected. In this paper we conduct a detailed examination of the spatial correlation of deaths in the first year of the pandemic in two neighbouring countries – Germany and Poland, which, among high income countries, seem particularly different in terms of the death toll associated with the COVID-19 pandemic. The analysis aims to yield evidence that spatial patterns of mortality can provide important clues as to the reasons behind significant differences in the consequences of the COVID-19 pandemic in these two countries. Methods Based on official health and population statistics on the level of counties, we explore the spatial nature of mortality in 2020 in the two countries – which, as we show, reflects important contextual differences. We investigate three different measures of deaths: the officially recorded COVID-19 deaths, the total values of excessive deaths and the difference between the two. We link them to important pre-pandemic regional characteristics such as population, health care and economic conditions in multivariate spatial autoregressive models. From the point of view of pandemic related fatalities we stress the distinction between direct and indirect consequences of COVID-19, separating the latter further into two types, the spatial nature of which is likely to differ. Results The COVID-19 pandemic led to much more excess deaths in Poland than in Germany. Detailed spatial analysis of deaths at the regional level shows a consistent pattern of deaths officially registered as related to COVID-19. For excess deaths, however, we find strong spatial correlation in Germany but little such evidence in Poland. Conclusions In contrast to Germany, for Poland we do not observe the expected spatial pattern of total excess deaths and the excess deaths over and above the official COVID-19 deaths. This difference cannot be explained by pre-pandemic regional factors such as economic and population structures or by healthcare infrastructure. The findings point to the need for alternative explanations related to the Polish policy reaction to the pandemic and failures in the areas of healthcare and public health, which resulted in a massive loss of life.
In our Beethoven project “Age-well” we examine the role of regional contextual factors for the relationship between individual wellbeing and material conditions over time, using a unique combination of individual and regional level longitudinal data. The analyses consider a broad range of regional, community-level indicators for two countries, Germany and Poland, both characterised by rapid population ageing and significant regional variation in the standard of living. These variables, including local indicators of economic conditions and public services, are combined with detailed individual-level information on wellbeing from the Survey of Health, Ageing and Retirement in Europe (SHARE). This data match allows us to study the degree to which regional contexts affect the relationship between individual material conditions and wellbeing in later life. Local public services are shown to mediate the importance of individual level resources for wellbeing confirming an important channel through which public policy can improve welfare of older people.
ZusammenfassungÄltere Menschen sind während der Covid-19-Pandemie einem höheren Risiko ausgesetzt, schwer zu erkranken oder zu sterben. Sie werden daher in der Pandemiebekämpfung als besonders zu schützende Gruppe aufgefasst und sind in hohem Maße von Kontaktbeschränkungen betroffen. Gleichzeitig hatten ältere Menschen bereits vor der Pandemie einen erhöhten Unterstützungsbedarf, der durch die Pandemie verstärkt wurde, da viele alltägliche Tätigkeiten mit einem höheren Risiko einer Ansteckung verbunden sind. Hinzu kommt, dass Ältere oftmals selbst Unterstützung leisten, die wegen der Kontaktbeschränkungen nicht fortgesetzt werden kann. Der Beitrag zeigt die Veränderung von Versorgung und Unterstützung Älterer in Deutschland vor dem Hintergrund der Pandemie auf. Zunächst werden die politischen Entscheidungsprozesse auf Bundesebene beschrieben, um anschließend die Rolle der Kommunen einzuordnen. Danach wird der Forschungsstand zur Versorgung und Unterstützung von älteren Menschen sowie zu der von ihnen geleisteten Unterstützung aufgezeigt. Daran schließt sich die Darstellung der Ergebnisse einer eigenen Studie an. Der quantitative Teil der Studie befasst sich mit Veränderungen der Unterstützungsmuster im privaten Bereich, während der qualitative Teil die Identifikation besonders belasteter Gruppen, entstandene Versorgungslücken und mögliche Maßnahmen zur Vorbereitung auf zukünftige Pandemien beleuchtet. Die Ergebnisse der Studie zeigen, dass sich soziale informelle Unterstützungsleistungen im Zuge der ersten Covid-19-Welle in vielen Fällen verändert haben und dass pflegende Angehörige sowie Menschen mit chronischen Krankheiten, mit Pflegebedarf und/oder sehr hohen Alters als besonders belastete Gruppen einzustufen sind.
