<p><strong>Introducción: </strong>En determinadas circunstancias<strong> </strong>la artritis séptica de cadera puede devenir en secuelas de diversa magnitud. Objetivos: diseñar y aplicar una clasificación radiográfica, concisa e inclusiva de las mismas. Confiabilidad: avalada por el índice de Kappa intra e interobservador.</p><p> </p><p><strong>Material y método: </strong>Estudio observacional retrospectivo de 32 pacientes (37 caderas) portadores de secuelas de sepsis articular, admitidos y tratados en nuestro hospital en un período de 14 años. Se incluyeron niños menores de 15 años, con seguimiento mínimo de 2 años. Análisis estadístico con el programa SPSS 15.0. Variables: edad, sexo y cadera afectada. Las caderas secuelares, según sus lesiones específicas, fueron incluidas en la siguiente clasificación por grados: I o sin cambios; II o incongruentes; III o excéntricas y IV o luxadas.</p><p> </p><p><strong>Resultados: </strong>Edad promedio 3 años y 9 meses. Predominio femenino (68%) Todos los casos bilaterales fueron niñas (p<0,02) Doce caderas se incluyeron en grado I, cuatro en II, once en III y, diez en IV. Secuelas predominantes: coxa vara y ascenso del trocánter mayor. De los pacientes secuelares (25 caderas), 16 tuvieron afectaciones múltiples. El índice de Kappa fue de 0,93 para el intra observador y 0,66 para el interobservador, expresando alta confiabilidad.</p><p> </p><p><strong>Conclusiones: </strong>La clasificación Ludovica avalada estadísticamente, cumple con los requisitos que nos propusiéramos: ser breve y concisa, inclusiva de las diferentes secuelas, requiere únicamente radiografías, fácil de retener y trasmitir y, finalmente, permite inferir lineamientos terapéuticos.</p>
Objetivo: Evaluar competencias profesionales de una residencia de Ortopedia y Traumatología Infantil. Instrumentos pedagógicos utilizados: Mini-Clinical Evaluation Exercise (Mini-CEX) y Direct Observation of Procedural Skills (DOPS) para competencias clínicas y quirúrgicas, respectivamente. Ambas evalúan la cúspide de la pirámide de Miller; se precisaron su confiabilidad y validez.
Materiales y Métodos: Estudio observacional prospectivo de una cohorte de seis residentes de primero, segundo y tercer año quienes fueron evaluados por seis docentes en diversos contextos y situaciones reales: consultorio externo y de guardia, sala de internación, interconsultas, quirófano y sala de yesos.
Resultados: Se realizaron 65 observaciones. Cada residente fue evaluado como media en 10 oportunidades por entre 3 y 6 docentes. Para las variables clínicas, en general, los residentes más antiguos obtuvieron valores sobresalientes y los residentes de primer año, valores satisfactorios. No hubo diferencias significativas para las competencias quirúrgicas globalmente, pero los residentes de tercer año fueron más competentes para resolver situaciones inesperadas. El coeficiente de Cronbach fue superior a 0,90.
Conclusiones: Ambos instrumentos de evaluación tuvieron una elevada confiabilidad. El método estadístico permitió individualizar exactamente las fragilidades y fortalezas de la residencia. El coeficiente de Cronbach obtuvo un valor de alto impacto psicométrico.
