Samenvatting In dit artikel wordt de ontwikkeling en toepassing van een cognitief-gedragstherapeutische groepstraining voor patie¨nten met een psychotische stoornis beschreven. De zelfcontrole-training (ZCT) is bedoeld voor patie¨nten die belemmeringen ervaren bij maatschappelijke reı¨ntegratie en bij wie op het moment van training de psychotische symptomen het opnemen van informatie niet te zeer in de weg staat. Doel van de training is het voorkomen van decompensatie. De fasen van de training worden geı¨llustreerd met twee voorbeelden. De resultaten van een pilot-studie tonen dat de ZCT een positieve invloed heeft op dichotoom denken, algemeen welbevinden, depressieve copingstijl en sociale interacties. De resultaten kunnen mogelijk worden verbeterd door de training een exclusief 'hier-en-nu'-karakter te geven en door deelnemers in tweetallen aan opdrachten te laten werken. InleidingEen psychotische stoornis gaat gepaard met inhoudelijke en/of formele denkstoornissen. Er wordt verondersteld dat deze cognitieve stoornissen mede ten grondslag liggen aan gebrekkig inzicht in moeilijke situaties en aan vaardigheidstekorten (Van den Bosch, 1993).Uit onderzoek is gebleken dat formele denkstoornissen niet opgeheven worden door training van cognitieve vaardigheden (Van der Gaag, 1992). Ook blijkt dat vaardigheidstraining in het algemeen een a-specifiek effect heeft op het functioneren van patie¨nten met schizofrenie (Appelo et al., 1997). Interventies worden daarom steeds minder gericht op gedragsmatige vaardigheden en steeds meer op inhoudelijke denkstoornissen die de gedragsmatige incompetentie kunnen veroorzaken en in stand houden. Cognitieve gedragstherapie bij psychotische stoornissen is in opkomst (Fowler et al., 1995 ;Chadwick et al., 1996). De behandeling wordt niet direct gericht op de psychotische symptomen, maar op de angstige en depressieve interpretaties ervan.De zelfcontrole-training (ZCT; Tak et al., 1995) is bedoeld voor mensen met een psychotische stoornis bij wie psychomotore armoede op de voorgrond staat en die belemmeringen ervaren bij maatschappelijke reı¨ntegra-tie. De training is gericht op het vaststellen en vervangen van gedachten die het uitvoeren van gewenst gedrag belemmeren. Ook het voorkomen van psychotische decompensatie is daarom een doelstelling. De inhoud van de training is gefaseerd en wordt in tabel 1 samengevat.De eerste fase duurt zes sessies. Er wordt stilgestaan bij het identificeren van gedachten, gedrag en gevoel; de zogenaamde drie G's. Met behulp van registratieopdrachten en de bespreking daarvan wordt de deelnemers duidelijk gemaakt dat de drie G's elkaar kunnen beı¨nvloeden en dat gedachten kunnen worden gezien als de oorzaak van gevoelens en gedragingen.Tijdens de tweede fase, die tien sessies duurt, leren de deelnemers om niet-doelgerichte gedachten die tot ongewenste consequenties leiden te vervangen door gedachten die wel tot gewenste consequenties leiden (doelgerichte alternatieven). Samenvatting Cognitieve-gedragstherapie is een veelbelovende therapievorm bij patie¨nten met een psychotische stoornis. Uit onderzoek blijkt dat zowel de psychotische als de niet-psychotische symptomen positief worden beı¨nvloed. Er is echter nog maar weinig gecontroleerd onderzoek voorhanden.In deze studie worden de effecten van Zelfcontroletraining, een cognitief-gedragstherapeutische groepstraining voor patie¨nten met een psychotische stoornis, vergeleken met de effecten van Sociale vaardigheidstraining. De resultaten tonen aan dat beide trainingen positieve effecten hebben op psychische klachten, sociaal functioneren, het zelfbeeld en de copingstijl van patie¨n-ten. Geen van beide trainingen heeft een gunstig effect op variabelen die gerelateerd zijn aan maatschappelijke integratie en zorgconsumptie. Ook werd op geen van de onderzochte variabelen een verschil gevonden tussen de effecten van Zelfcontrole-training en Sociale vaardigheidstraining. Gegeven deze resultaten wordt geconcludeerd dat Zelfcontrole-training wel zinvol kan zijn in de behandeling van patie¨nten met een psychotische stoornis, maar dat de verwachtingen niet te hoog gespannen mogen zijn. InleidingCognitieve-gedragstherapie speelt een steeds grotere rol in de behandeling van psychotische stoornissen (Appelo & Van der Gaag, 1998). Hieraan ligt de veronderstelling ten grondslag dat bepaalde vaardigheden wel in het gedragsrepertoire aanwezig zijn, maar door de invloed van inhoudelijke denkstoornissen niet worden gebruikt. Trainingen die gericht zijn op het activeren of aanleren van vaardigheden leiden volgens deze opvatting vaak tot teleurstellende resultaten, omdat het belangrijkste probleem niet van gedragsmatige, maar van cognitieve aard is.De prognose voor het effectief behandelen van inhoudelijke denkstoornissen bij psychotische problematiek lijkt gunstig. De laatste tijd verschijnen er in elk geval steeds meer publicaties waaruit blijkt dat niet alleen disfunctionele gedachten die optreden bij depressies en angsten, maar ook wanen en hallucinaties effectief kunnen worden gereduceerd met cognitief-gedragstherapeutische technieken (Kuipers et al., 1997 ;. De resultaten zijn gunstig bij patie¨nten met chronische stoornissen en bij psychotische patie¨nten met een korte ziektegeschiedenis (Drury, Birchwood, Cochrane, & MacMillan, 1996 ;Haddock et al., 1998).Psychotische decompensatie wordt meestal voorafgegaan door symptomen die als waarschuwingssignalen kunnen worden aangemerkt. Het betreft vooral onrust, angst, depressieve klachten en een toenemende impulsiviteit en achterdocht (Birchwood & Tarrier, 1992). Tijdens een prepsychotische fase verliest de patie¨nt steeds meer het vermogen om zijn gedachten, gevoelens en gedragingen te controleren.
