Systems Training for Emotional Predictability and Problem Solving (STEPPS) is a group treatment for persons with borderline personality disorder (BPD) that is relatively easy to implement. We investigated the efficacy of a Dutch version of this treatment (VERS). Seventy-nine DSM-IV BPD patients were randomly assigned to STEPPS plus an adjunctive individual therapy, or to treatment as usual. Assessments took place before and after the intervention, and at a 6-month follow-up. STEPPS recipients showed a significantly greater reduction in general psychiatric and BPD-specific symptomatology than subjects assigned to treatment as usual; these differences remained significant at follow-up. STEPPS also led to greater improvement in quality of life, especially at follow-up. No differences in impulsive or parasuicidal behavior were observed. Effect sizes for the differences between the treatments were moderate to large. The results suggest that the brief STEPPS program combined with limited individual therapy can improve BPD-treatment in a number of ways.
Background: Systems Training for Emotional Predictability and Problem Solving (STEPPS) is a group treatment for borderline personality disorder (BPD). Two prior randomized controlled trials (RCTs) have shown the efficacy of this training. In both RCTs, patients with borderline features who did not meet the DSM-IV criteria for BPD were excluded, which were many. We investigated the effectiveness of STEPPS in a sample representative of routine clinical practice and examined whether DSM-IV diagnosis and/or baseline severity were related to differential effectiveness. Methods: Patients whom their practicing clinician diagnosed with BPD were randomized to STEPPS plus adjunctive individual therapy (STEPPS, n = 84) or to treatment as usual (TAU, n = 84). Results: STEPPS recipients showed more improvement on measures of general and BPD-specific psychopathology as well as quality of life than TAU recipients, both at the end of treatment and at a 6-month follow-up. Presence of DSM-IV-diagnosed BPD was not related to differential treatment effectiveness, but dimensional measures of symptom severity were; STEPPS was superior to TAU particularly in patients with higher baseline severity scores. Conclusions: The findings show the effectiveness of STEPPS in a ‘real-world’ sample, and underscore the importance of dimensional versus categorical measures of personality disturbance.
Samenvatting De Vaardigheidstraining emotionele regulatiestoornis (VERS) is een educatieve en cognitief-gedragstherapeutische groepstraining voor mensen met borderline persoonlijkheidsproblematiek. Het doel van de training is een beter begrip van en meer controle over de eigen klachten. Uit zowel subjectieve rapportage als een effectmeting gericht op psychische klachten blijkt dat de VERS een veelbelovende training is. Wel is er een vrij hoog drop-outpercentage dat mogelijk gerelateerd is aan de groepsgrootte en de manier waarop met 'dreigend afhaken' en registratie-en huiswerkopdrachten wordt omgegaan. Gecontroleerd onderzoek zal moeten uitwijzen in hoeverre de VERS echt iets toevoegt aan de kwaliteit van leven. InleidingUit het Dth-themanummer 'persoonlijkheidsstoornissen' (Korrelboom & Lange, 2000 ) blijkt dat men het er nog lang niet over eens is of deze stoornissen wel echt bestaan. De voorstanders van 'as-II' zijn het er vervolgens weer niet over eens wat een persoonlijkheidsstoornis precies is. Ook valt op dat nog maar weinig wetenschappelijk onderzoek is gedaan naar de behandeling van persoonlijkheidsproblematiek.Waar weinig consensus is, passen een breed theoretisch verklaringsmodel en een multidisciplinaire behandeling. Zo ook bij de borderline persoonlijkheidsstoornis (BPS). Niemand spreekt tegen dat het bij het ontstaan en beloop van deze stoornis om een combinatie van biologische, psychologische en sociale factoren gaat. En iedereen lijkt het erover eens dat zowel medicamenteus, psychotherapeutisch als systemisch moet worden ingegrepen. Gebaseerd op deze holistische benadering wordt de BPS wel opgevat als een emotionele regulatiestoornis, die ontstaat uit een combinatie van een aangeboren zwakte van het emotionele regelsysteem en een invaliderende, door verwaarlozing en aantasting van de autonomie gekenmerkte omgeving tijdens de jeugd (Linehan, 1987 ;1993). Iemand met een biologisch bepaalde verhoogde gevoeligheid voor emotionele stimuli zal heftiger en langer op stressoren reageren dan iemand met een 'normale' gevoeligheid. De arousal keert vertraagd terug naar het basisniveau. Karakteristiek voor een emotionele regulatiestoornis is dat gemiddeld genomen de tijd tussen twee stressoren onvoldoende is om de verhoogde activatie te laten dalen. De patie¨nt krijgt dus een nieuwe stressor te verwerken terwijl de effecten van de vorige nog niet zijn uitgewerkt. Hierdoor ontstaat een chronische toestand van verhoogde emotionele activatie die met regelmaat tot explosies leidt. Deze gaan gepaard met heftige, extreme gedragingen die door de omgeving als schadelijk en pathologisch worden beoordeeld, maar voor de patie¨nt in eerste instantie vaak opluchting geven. Dit zijn de instabiliteit en de impulsiviteit die fenomenologisch zo typerend zijn voor de BPS (APA, 1995 ).Uit de populariteit van Linehan en de relatieve successen die zij met haar Dialectische gedragstherapie boekt (Linehan, Armstrong, Suarez, Allmon & Heard, 1991 ;Linehan, Heard & Armstrong, 1993 )
