Resumo O crescente envelhecimento populacional e as transformações socioculturais do nosso tempo conduzem a novas necessidades, não somente de cuidado, mas também para atender a uma nova perspectiva do conceito de saúde, no qual cuidar da doença se tornou ação limitada. Este estudo trata-se de um ensaio baseado na literatura científica e se propõe a pensar sobre uma pequena parte desta grande questão: o apoio social como parte da atenção integral à saúde do idoso. Para isso, realizou-se uma revisão narrativa da literatura nas bases de dados Pub Med/MedLine, Crochane e Scopus, bem como nas listas de referências dos artigos encontrados. Parte-se da compreensão do papel do apoio social e das redes microssociais na saúde do idoso. A seguir, discutimos formas de avaliar o apoio e a rede social e refletimos sobre o papel do apoio social no Sistema Único de Saúde. Por fim, aborda-se o manejo do apoio social. Como considerações finais, são destacados os principais desafios para que o Apoio Social seja realmente considerado um determinante da saúde das pessoas.
This study aims to contribute to a better understanding of the main family caregivers of highly dependent elderly. Performed in a geriatric outpatient unit, the study used a content analysis methodology, and data were collected with interviews and 14 group sessions with 24 subjects. The following categories emerged: denial versus acceptance of the disease before and after diagnosis; how and why the main caregiver assumes this role; caregiver's characteristics; caregiver's experiences; meanings ascribed to care-giving; history of the caregiver's relationship to the elder before the disease; caregiver/elder relationship since onset of the disease; changes in the caregiver's life; changes in the family; and causes for the appearance of the disease. In conclusion, there are three dimensions in the target issue: (1) implications of the legacies, multi-generational transmission, repetition of the patterns, and myths and beliefs in each family system; (2) seniors as active participants in the care-giving decision; and (3) the support group as a key resource for the family.
O movimento Universidades da Terceira Idade vem experimentando incremento substancial desde os anos 70, difundindo conceitos e experiências práticas que representam uma nova forma de promover a saúde da pessoa que envelhece, a partir de uma ação interdisciplinar comprometida com a inserção do idoso como cidadão ativo na sociedade. O movimento visa contribuir para a promoção da saúde física, mental e social das pessoas idosas, lançando mão das possibilidades existentes nas universidades. No Brasil, existem pelo menos 150 programas dessa natureza. Os resultados vêm sendo sistematizados e debatidos, trazendo novas perspectivas de inserção e ampliação da participação social e de melhoria das condições de saúde, e qualidade de vida dos seus participantes. O artigo discute a importância do movimento como estratégia para a melhoria da qualidade de vida da população idosa. A partir da contextualização do movimento no mundo e no Brasil, uma experiência desenvolvida na Universidade do Estado do Rio de Janeiro (UERJ), desde 1993, é descrita e analisada. Tendo como pressupostos básicos a interdisciplinaridade, a participação social e a promoção da saúde, a proposta da UERJ utiliza metodologia adaptada às especificidades desta clientela no desenho de um programa amplo de atenção integral à saúde do idoso.
A detailed review was conducted of the literature on models evaluating the
effectiveness of integrated and coordinated care networks for the older
population. The search made use of the following bibliographic databases:
Pubmed, The Cochrane Library, LILACS, Web of Science, Scopus and SciELO. Twelve
articles on five different models were included for discussion. Analysis of the
literature showed that the services provided were based on primary care,
including services within the home. Service users relied on the integration of
primary and hospital care, day centers and in-home and social services. Care
plans and case management were key elements in care continuity. This approach
was shown to be effective in the studies, reducing the need for hospital care,
which resulted in savings for the system. There was reduced prevalence of
functional loss and improved satisfaction and quality of life on the part of
service users and their families. The analysis reinforced the need for change in
the approach to health care for older adults and the integration and
coordination of services is an efficient way of initiating this change.
