El lupus eritematoso (LES) es una enfermedad autoinmune crónica, donde el sistema inmunológico ataca a sus propias células produciendo infamación y daño, debido a la unión de nuestros anticuerpos a las células del organismo y al depósito de complejos antígeno-anticuerpo. Las lesiones no específicas pueden estar asociadas a otras enfermedades autoinmunes e incluyen cambios vasculares. A pesar de su complejidad; los mecanismos patógenos comienzan a ser comprendidos involucrando a factores genéticos, hormonales y ambientales que conducen a una alteración del sistema inmunitario, con una respuesta de autoreactividad frente a las estructuras propias del organismo. Uno de los factores que se ha planteado como precursor de autoinmunidad es el estrés y factores inherentes a la personalidad de los pacientes. Entendiendo que la piel ha de ser uno de los órganos principales órganos afectados se plantea la investigación con el fin de encontrar las lesiones cutáneas de dicha enfermedad y realizar una revisión bibliográfica para analizar y sintetizar las características de esta enfermedad y las actualizaciones en las publicaciones académicas de la misma. Los avances en las publicaciones científicas llevan no solo a conocer mejor el desarrollo y tratamiento de la enfermedad, sino que por supuesto se traduce en una mejora en las expectativas de vida de los pacientes diagnosticados. La evolución en el estudio del LES es considerable, en ninguno de los trabajos analizados se descartan los hallazgos en el tema puesto que la enfermedad tiene distintas formas de manifestación y desarrollo. La mayoría de los estudios se centran en dos temas específicos que son el descarte de otras enfermedades con manifestaciones clínicas parecidas que pueden llevar a un diagnóstico erróneo y por otra parte la profundización de los tratamientos a utilizar. Se sabe que la genética, las condiciones climáticas, la raza, la edad y el sexo influyen en la aparición, evolución y pronóstico de la enfermedad, se hace necesario realizar estudios locales de la incidencia y tratamientos efectivos para concretar guías clínicas funcionales para el manejo, diagnostico y tratamiento de las lesiones cutáneas en LES.
La presente obra es el resultado del esfuerzo académico de un grupo de profesionales de la salud, quienes han recopilado la información disponible con la evidencia científica más actualizada sobre temas que esperamos ayuden a los profesionales de la salud y a médicos en formación interesados en profundizar sus conocimientos sobre patologías frecuentes que pueden presentarse dentro de su ámbito profesional.
El megacolon tóxico es una grave complicación de las enfermedades inflamatorias intestinales que altera el funcionamiento del intestino grueso. Aunque los raros reportes del origen de esta condición deriven del mal control de patologías como la de Crohn; o por infecciones del colon, por lo general, su causa radica en una colitis inflamatoria, isquémica o infecciosa. El trastorno consiste en el ensanchamiento del intestino grueso originado en pocos días que, aun siendo infrecuente, no deja de ser potencialmente fatal. El objetivo del presente estudio se centra en la aportación de un material bibliográfico actualizado que sirva no solo para el fortalecimiento del conocimiento científico académico en relación al megacolon tóxico, su diagnóstico y tratamiento, sino también, como fundamento de futuras investigaciones. En razón de ello, se ha decidido adelantar un estudio de diseño bibliográfico con el cual recopilar varios criterios de tratadistas y expertos, mediante una metodología de revisión. La evidencia plasmada en los resultados incluye definiciones de megacolon tóxico, mención de sus principales síntomas y causas, y algunos los criterios diagnósticos y terapéuticos. Se concluye que el fundamento que determina el MT esta apoyado en tres aspectos: la historia clínica, los datos de respuesta inflamatoria sistémica y la evidencia imagenológica, principalmente radiográfica, de la dilatación colónica. En cuanto al tratamiento, las principales recomendaciones se centran en el bordaje multidisciplinar y el ingreso a la unidad de cuidados intensivos; mientras que el protocolo terapéutico se enfocará fundamentalmente en el tratamiento de la causa subyacente, la aplicación de medidas generales y, por último, en la cirugía.
Desde junio de 2020, se han publicado varios informes de casos y series que informan de un síndrome inflamatorio multisistémico en adultos, como respuesta inflamatoria severa retardada a la infección por SARS-CoV-2 y que lleva a daño en múltiples órganos, como el corazón, los vasos sanguíneos, los riñones, el sistema digestivo, el cerebro, la piel o los ojos. Entre los síntomas compatibles con esta patología, destacan: fiebre que se prolonga por 24 horas o más y latidos cardíacos irregulares; evidencia de disfunción cardíaca; síntomas gastrointestinales y erupciones cutáneas. El tratamiento para esta patología depende del tipo y de la gravedad de los síntomas y de qué órganos y otras partes del cuerpo se ven afectados por la inflamación, y generalmente se basa en inmunomoduladores, tales como corticosteroides y/o plasmaféresis, dado que se han reportado resistencias a la inmunoglobulina intravenosa. El objetivo de este trabajo investigativo, desarrollado bajo una metodología documental, se centra en realizar una revisión a la literatura científica disponible, que principalmente aborde al Síndrome inflamatorio multisistémico en el adulto, cómo detectar esta patología, cuales son los síntomas y los diversos tratamientos a aplicar, así como recomendar medidas de prevención, todo ello con la finalidad de proporcionar un material actualizado que defina dichos aspectos. Para ello se escogieron más de 12 fuentes digitales, ubicadas en portales tales como: Centro para el Control y la Prevención de Enfermedades, Clínica Mayo, intramed, entre otras, que fundamentan las ideas aquí planteadas, y que consecuentemente sirven de base para concluir que se trata de una enfermedad compleja cuyo abordaje en los adultos aún está en investigación, por tratarse de una patología que inicialmente había sido presentada solo en pacientes pediátricos, y que teniendo en cuenta que está relacionado con la enfermedad COVID-19, es muy importante cumplir con las medidas de prevención recomendadas para la infección por SARS-CoV-2.
