El lupus eritematoso (LES) es una enfermedad autoinmune crónica, donde el sistema inmunológico ataca a sus propias células produciendo infamación y daño, debido a la unión de nuestros anticuerpos a las células del organismo y al depósito de complejos antígeno-anticuerpo. Las lesiones no específicas pueden estar asociadas a otras enfermedades autoinmunes e incluyen cambios vasculares. A pesar de su complejidad; los mecanismos patógenos comienzan a ser comprendidos involucrando a factores genéticos, hormonales y ambientales que conducen a una alteración del sistema inmunitario, con una respuesta de autoreactividad frente a las estructuras propias del organismo. Uno de los factores que se ha planteado como precursor de autoinmunidad es el estrés y factores inherentes a la personalidad de los pacientes. Entendiendo que la piel ha de ser uno de los órganos principales órganos afectados se plantea la investigación con el fin de encontrar las lesiones cutáneas de dicha enfermedad y realizar una revisión bibliográfica para analizar y sintetizar las características de esta enfermedad y las actualizaciones en las publicaciones académicas de la misma. Los avances en las publicaciones científicas llevan no solo a conocer mejor el desarrollo y tratamiento de la enfermedad, sino que por supuesto se traduce en una mejora en las expectativas de vida de los pacientes diagnosticados. La evolución en el estudio del LES es considerable, en ninguno de los trabajos analizados se descartan los hallazgos en el tema puesto que la enfermedad tiene distintas formas de manifestación y desarrollo. La mayoría de los estudios se centran en dos temas específicos que son el descarte de otras enfermedades con manifestaciones clínicas parecidas que pueden llevar a un diagnóstico erróneo y por otra parte la profundización de los tratamientos a utilizar. Se sabe que la genética, las condiciones climáticas, la raza, la edad y el sexo influyen en la aparición, evolución y pronóstico de la enfermedad, se hace necesario realizar estudios locales de la incidencia y tratamientos efectivos para concretar guías clínicas funcionales para el manejo, diagnostico y tratamiento de las lesiones cutáneas en LES.
En la actualidad, las alteraciones visuales constituyen un problema de salud importante a nivel mundial, con relevante repercusión para las personas, las familias y la sociedad. Uno de estos problemas incluye los daños sufridos por el nervio óptico, entre ellos, la Neuritis Óptica (NO). El término Neuritis Óptica se refiere a un proceso inflamatorio, generalmente idiopático que condiciona la desmielinización del nervio óptico. El deterioro de la visión en niños tiene un gran impacto personal. En consecuencia, la presente investigación tiene el propósito de compendiar algunas generalidades acerca de la Neuritis Óptica (NO) en pacientes pediátricos. La investigación se realizó bajo una metodología de tipo documental bibliográfica, bajo la modalidad de revisión. La Neuritis Óptica es rara en niños, no obstante, es una patología importante con grandes repercusiones en la vida de los pacientes y asociada a varias patologías, también importantes. Las más frecuentes en pacientes pediátricos es la bilateral y anterior (papilitis). Asimismo, estos pacientes presentan mejor pronóstico que los adultos. El diagnóstico es clínico, debe incluir pruebas de imágenes, analítica de sangre y líquido cefalorraquídeo. El tratamiento extrapolado de adultos consiste en el tratamiento con corticoesteroides intravenosos (metilprednisolona) o esteroides orales (prednisona). Otros tratamientos usados en enfermedades neuroinmunológicas son las inmunoglobulinas intravenosas (IgIV) y la plasmaféresis. Es importante el control de estos pacientes unos tres meses después del tratamiento. No se han establecido protocolos específicos referentes a un manejo óptimo de esta patología en la población pediátrica. La protocolización de la actividad frente a esta patología permite minimizar la variabilidad de las actuaciones de los profesionales de salud y facilita la toma de decisiones en cuanto al diagnóstico, seguimiento y tratamiento de la Neuropatía Óptica en pacientes pediátricos.
La amiotrofia monomiélica es un trastorno neurológico poco común que produce atrofia de una extremidad. La afección de la extremidad superior es más frecuente que la extremidad inferior, esta entidad clínica predomina en el sexo masculino. Se presenta caso clínico de Varón, 52 años de edad, referido para evaluación por debilidad y atrofia de mano derecha. Su historia comenzó cerca de un año antes, con paresia inducida por el frio, parestesias del ante brazo seguido de debilidad en su mano que progresó hacia su antebrazo y brazo derecho. El diagnostico luego de la exploración física más los exámenes de imágenes, confirman que hay un signo de afectación de la neurona motora inferior (monomélica), y como posibilidad lejana una afectación C7-C8-T1. No se encuentran signos de afectación al plexo. En base a la literatura consultada, se desprende que la Amiotrofia Monomiélica es una enfermedad de progreso lento, que afecta a hombres después de los 30 años, aseveración que va acorde con la edad del paciente, expuesto en el caso clínico presentado en esta investigación, así como los síntomas presentados en cuanto a la disminución de fuerza en las extremidades superiores, que le puede dar un carácter de degenerativa. Tiene muy pocas opciones de tratamiento, ya que no es una enfermedad que genere dolor, sin embargo, dentro de la literatura se indica como opción terapéutica la rehabilitación física de leve a moderada, existiendo todavía muchas ambigüedad y discusión en cuanto a su efectividad en la fuerza muscular.
Se presenta el caso de una paciente femenina 44 años de edad, con diagnóstico de rotura aneurismática de carótida derecha en relación con arteria comunicante posterior asociado a hipoplasia de arteria cerebral posterior ipsilateral. A inicios del mes de febrero del año en curso mientras dormía, en forma súbita presentó cefalea holocraeana acompañado de náuseas y vómito por varias ocasiones sin pérdida de la conciencia. En horas de la mañana ante la persistencia de su cefalea, que no había cedido al tratamiento analgésico, acudió a consulta particular; el estudio tomográfico cerebral demostró hemorragia subaracnoidea en las cisternas selar, Silviana derecha e interhemisférica anterior. En las siguientes tres horas se sometió a angiotac que demostró la presencia de aneurisma de la arteria carótida derecha en relación con la arteria comunicante posterior, el flujo sanguíneo de la arteria comunicante posterior proveniente de la carótida atravesaba el aneurisma; siendo el motivo de no ser tributario de cirugía temprana. En el trascurso la paciente se mantuvo orientada, sin déficit motor ni sensitivo y estable hemodinámicamente, decidiéndose tratamiento ulterior en 15 días con diversor de flujo a nivel de la arteria carótida, iniciando anti agregación con clopidogrel y ácido acetil salicílico (ASA). En condiciones estables egreso a su domicilio por el tiempo descrito para tratamiento definitivo. Trascurrido los 15 días, bajo anestesia general se procedió a la colocación del dispositivo Pipeline flex (PED) a nivel de arteria carótida derecha vía endovascular; no se presentó ninguna complicación y se concedió alta domiciliaria con control en seis meses. Al momento evolución satisfactoria sin ningún déficit.
La Insuficiencia Renal Aguda (IRA) es una patología que ha cobrado mayor importancia en la actualidad por sus crecientes tasas de morbimortalidad en todo el mundo. La IRA es un síndrome que se presenta por múltiples causas que provocan una injuria y se caracteriza por la disminución abrupta de la filtración glomerular, la cual resulta por la incapacidad del riñón para excretar los productos nitrogenados y para mantener la homeostasis de líquidos y electrolitos. Las técnicas de diagnóstico por imagen vienen experimentando en los últimos años un desarrollo lento pero sostenido que hace que se haya mejorado significativamente su capacidad para detectar patología a la vez que se intenta disminuir sus efectos adversos. El objetivo general de la presente revisión es compendiar los aspectos básicos del uso de la ecografía en la Insuficiencia Renal Aguda, con énfasis en su utilidad. La investigación se realizó bajo una metodología de tipo documental bibliográfica, bajo la modalidad de revisión. De los estudios revisados se desprende que la ecografía ha demostrado ser una herramienta útil en la evaluación inicial de la Insuficiencia Renal Aguda (IRA), con una gran cantidad de bondades entre las que destacan la carencia de toxicidad, la visualización de las estructuras anatómicas en tiempo real, un bajo costo, además de la disponibilidad de poder llevar los equipos hasta el lugar donde se encuentre el paciente, dada sus condiciones. Asimismo, es de fácil manejo con respecto a otros equipos para estudios de imagen y ofrece una gran cantidad de información capaz de orientar el diagnóstico, guiar terapéuticas y seguir la evolución de la patología. En virtud de todas estas ventajas, es la prueba más usada y útil para la detección y orientación rápida y eficaz de esta patología renal.
