El trauma es la principal causa de muerte en personas menores de 45 años en y responsable de un tercio de todos los ingresos a la unidad de cuidados intensivos. El conocimiento cada vez mayor sobre los métodos de diagnóstico por ultrasonido y su disponibilidad para lesiones potencialmente mortales como el taponamiento cardíaco y la rotura de órganos sólidos de la cavidad abdominal, que conduce a un shock hemorrágico, el diagnóstico y el monitoreo, conducen al desarrollo de FAST (Evaluación enfocada con ecografía para traumatismos) protocolo, destinado a ser utilizado tanto en el ámbito de la unidad de cuidados intensivos como de emergencia. Debido a su reproducibilidad, falta de exposición a la radiación y viabilidad al lado de la cama, esta tecnología se acepta cada vez más. Una nueva extensión del protocolo, el Extended-FAST (eFAST), proporciona información valiosa para mejorar el manejo de los pacientes, extendiendo su disponibilidad desde las afecciones abdominales a otros diagnósticos como hemotórax, derrame pleural y neumotórax. Se debe subrayar que esta técnica es capaz de reemplazar la tomografía computarizada y el diagnóstico de lavado peritoneal, y no retrasar el procedimiento quirúrgico en lugar de realizar este examen. Por lo tanto, su valoración cuidadosa en relación con la información clínica debe orientar los enfoques terapéuticos, especialmente en lugares inhóspitos como unidades de cuidados intensivos en zonas de guerra, lugares rurales o distantes, donde otros métodos de imagenología no están disponibles.
Los trastornos de la piel, generalmente desatendidos y frecuentemente infradiagnosticados entre los pacientes diabéticos, son complicaciones comunes y encuentran un amplio espectro de trastornos tanto en la diabetes mellitus (DM) tipo 1 como en el tipo 2, por ejemplo, infección cutánea, piel seca, prurito. Los trastornos de la piel están altamente asociados con un mayor riesgo de resultados importantes, como lesiones cutáneas, ulceraciones y pie diabético, que pueden provocar complicaciones importantes y girar en torno a factores multifactoriales además de la hiperglucemia y los productos finales de glicación avanzada. Aunque los trastornos cutáneos de los diabéticos son consistentes, existen datos limitados sobre los trastornos cutáneos en etapa temprana en pacientes con DM. El control de enfermedades, el tratamiento en etapa temprana (por ejemplo, hidratación de la piel, dispositivos ortopédicos) y la conciencia pueden reducir la morbilidad de los pacientes con DM. Así, una mejor comprensión de la carga de los trastornos cutáneos en los pacientes con DM aumenta la conciencia sobre la prevención y el tratamiento. Por tanto, el objetivo de este estudio es realizar una revisión de la literatura para evaluar las principales características clínicas y complicaciones de los trastornos cutáneos en pacientes diabéticos. Además, también se revisaron los trastornos cutáneos en estadio temprano fisiopatológico y el manejo dermocosmético.
Desde junio de 2020, se han publicado varios informes de casos y series que informan de un síndrome inflamatorio multisistémico en adultos, como respuesta inflamatoria severa retardada a la infección por SARS-CoV-2 y que lleva a daño en múltiples órganos, como el corazón, los vasos sanguíneos, los riñones, el sistema digestivo, el cerebro, la piel o los ojos. Entre los síntomas compatibles con esta patología, destacan: fiebre que se prolonga por 24 horas o más y latidos cardíacos irregulares; evidencia de disfunción cardíaca; síntomas gastrointestinales y erupciones cutáneas. El tratamiento para esta patología depende del tipo y de la gravedad de los síntomas y de qué órganos y otras partes del cuerpo se ven afectados por la inflamación, y generalmente se basa en inmunomoduladores, tales como corticosteroides y/o plasmaféresis, dado que se han reportado resistencias a la inmunoglobulina intravenosa. El objetivo de este trabajo investigativo, desarrollado bajo una metodología documental, se centra en realizar una revisión a la literatura científica disponible, que principalmente aborde al Síndrome inflamatorio multisistémico en el adulto, cómo detectar esta patología, cuales son los síntomas y los diversos tratamientos a aplicar, así como recomendar medidas de prevención, todo ello con la finalidad de proporcionar un material actualizado que defina dichos aspectos. Para ello se escogieron más de 12 fuentes digitales, ubicadas en portales tales como: Centro para el Control y la Prevención de Enfermedades, Clínica Mayo, intramed, entre otras, que fundamentan las ideas aquí planteadas, y que consecuentemente sirven de base para concluir que se trata de una enfermedad compleja cuyo abordaje en los adultos aún está en investigación, por tratarse de una patología que inicialmente había sido presentada solo en pacientes pediátricos, y que teniendo en cuenta que está relacionado con la enfermedad COVID-19, es muy importante cumplir con las medidas de prevención recomendadas para la infección por SARS-CoV-2.
