La estenosis subglótica (ES) plantea un desafío clínico constante dado su alta morbimortalidad, diversidad de causas, localización, severidad y variedad de procedimientos terapéuticos. Objetivos: Determinar el perfil de pacientes con diagnóstico de ES, evaluar el tratamiento y sus resultados.
Propranolol for the treatment of subglotic hemangioma-A new therapeutic alternativeInfantile Subglottic Hemangiomas (ISH) are rare and potentially fatal. Many treatments have been described for this condition. Propranolol is a new therapeutic alternative, with successful results in skin and airway infantile hemangiomas. Case study: The case of an infant with ISH and 90% blockage of his airway is presented. The patient underwent a tracheostomy procedure and then treated with oral propranolol, showing a good response after a month of treatment, presenting 10% obstruction and consequently, ending the tracheostomy early. Conclusion: The discovery of the effect of propranolol in the ISH has revolutionized the management of this condition, displacing steroids and surgery as first-line treatments. This study discusses the current ISH management. RESUMENLos hemangiomas infantiles subglóticos (HIS) son poco frecuentes y potencialmente mortales. Muchos tratamientos se han descrito para los HIS. El propranolol es una nueva alternativa terapéutica, con resultados exitosos en hemangiomas infantiles de piel y de vía áerea. Caso clínico: Se reporta el manejo de un lactante con HIS que presentaba una obstrucción del 90% de su vía aérea, que debió ser traqueostomizado, luego se inicio propranolol oral, presentando una muy buena respuesta al mes de tratamiento, con 10% de obstrucción, retirando traqueostomía precozmente. Conclusión: El descubrimiento del efecto del propranolol en los HIS ha revolucionado su manejo, desplazando a los corticoides y a la cirugía como primera línea de tratamiento. En el presente reporte se discute el manejo actual de los HIS.
RESUMENIntroducción: El angiomiolipoma es un tumor renal benigno constituido fundamentalmente por vasos sanguíneos, músculo liso y tejido adiposo. El tratamiento depende del tamaño, sintomatología y condición general del paciente. El manejo quirúrgico se debe enfocar en la preservación de la mayor cantidad de parénquima renal posible, siendo la nefrectomía parcial laparoscópica una alternativa a considerar. Reporte del caso: Paciente de 31 años de edad, sexo femenino, consulta en agosto del 2002 por dolor lumbar inespecífico y sintomatología de infección urinaria. La ecografía renal mostró una tumoración sólida de 4 cm en el polo superior del riñón derecho. La tomografía axial computada confirmó el diagnostico imagenológico de angiomiolipoma renal, por lo que se decide mantener una conducta expectante. En abril del 2006 se le diagnostica un embarazo molar que requiere manejo con quimioterapia. En el estudio de diseminación se demostró un aumento de tamaño del angiomiolipoma renal de 4 cm a 9 cm de diámetro, por lo que se planifica una nefrectomía parcial laparoscópica. En enero del 2006 se realiza la nefrectomía parcial laparoscópica sin incidentes. El tiempo de isquemia fue de 47 minutos con un sangrado aproximado de 250 cc y con un tiempo operatorio de 150 minutos. No se registraron complicaciones. La biopsia de la pieza operatoria informó una lesión compatible con un angiomiolipoma. Discusión: El angiomiolipoma es un tumor renal benigno más frecuente en mujeres en edad media. Se puede presentar con dolor en flanco, masa palpable y hematuria, sin embargo, sobre un 50% se pesquisan de manera incidental. La tomografía axial computada es el examen que entrega mayor información respecto a la naturaleza de la lesión. El tratamiento depende del tamaño tumoral, velocidad de crecimiento, condiciones del paciente y de la sintomatología. En relación al manejo quirúrgico, la evolución de la laparoscopia ha permitido que la nefrectomía parcial laparoscópica sea una alternativa en los tumores de entre 4 a 7 cm y en localizaciones complejas, con resultados oncológicos comparables a la cirugía radical.
Introducción: Los pacientes con tinnitus y audición normal son poco frecuentes. No hay referencias en la literatura sobre los hallazgos del examen vestibular en este tipo de enfermos. Objetivo: Analizar los hallazgos de la exploración clínica cocleo-vestibular en un grupo de pacientes con tinnitus y audición (audiometría tonal clásica) normal. Material y método: Estudio retrospectivo de un grupo de 17 pacientes con tinnitus y audición normal examinados en un período de 10 años; sus resultados se compararon con los de un grupo control de 17 sujetos sin tinnitus y con audición normal. Resultados: En el grupo de pacientes la audiometría tonal clásica era normal y en el examen vestibular la mayoría de ellos (15) mostró una paresia vestibular unilateral en la prueba calórica; en sólo dos ésta era normal. En marcado contraste con el grupo de pacientes, en la mayoría de los sujetos normales (15) la prueba calórica era normal y en 2 se encontró una paresia vestibular unilateral. Conclusión: Se formula la hipótesis que, en nuestro grupo de pacientes, el tinnitus sería el único síntoma de una lesión "subclínica" cocleo-vestibular, la cual en la exploración cocleo-vestibular se manifestaría sólo por una paresia vestibular.
La protuberancia coriónica (del inglés chorionic bump) es una condición que involucra al saco gestacional y que puede ser visualizada en la ecografía de primer trimestre. Ha sido descrita como una convexidad irregular que protruye hacia el saco gestacional y que probablemente corresponda a la formación de un hematoma en la superficie coriodecidual. Nosotros reportamos los hallazgos y el seguimiento ecográfico de un caso, junto con una revisión de la literatura.
