Propranolol for the treatment of subglotic hemangioma-A new therapeutic alternativeInfantile Subglottic Hemangiomas (ISH) are rare and potentially fatal. Many treatments have been described for this condition. Propranolol is a new therapeutic alternative, with successful results in skin and airway infantile hemangiomas. Case study: The case of an infant with ISH and 90% blockage of his airway is presented. The patient underwent a tracheostomy procedure and then treated with oral propranolol, showing a good response after a month of treatment, presenting 10% obstruction and consequently, ending the tracheostomy early. Conclusion: The discovery of the effect of propranolol in the ISH has revolutionized the management of this condition, displacing steroids and surgery as first-line treatments. This study discusses the current ISH management. RESUMENLos hemangiomas infantiles subglóticos (HIS) son poco frecuentes y potencialmente mortales. Muchos tratamientos se han descrito para los HIS. El propranolol es una nueva alternativa terapéutica, con resultados exitosos en hemangiomas infantiles de piel y de vía áerea. Caso clínico: Se reporta el manejo de un lactante con HIS que presentaba una obstrucción del 90% de su vía aérea, que debió ser traqueostomizado, luego se inicio propranolol oral, presentando una muy buena respuesta al mes de tratamiento, con 10% de obstrucción, retirando traqueostomía precozmente. Conclusión: El descubrimiento del efecto del propranolol en los HIS ha revolucionado su manejo, desplazando a los corticoides y a la cirugía como primera línea de tratamiento. En el presente reporte se discute el manejo actual de los HIS.
INTRODUCCIÓNEl Fibroangioma Nasofaríngeo Juvenil (FNJ) es una neoplasia vascular benigna, infrecuente, que se representa en el 0,05% a 0,5% de todos los tumores de cabeza y cuello 1 . Aunque histológicamente es benigno, en ocasiones su compleja localización anatómica, su tendencia a la invasión local, la recurrencia y su crecimiento agresivo, hacen que sea un tumor nasosinusal con características peculiares. Ocurre casi exclusivamente en la nasofaringe de adolescentes de sexo masculino 2,3 . Los FNJ tienen un crecimiento lento, centrado en el foramen esfenopalatino y se expanden dentro del la cavidad nasal, nasofaringe y luego en la fosa pterigomaxilar 4 . Eventualmente pueden erosionar el hueso e invadir la fosa infratemporal, órbita y fosa craneal media. La irrigación principal proviene de la arteria maxilar interna ipsilateral, pero además puede estar dada por la faríngea ascendente, maxilar interna contralateral o carótida interna. Los FNJ clásicamente se presentan con obstrucción nasal unilateral, epistaxis recurrente y tumoración nasal 5,6 . Existen otras manifestaciones clínicas según la extensión tumoral, tales como hipoacusia de conducción, proptosis ocular y hemorragia masiva 7 . En la evaluación inicial puede usarse la nasofibroscopía, tomografía computarizada y la resonancia nuclear magnética para determinar la extensión tumoral 8 . La tomografía computarizada es excelente para evaluar el compromiso óseo y extensión tumoral. El abombamiento de la pared posterior del maxilar debido a la presencia de la masa en el espacio pterigomaxilar, conocido como signo de Holman-Miller, es un hallazgo típico de este examen. La resonancia nuclear magnética permite evaluar los tejidos blandos y diferenciar el tumor de Inflamación de mucosa, lo cual es y esta es fundamental cuando existe compromiso intracraneano (Figura 1). Existe una variedad de clasificaciones de estos tumores 9-13 . La clasificación de Radkowski, desarrollada en 1996 es la más utilizada en la literatura en los últimos años (Tabla 1). Existen además otras clasificaciones como la de Fisch, Andrews y Sessions. Todas ellas se basan en la extensión e invasión de estructuras por el tumor, excepto la clasificación de la Universidad de Pittsburgh 14 que además incluye la vascularización residual posembolización. El papiloma invertido es un tumor benigno inusual del tracto nasosinusal, caracterizado por el riesgo de recurrencia y transformación maligna. Objetivo: Nuestro objetivo es describir la serie de casos diagnosticados con papiloma invertido del tracto nasosinusal entre 2002 y 2010. Material y método: Se trata de un análisis retrospectivo de los casos nuevos de papiloma invertido del tracto nasosinusal atendidos en nuestro Hospital entre los años 2002 y 2010. Resultados: Se presentaron 25 casos nuevos de papiloma invertido. Los síntomas tenían en promedio 2 años de evolución, siendo la obstrucción nasal y rinorrea los más frecuentes. La etapificación preoperatoria fue realizada mediante tomografía computarizada en 15 pacientes. El abordaje endoscópico fue el preferido. Cincuenta y seis por ciento de los pacientes correspondía a la etapa 2 de la clasificación de Krause. El promedio de seguimiento fue 29 meses. Durante ese periodo, 4 casos presentaron recurrencia, correspondiendo al 16% de nuestra serie. La mitad de ellos presentó displasia o atipia al momento de la recurrencia. Ocurrió 1 caso de transformación maligna, el cual se presentó como un carcinoma indiferenciado. Conclusión: Las características generales son similares a los reportes internacionales. En nuestro medio los pacientes son diagnosticados en etapas avanzadas de la enfermedad, no obstante, el abordaje endoscópico es el método quirúrgico preferido.
Introducción: Dentro del estudio diagnóstico de pacientes con masa cervical la punción por aspiración con aguja fina (PAAF) es una herramienta útil debido a su facilidad y seguridad.Objetivo: Evaluar el rendimiento de las PAAF realizadas en nuestro servicio en los últimos diez años.Material y método: Estudio retrospectivo y descriptivo. Se obtuvieron los datos de pacientes sometidos a PAAF en el Servicio de Otorrinolaringología del Hospital Barros Luco Trudeau entre los años 2001 a 2010 y se correlacionaron con las biopsias definitivas atingentes.Resultados: Se realizaron 197 PAAF, de las cuales 140 (71%) fueron aptas para estudio citológico y 77 tenían biopsias concomitantes. Se obtuvo una sensibilidad de 46%, especificidad de 85%, valor predictivo positivo de 74% y valor predictivo negativo de 62%. La localización para la cual se obtuvo un mejor rendimiento de la técnica fue para glándulas salivales y los diagnósticos con mejor rendimiento fueron carcinoma escamoso y tumor mixto.Discusión: La PAAF es una herramienta subutilizada. Obtuvimos una alta tasa de muestras insuficientes para estudio diagnóstico y, en nuestro medio, es útil para descartar neoplasia maligna en masas de glándulas salivales y metástasis de carcinoma escamoso.Palabras clave: Masa cervical, punción por aspiración con aguja fina, citología.
